Current advice regarding implant choice is based on estimates of cost-benefit derived from implant survival to an endpoint of revision. Current estimates do not account for many implant failures which are treated with non-revision surgery and may not be accurate. The aim of this study was to estimate survival of major stem implant design groups to an endpoint of reoperation.

Primary total hip replacement and linked revision form the National Joint Registry (NJR) and Hospital Episode Statistics (HES) data linked by unique identifier were used. Survival of femoral implant groups (cemented stainless steel polished taper [PTSS], cemented cobalt chrome polished taper [PTCC], cemented composite beam [CB], collarless cementless [NCOL] and collared cementless [COL]) was estimated using Kaplan-Meier method.

809,832 patients with valid NJR and HES data from England, were included. Cumulative failure at ten years for PTSS increased overall from 2.9% (95%CI 2.8–2.9) to 3.6% (95%CI 3.6–3.7) after inclusion of reoperations. Cumulative failure at ten years for PTSS increased from 2.5% (95%CI 2.5–2.6) to 3.3% (95%CI 3.2–3.4), for PTCC increased from 3.8% (95%CI 3.5–4.0) to 5.4% (95%CI 5.1–5.6), for CB increased from 3.1% (95%CI 2.9–3.3) to 4.1% (95%CI 3.8–4.3), for NCOL increased from 3.4% (95%CI 3.3–3.5) to 3.9% (95%CI 3.8–4.0), and for COL increased from 2.5% (95%CI 2.4–2.6) to 3.1% (95%CI 2.9–3.2), after inclusion of reoperations.

Re-operation for internal fixation is as significant life event for the patient as revision. When a more inclusive metric is used, the patient and clinician's perspective on what constitutes a GIRFT implant may not be the same. Further work is required to update implant selection guidance in view of the change in implant performance.

Current estimates of periprosthetic fracture risk associated with femoral implants is mostly limited to revision only estimates and does not accurately represent stem performance. The aim of this study was to estimate the risk of surgically treated post-operative periprosthetic femoral fracture (POPFF) more accurately associated with frequently used femoral implants used for total hip arthroplasty (THA).

A cohort study of patients who underwent primary THA in England between January 1, 2004, and December 31, 2020. Periprosthetic fractures were identified from prospectively collected revision records and national procedure coding records. Survival modelling was used to estimate POPFF incidence rates, adjusting for potential confounders. Subgroup analyses were performed for patients over 70 years, with non-osteoarthritic indications, and neck of femur fracture.

POPFF occurred in 0.6% (5100/809,832) of cases during a median (IQR) follow up of 6.5 (3.9 to 9.6) years. The majority of POPFF were treated with fixation after implantation of a cemented stem. Adjusted patient time incidence rates (PTIR) for POPFF varied by stem design, regardless of cement fixation. Cemented composite beam stems (CB stems) demonstrated the lowest risk of POPFF. Collared cementless stems had an equivalent or lower rate of POPFF versus the current gold standard polished taper slip cemented stem.

POPFF account for a quarter of all revisions following primary THA. Cemented CB stems are associated with the lowest POPFF risk. Stem design is strongly associated with POPFF risk, regardless of the presence of cement. Surgeons, policymakers, and patients should consider these findings when recommending femoral implants in those most at risk of POPFF.

Aims

Prophylactic antibiotic regimens for elective primary total hip and knee arthroplasty vary widely across hospitals and trusts in the UK. This study aimed to identify antibiotic prophylaxis regimens currently in use for elective primary arthroplasty across the UK, establish variations in antibiotic prophylaxis regimens and their impact on the risk of periprosthetic joint infection (PJI) in the first-year post-index procedure, and evaluate adherence to current international consensus guidance.

Methods

The guidelines for the primary and alternative recommended prophylactic antibiotic regimens in clean orthopaedic surgery (primary arthroplasty) for 109 hospitals and trusts across the UK were sought by searching each trust and hospital’s website (intranet webpages), and by using the MicroGuide app. The mean cost of each antibiotic regimen was calculated using price data from the British National Formulary (BNF). Regimens were then compared to the 2018 Philadelphia Consensus Guidance, to evaluate adherence to international guidance.

Abstract

Background

Daycase pathways which aim to discharge patients the same day following Unicompartmental Knee Replacement have been introduced in some centres, though most continue with Standard pathways. While Daycase pathways have cost savings, recovery data comparing pathways is limited. This study aims to compare patient-reported early recovery between Daycase and Standard pathways following UKR.

Method

This study was carried out in two centres that originally used the same Standard recovery pathway for UKR. In one centre, the Standard pathway was modified into a Daycase pathway. 26 Daycase-Outpatient, 11 Daycase-Inpatient, and 18 Standard patients were recruited. Patients completed the Oxford Arthroplasty Early Recovery Score (OARS) and SF-36 (Acute) measure between Days 1–42.

Management of ankle arthritis in young patients is challenging. Although ankle arthrodesis gives consistent pain relief, it leads to loss of function and adjacent joint arthritis. Ankle joint distraction (AJD) has been shown to give good outcomes in adults with osteoarthritis or post-traumatic arthritis. The efficacy in children or young adults and those with juvenile idiopathic arthritis is less well evidenced.

Clinical notes and radiographs of all patients (n=6) managed with AJD in one tertiary referral centre were retrospectively reviewed. Radiographs were taken pre-surgery, intra-operatively, 1 month following frame removal and at the last follow up, tibiotalar joint space was assessed using ImageJ software. Measurements were taken at the medial, middle and lateral talar dome using frame components as reference. Radiographic data for patients with a good clinical outcome was compared with those who did not.

At time of surgery mean age was 16.1 years (12 – 25 years). Mean follow up was 3.4 years (1.5 – 5.9 years). Indications were juvenile idiopathic arthritis (4) post-traumatic (1) and post-infective arthritis (1). Three patients at last follow up had a good clinical outcome. Two patients required revision to arthrodesis (1.3 and 2.4 years following distraction). One patient had spontaneous fusion. One patient required oral antibiotics for pin site infection.

Inter-observer reliability was 95%. Mean joint space was 1.17mm (SD = 0.87mm) pre-operatively which increased to 6.72mm (SD = 2.23mm) at the time of distraction and 2.09mm (SD = 1.14mm) at the time of removal. At one-year follow up, mean joint space was 1.96mm (SD = 1.97mm).

Outcomes following AJD in this population are variable although significant benefits were demonstrated for 50% of the patients in this series. Radiographic joint space preoperatively did not appear to be associated with need for arthrodesis. Further research in larger groups of young patients is required.

Multiple biochemical biomarkers have been previously investigated for the diagnosis, prognosis and response to treatment of articular cartilage damage, including osteoarthritis (OA). Synovial fluid (SF) biomarker measurement is a potential method to predict treatment response and effectiveness. However, the significance of different biomarkers and their correlation to clinical outcomes remains unclear. This systematic review evaluated current SF biomarkers used in investigation of cartilage degeneration or regeneration in the knee joint and correlated these biomarkers with clinical outcomes following cartilage repair or regeneration interventions.

PubMed, Institute of Science Index, Scopus, Cochrane Central Register of Controlled Trials, and Embase databases were searched. Studies evaluating SF biomarkers and clinical outcomes following cartilage repair intervention were included. Two researchers independently performed data extraction and QUADAS-2 analysis. Biomarker inclusion, change following intervention and correlation with clinical outcome was compared.

9 studies were included. Study heterogeneity precluded meta-analysis. There was significant variation in sampling and analysis. 33 biomarkers were evaluated in addition to microRNA and catabolic/anabolic ratios. Five studies reported on correlation of biomarkers with six biomarkers significantly correlated with clinical outcomes following intervention. However, correlation was only demonstrated in isolated studies.

This review demonstrates significant difficulties in drawing conclusions regarding the importance of SF biomarkers based on the available literature. Improved standardisation for collection and analysis of SF samples is required. Future publications should also focus on clinical outcome scores and seek to correlate biomarkers with progression to further understand the significance of identified markers in a clinical context.

Background

Involving research users in setting priorities for research is essential to ensure research outcomes are patient-centred and to maximise research value and impact. The Musculoskeletal (MSK) Disorders Research Advisory Group Versus Arthritis led a research priority setting exercise across MSK disorders.

Methods

The Child Health and Nutrition Research Initiative (CHRNI) method of setting research priorities with a range of stakeholders were utilised. The MSKD RAG identified, through consensus, four research Domains: Mechanisms of Disease; Diagnosis and Impact; Living Well with MSK disorders and Successful Translation. Following ethical approval, the research priority exercise involved four stages and two surveys, to: 1) gather research uncertainties; 2) consolidate these; 3) score uncertainties using agreed criteria of importance and impact on a score of 1–10; and 4) analyse scoring, for prioritisation.

Abstract

Introduction

This multi-centre randomised controlled trial evaluated the clinical and cost effectiveness of liposomal bupivacaine for pain and recovery following knee replacement.

Methodology

533patients undergoing primary knee replacement were randomised to receive either liposomal bupivacaine (266mg) plus bupivacaine hydrochloride (100mg) or control (bupivacaine hydrochloride 100mg), administered at the surgical site. The co-primary outcomes were pain visual analogue score (VAS) area under the curve (AUC) 6 to 72hours and the Quality of Recovery 40 (QoR-40) score at 72hours.

Abstract

Introduction

This study aimed to assess the effect of PRP on knee articular cartilage content (thickness and/or volume) and establish if there is a correlation between changes in cartilage and clinical outcomes in patients with knee osteoarthritis.

Methodology

A systematic review was performed following the Cochrane methodology. Studies were included if they reported on cartilage content with MRI or Ultrasound before and after the injection. A random-effects model meta-analysis was performed.

Periprosthetic femoral fracture (PFF) incidence following hip replacement surgery continues to rise. There is a national drive to centralise PFF treatment within specialist centres to improve clinical outcomes and cost-effectiveness. The financial implications of treating PFFs must be analysed to guide allocation of funding.

Data were collected for 129 PFFs admitted from 02/04/2014–19/05/2020. Financial data were provided by the Patient Level Information and Costing Systems (PLICS) team. Primary outcomes were cost, revenue and margin for each PFF. Additional data were collected on length of stay (LOS), critical care requirements and clinical outcomes. Statistical comparisons were made between treatment type (fixation vs revision). Significance was set to p<0.05.

Across the entire cohort, total cost was £2,389,901, total revenue was £1,695,435 and total loss was £694,481. Highest costs were ward stay (£714,591), theatre utilisation (£382,625), and overheads (£249,110). Median cost was £15,863 (IQR, £11,092-£22,221), median revenue was £11,305 (IQR, £7,147-£15,222) and median loss was £3,795 (IQR, £605-£8687). Median LOS was 21 days (IQR 13–34) and 28.7% patients required critical care admission.

Ninety-six patients were treated operatively with either fixation (n=53) or revision (n=43). Median operating time was lower for fixation versus revision (132 [IQR, 115–185] vs 201 [IQR, 159–229] minutes, p=0.001). Median cost (£17,455 [IQR, £13,095-£22,824] vs £17,399 [£13,394-£23,404]) and median loss (£5,774 [IQR, £2,092-£10,472] vs £3,860 [IQR, £96-£7,601]) were similar for fixation and revision (p=0.99 and p=0.18, respectively). Median revenue was greater for revision versus fixation (£13,925 [IQR, £11,294-£17,037] vs £12,160 [IQR, £8,486-£14,390], p=0.02). There was no difference in LOS (21 [13–34] vs 21 [14–30] days, p=0.94), critical care requirements (20 [37.7%] vs 11 [25.6%], p=0.30), reoperations (3 [5.7%] vs 6 [14.0%], p=0.29], local complications (8 [15.1%) vs 12 [27.9%], p=0.20) or systemic complications (11 [20.8%] vs 11 [25.6%], p=0.75) between fixation and revision.

PFF treatment costs are high with inadequate reimbursement through tariff. Work is needed to address this disparity and reduce costs associated with LOS, theatre utilisation and implants. Treatment cost should not be used when deciding between fixation and revision surgery.

Abstract

Objectives

Current use of hard biomaterials such as cobalt-chrome alloys or ceramics to articulate against the relatively soft, compliant native cartilage surface reduces the joint contact area by up to two thirds. This gives rise to high and abnormal loading conditions which promotes degradation and erosion of the mating cartilage leading to pain, stiffness, and loss of function. Biomimetic soft lubrication strategies have been developed by grafting hydrophilic polymers onto substrates to form a gel-type surface. Surface grafted gels mimic the natural mechanisms of friction dissipation in synovial joints, showing a promising potential for use in hemiarthroplasty. This project aims to develop implant surfaces with properties tailored to match articular cartilage to retain and promote natural joint function ahead of total joint replacement.

Methods

Four different types of monomers were grafted in a one-step photopolymerisation procedure onto polished PEEK substrates. The functionalised surfaces were investigated using surface wettability, FTIR, and simplified 2D-tribometry tests against glass and animal cartilage specimens to assess their lubricity and mechanical properties for hemiarthroplasty articulations.

Cemented total hip replacement (THR) provides excellent outcomes and is cost-effective. Polished taper-slip (PTS) stems demonstrate successful results and have overtaken traditional composite-beam (CB) stems. Recent reports indicate they are associated with a higher risk of postoperative periprosthetic femoral fracture (PFF) compared to CB stems. This study evaluates risk factors influencing fracture characteristics around PTS and CB cemented stems.

Data were collected for 584 PFF patients admitted to eight UK centres from 25/05/2006-01/03/2020. Radiographs were assessed for Unified Classification System (UCS) grade and Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Association (AO/OTA) type. Statistical comparisons investigated relationships by age, gender, and stem fixation philosophy (PTS versus CB). The effect of multiple variables was estimated using multinomial logistic regression to estimate odds ratios (OR) with 95% confidence intervals (CI).

Median (IQR) age was 79.1 (72.0–86.0) years, 312 (53.6%) patients were female, and 495 (85.1%) stems were PTS. The commonest UCS grade was type B1 (278, 47.6%). The commonest AO/OTA type was spiral (352, 60.3%). Metaphyseal-split fractures occurred only with PTS stems with an incidence of 10.1%. Male gender was associated with a five-fold reduction in odds of a type C fracture (OR 0.22, 95% CI 0.12 to 0.41, p<0.001) compared to a type B fracture. CB stems were associated with significantly increased odds of transverse fracture (OR 9.51, 95% CI 3.72 to 24.34, p <0.001) and wedge fracture (OR 3.72, 95% CI 1.16 to 11.95, p <0.05) compared to PTS stems.

This is the largest study investigating PFF characteristics around cemented stems. The commonest fracture types are B1 and spiral fractures. PTS stems are exclusively associated with metaphyseal-split fractures, but their incidence is low. Males have lower odds of UCS grade C fractures compared to females. CB stems had higher odds of bending type fractures (transverse and wedge) compared to PTS stems. Biomechanical testing is needed for validation and investigation of modifiable factors which may reduce the risk of unstable fracture patterns requiring complex revision surgery over internal fixation.

The aim of this study is to determine whether fixation, as opposed to revision arthroplasty, can be safely used to treat reducible Vancouver B type fractures in association with a cemented collarless polished tapered femoral stem (the Exeter).

This is a retrospective cohort study of 152 operatively managed consecutive unilateral Vancouver B fractures involving Exeter stems. 130 were managed with open reduction and internal fixation (ORIF) and 22 with revision arthroplasty. Radiographs were assessed and classified by 3 observers. The primary outcome measure was revision of ≥1 component. Kaplan Meier survival analysis was performed. Logistic regression was used to identify risk factors for revision following ORIF. Secondary outcomes included any reoperation, complications, blood transfusion, length of hospital stay and mortality.

Fractures (B1 n=74 (49%); B2 n=50 (33%); and B3 n=28 (18%)) occurred at mean 6.7±10.4 years after primary THA (n=143) or hemiarthroplasty (n=15). Mean follow up was 6.5 ±2.6 years (3.2 to 12.1). Rates of revision and reoperation were significantly higher following revision arthroplasty compared to ORIF for B2 (p=0.001) fractures and B3 fractures (p=0.05). Five-year survival was significantly better following ORIF: 92% (86.4 to 97.4 95%CI) Vs 63% (41.7 to 83.3), p<0.001. No independent predictors of revision following ORIF were identified: fixation of B2 or B3 fractures was not associated with an increased risk of revision. Dislocation was the commonest mode of failure after revision arthroplasty. ORIF was associated with reduced blood transfusion requirement and reoperations, but there were no differences in medical complications, hospital stay or mortality between surgical groups.

When the bone-cement interface was intact and the fracture was anatomically reducible, Vancouver B2 fractures around Exeter stems can be treated with fixation as opposed to revision arthroplasty. Fixation of Vancouver B3 fractures can be performed in frail elderly patients without increasing revision risk.

In March 2020, the World Health Organisation declared the COVID-19 outbreak a pandemic. Multiple new guidelines were proposed and existing models of social, domestic and hospital care altered. Most healthcare systems were largely unprepared for this and this pandemic has tested their adaptability. This study is aimed at assessing the impact of covid-19 on the demographics, presentation and clinical management of patients with proximal femoral (hip) fractures.

This retrospective multi-centre cohort study compared all patients admitted with hip fractures, between 1st March and 30th May 2019 (Group PC: Pre-Covid) with hip fracture patients admitted over the same time period during the pandemic in 2020 (Group C: Covid). The data was obtained from the hospitals' local and National Hip Fracture Databases. Mortality data was checked with the Office for National Statistics (ONS). Primary outcomes were time to theatre, in-patient length of stay and 30-day mortality.

580 patients were included (304 PC, 276 C). Patient Charlson comorbidity index and Nottingham Hip Fracture scores were broadly similar across the two cohorts. There was a significant reduction in percentage of total hip replacements in Group C (11% to 5%, p=0.006). There was an increase in conservative management in group C (1% to 5%, p=0.002). The time to theatre was significantly delayed in Group C (43.7 hours C versus 34.6 hours PC, p<0.001). Overall length of hospital stay was similar in both groups (16.6 days PC versus 15 days C, p=0.089). 30-day mortality rate in Group C was 9.8% compared to 8.2% in Group PC (p=0.431), but for covid (+) patients it was significantly higher at 38.2% versus 5.8% in covid (−) patients (p<0.001).

This is one of the largest multi-centre comparative cohort study in the literature to date, examining the impact of the covid-19 pandemic on the management of hip fracture patients. Whilst mortality rates were similar in both groups, covid patients were almost seven times more likely to die, reflecting the seriousness of the covid-19 infection and its sequelae in such elderly, vulnerable patients.

Post-operative periprosthetic fracture of the femur (POPFF) is a growing problem associated with increased mortality. Most registry derived estimates of mortality only record patients who undergo revision and cohort studies are generally limited to a single center, which makes comparison for the purposes of service improvement difficult. The aim of this study is to perform a systematic review and meta-analysis of cohort studies reporting mortality following POPFF in the last decade.

Study methodology was peer-reviewed (PROSPERO: CRD42020170819). Literature search was conducted using Medline and EMBASE. Primary exposure was the diagnosis of POPFF, and the primary outcome measure was all-cause mortality: whilst an inpatient, within 30-days, within 90-days and within one year of POPFF. Proportion of patients dying (95% CI [confidence interval]) was estimated using metaregression. Results were compared to mortality following neck of femur fracture (NOF) from international NOF registry data.

4841 patients from 35 cohort studies were included. Study quality was generally low with a majority limited to a single centre. Weighted mean follow-up was 2.3 years and the most common POPFF was UCS B. Pooled proportion dying as an inpatient was 2.4% (95% CI 1.6% to 3.4%). Pooled proportion dying within 30 days was 3.3% (95% CI 2.0% to 5.0%). Pooled proportion dying within 90 days was 4.8% (95% CI 3.6% to 6.1%). Pooled proportion dying within one year was 13.4% (95% CI 11.9% to 14.8%). Mortality following POPFF was similar to that of NOF up to 30 days, but better at one year.

3.3% of patients die following POPFF within 30 days of injury. Mortality is similar to that experienced by patients following NOF up to 30 days, but better at one year, which may represent the lower underlying risk of death in the POPFF cohort. These results may form the basis for evaluation of services treating POPFF in the future.


There is a strong association between the presence of a calcar collar on a cementless stem and a reduced risk of revision surgery for periprosthetic fracture of the femur (PFF). A medial calcar collar may act to reduce relative movement between the implant and femur during PFF, through calcar-collar contact (CCC). The aims were:

Estimate the effect of CCC on periprosthetic fracture mechanics.

Estimate the effect of initial calcar-collar separation on the likelihood of CCC.

Three groups of six composite femurs were implanted with a fully coated collared cementless femoral stem. Neck resection differed between groups (group 1 = no additional resection, group 2 = 3mm additional resection, group 3 = 6mm additional resection). PFF were simulated using a previously published technique. Fracture torque and rotational displacement were measured and torsional stiffness and rotational work prior to fracture were estimated. Results between trials where CCC did and did not occur where compared using Mann-Whitney U tests. Logistic regression estimated the odds (OR) of failing with 95% confidence interval (CI) to achieve CCC for a given initial separation.

Where CCC occurred fracture torque was greater (47.33 [41.03 to 50.45] Nm versus 38.26 [33.70 to 43.60] Nm, p= 0.05) and torsional stiffness was greater (151.38 [123.04 to 160.42] rad.Nm−1 versus 96.86 [84.65 to 112.98] rad.Nm−1, p <0.01).

CCC was occurred in all cases in group one, 50% in group two and 0% in group three. OR of failure to obtain CCC increased 3.8 fold (95% CI 1.6 to 30.2, p <0.05) for each millimetre of separation.

Resistance to fracture and construct stiffness increased when a the collar made contact with the calcar prior to fracture and the chances of contact decrease with increasing initial separation at the time of implantation. Surgeons should aim to achieve a calcar-collar distance of less than 1mm following implantation to ensure CCC and to reduce the risk of fracture

Aims

This study evaluates risk factors influencing fracture characteristics for postoperative periprosthetic femoral fractures (PFFs) around cemented stems in total hip arthroplasty.

Methods

Data were collected for PFF patients admitted to eight UK centres between 25 May 2006 and 1 March 2020. Radiographs were assessed for Unified Classification System (UCS) grade and AO/OTA type. Statistical comparisons investigated relationships by age, gender, and stem fixation philosophy (polished taper-slip (PTS) vs composite beam (CB)). The effect of multiple variables was estimated using multinomial logistic regression to estimate odds ratios (ORs) with 95% confidence intervals (CIs). Surgical treatment (revision vs fixation) was compared by UCS grade and AO/OTA type.

Results were compared between collared and collarless group and the comparison was evaluated against previously published work using fresh frozen femurs and the same protocol.

The angle of acetabular inclination is an important measurement in total hip replacement (THR) procedures. Determining the acetabular component orientation intra-operatively remains a challenge. An increasing number of innovators have described techniques and devices to achieve it. This paper describes a mechanical inclinometer design to measure intra-operative acetabular cup inclination. Then, the mechanical device is tested to determine its accuracy. The aim was to design an inclinometer to measure inclination without existing instrumentation modification. The device was designed to meet the following criteria: 1. measure inclination with acceptable accuracy (+/− 5o); 2. easy to use intra-operatively (handling & visualization); 3. adaptable and useable with majority of instrumentation kits without modification; 4. sterilizable by all methods; 5. robust/reusable.

The prototype device was drafted by computer aided design (CAD) software. Then a prototype was constructed using a 3D printer to establish the final format. The final device was CNC machined from SAE 304 stainless steel. The design uses an eccentrically weighted flywheel mounted on two W16002-2RS ball bearings pressed into symmetrical housing components. The weighted wheel is engraved with calibrated markings relative to its mass centre. Device functioning is dependent on gravity maintaining the weighted wheel in a fixed orientation while the housing can adapt to the calibration allowing for determining the corresponding measurement. The prototype device accuracy was compared to a digital device. A digital protractor was used to create an angle. The mechanical inclinometer (user blinded to digital reading) was used to determine the angle and compared to the digital reading.

The accuracy of the device compared to the standard freehand technique was assessed using a saw bone pelvis fixed in a lateral decubitus position. 18 surgeons (6 expert, 6 intermediate, 6 novice) were asked to place an uncemented acetabular cup in a saw bone pelvis to a target of 40 degrees. First freehand then using the inclinometer. The inclination was determined using a custom-built inertial measurement unit with the user blinded to the result. Comparison between the mechanical and digital devices showed that the mechanical device had an average error of −0.2, a standard deviation of 1.5, and range −3.3 to 2.6. The average root mean square error was 1.1 with a standard deviation of 0.9. Comparison of the inclinometer to the freehand technique showed that with the freehand component placement 50% of the surgeons were outside the acceptable range of 35–45 degrees. The use of the inclinometer resulted all participants to achieve placement within the acceptable range. It was noted that expert surgeons were more accurate at achieving the target inclination when compared to less experienced surgeons.

This work demonstrates that the design and initial testing of a mechanical inclinometer is suitable for use in determining the acetabular cup inclination in THR. Experimental testing showed that the device is accurate to within acceptable limits and reliably improved the accuracy of uncemented cup implantation in all surgeons.

Aims

The mobile bearing Oxford unicompartmental knee arthroplasty (OUKA) is recommended to be performed with the leg in the hanging leg (HL) position, and the thigh placed in a stirrup. This comparative cadaveric study assesses implant positioning and intraoperative kinematics of OUKA implanted either in the HL position or in the supine leg (SL) position.

Methods

A total of 16 fresh-frozen knees in eight human cadavers, without macroscopic anatomical defects, were selected. The knees from each cadaver were randomized to have the OUKA implanted in the HL or SL position.

Introduction

We investigated predictors of poor outcomes following metal-on-metal hip arthroplasty (MoMHA) revision surgery performed for adverse reactions to metal debris (ARMD), to help inform the revision threshold and type of reconstruction.

Patients and Methods

A retrospective cohort study was performed involving 346 MoMHAs revised for histologically confirmed ARMD at two specialist centres (245=hip resurfacing, 101=total hip arthroplasty). Numerous preoperative (blood metal ions and imaging) and intraoperative (findings, and components removed/implanted) factors were used to predict poor outcomes. Poor outcomes were postoperative complications (including re-revisions), 90-day mortality, and poor Oxford Hip Scores (<27/48). Multivariable logistic regression models for predicting poor outcomes were developed using stepwise selection methods.

Background

Method of fixation in THA is a contentious issue, with proponents of either technique citing improved implant survival and outcomes. Current comparisons rely on insufficiently powered studies with short-term follow up or larger poorly controlled registry studies. Patient factors are considered a key variable contributing to the risk of implant failure. One way to overcome this confounder is to compare the survival of cementless and cemented THAs patients who have undergone bilateral THAs with cemented hip on one side and cementless hip on the other. We compared stem survival of patients who have bilateral THA with one cemented stem in one hip and a cementless stem in the contralateral hip in the National Joint Registry.

Methods

UK National Joint Registry is the largest registry of its kind in the world. This study included 2934 patients with 5868 THAs who underwent bilateral THAs s between 2003 and 2016. These patients had undergone bilateral sequential THAs within 3 years of each other: cemented THA on one side and cementless on the other, Patients had identical pre-operative American Society of Anaesthesiologists group for both THAs and same indication for surgery. Implant survival was compared using Cox regression with an endpoint of stem revision.

Introduction

Smoking, a modifiable factor, may adversely affect post-operative outcomes. Healthcare providers are increasingly denying smokers access to total hip arthroplasty (THA) until they stop smoking. Evidence supporting this is unclear. We assessed the effect of smoking on outcomes following THA.

Patients and Methods

We performed a retrospective observational study involving 60,812 THAs (12.4%=smokers, 31.2%=ex-smokers, 56.4%=non-smokers) from the Clinical Practice Research Datalink. Data were linked with Hospital Episode Statistics and the Office for National Statistics to identify outcomes. The effect of smoking on post-operative outcomes (complications, medications, revision, mortality, PROMs) was assessed using adjusted regression.

Background

Post-operative periprosthetic femoral fractures (PFF) are a devastating complication associated with high mortality and are costly. Few risk factors are modifiable apart from implant choice. The design features governing risk of PFF are unknown. We estimated the 90-day risk of revision for PFF associated with design features of cementless femoral stems and to investigate the effect of a collar on early PFF risk using a biomechanical in-vitro model.

Patients, materials and methods

337 647 primary THAs from the National Joint Registry (UK) were included in a multivariable survival and regression analysis to identify the adjusted hazard of PFF revision following primary THA using cementless stems. The effect of a collar in cementless THA on early PFF was evaluated in an in-vitro model using paired fresh frozen cadaveric femora.

Introduction

Intraoperative periprosthetic femoral fractures (IOPFF) lead to reduced implant survival. A deeper understanding of predictors enables surgeons to modify techniques and patient selection to reduce the risk of IOPFF. The aim of this study was to estimate predictors of IOPFF and each anatomical subtype (calcar crack, trochanteric fracture, femoral shaft fracture) during primary THA.

Methods

This retrospective cohort study included 793823 primary THAs between 2004 and 2016. Relative risks for patient, surgical and implant factors are estimated for any IOPFF fracture and for all anatomical subtypes of IOPFF.

Component alignment and soft tissue constraints are key factors affecting function and implant survival after total knee replacement (TKR). Knee kinematics contribute to knee function whilst soft tissue constraints and component alignment impact polyethylene wear. This study experimentally investigated the effect of soft tissue constraints and component alignment on the kinematics and wear of a TKR.

A six station electromechanical ProSim knee simulator was used with the ISO 14243-1:2009 standard force control inputs; axial force, flexion-extension (FE), tibial rotation (TR) torque and anterior-posterior (AP) force. This allowed the kinematics to vary with the test conditions. The soft tissue constraints were simulated using virtual springs.

DePuy Sigma XLK fixed bearing TKRs were tested in 25% bovine serum (in 0.04% sodium azide) lubricant. The average output kinematics across 6 stations were found for each test and the peak values compared. The wear rates were calculated over 2 million cycles (MC), the serum was changed every 350,000 cycles and the tibial inserts weighed after every MC. A one way ANOVA and post hoc Tukey's test was used to compare the kinematics and wear with significance taken at p<0.05.

The kinematics and wear rates for three soft tissue conditions were established under ideal alignment (Table 1). The ISO standard springs for a cruciate substituting (CS) and a cruciate retaining (CR) prosthesis were used to represent a knee with a resected ACL and PCL and a knee with a resected ACL respectively. The third spring condition was based on clinical data to represent a “stiff” knee.

Three other alignment conditions were then assessed using “stiff” knee springs; 4° varus, 14° rotational mismatch and 10° posterior tibial slope. These alignments were chosen to represent the range found in clinical data.

Under ideal alignment the “stiff” knee springs had significantly lower peak AP and TR displacements (0.9mm, 2mm, 2mm and 3.6°, 7.1°, 7.8° for the “stiff”, CR and CS springs respectively) than the other springs (p<0.01). The “stiff” knee spring had a significantly lower wear rate than the CR spring; 1.58 ±1.20mm³/MC compared to 4.71±1.29 mm³/MC (p<0.01).

The varus and rotated components had significantly larger peak AP displacements of 2.56mm and 2.42mm respectively, than the ideal and tibial slope fixtures (1.97mm and 0.92mm respectively) (p<0.01). The rotated components had significantly higher internal rotation of 12.2° compared to 4.4°, 3.7° and 3.5° for the tibial slope, varus and ideal components respectively (p<0.01).

The ideal and varus components had significantly lower wear than the tibial slope and rotated components (1.58±1.20mm³/MC and 0.15±0.83mm³/MC compared to 8.24±7.72mm³/MC and 5.19±1.12mm³/MC respectively) (p<0.01). This may be due to increased AP and TR displacements with the rotated components and the increased anterior AP displacement with the tibial slope components, resulting in wear on the posterior edge of the tibial insert.

Soft tissue constraints and component alignment had a significant effect on the kinematics and wear. Experimental simulation should test a variety of soft tissue and alignment conditions to reflect the range observed clinically and determine causes for early failure.

Introduction

The revision rate of unicompartmental knee replacement (UKR) in national joint registries is much higher than that of total knee replacements and that of UKR in cohort studies from multiple high-volume centres. The reasons for this are unclear but may be due to incorrect patient selection, inadequate surgical technique, and inappropriate indications for revision. Meniscal bearing UKR has well defined evidence based indications based on preoperative radiographs, the surgical technique can be assessed from post-operative radiographs and the reason for revision from pre-revision radiographs. However, for an accurate assessment aligned radiographs are required. The aim of the study was to determine why the revision rate of UKR in registries is so high by undertaking a radiographic review of revised UKR identified by the United Kingdom's (UK) National Joint Registry (NJR).

Methods

A novel cross-sectional study was designed. Revised medial meniscal bearing UKR with primary operation registered with the NJR between 2006 and 2010 were identified. Participating centres from all over the country provided blinded pre-operative, post-operative, and pre-revision radiographs. Two observers reviewed the radiographs.

Introduction

Reverse hybrid total hip replacement (THR) offers significant theoretical benefits but is uncommonly used. Our primary objective was to evaluate implant survival with all cause revision and revision for aseptic loosening of either component as endpoints.

Patients/Materials & Methods

Data was collected prospectively on 1, 088 (988 patients) consecutive reverse hybrid THRs. Mean patient age was 69.3 years (range, 21–94) and mean follow-up was 8.2 years (range, 5–11.3). No patients were lost to follow-up. Overall, 194 (17.8%) procedures were performed in patients under 60 years, 666 (61.1%) were performed in female patients and 349 (32.1%) were performed by a trainee. Acetabular components were ultra-high molecular weight polyethylene in 415 (38.1%) hips, highly cross-linked polyethylene in 669 (61.5%) hips and vitamin E stabilised polyethylene in 4 (0.4%) hips. Femoral stems were collared in 757 (69.7%) hips and collarless in 331 (30.3%) hips. Femoral head sizes were 28 mm in 957 (87.9%) hips and 32 mm in 131 (12.1%) hips. Survival analysis was performed using Kaplan Meier methodology. Log rank tests were used to asses differences in survival by age, gender, head size and surgeon grade.

Pseudotumours have recently been reported in non-metal-on-metal total hip replacements (non-MoM THRs), however the magnitude and risk factors for this complication are unknown, as is the outcome of its treatment.

3340 primary THR undergoing revision for pseudotumour between 2008 and 2015 were identified in the National Joint Registry for England and Wales. 7.5% (n=249) of these pseudotumour revisions, had non-MoM bearing surfaces. The risk of revision for pseudotumour in non-MoM hips was 0.032% (249/789,397; 95% CI 0.028%–0.036%). The risk of pseudotumour revision was 2.35 times (95% CI 1.76–3.11) higher in ceramic-on-ceramic compared with hard-on-soft bearings, and 2.80 times (95% CI 1.74–4.36) higher in 36mm metal-on-polyethylene bearings compared to 28mm and 32mm metal-on-polyethylene bearings.

The outcome of revision for pseudotumour non-MoM hips was studied in 185 hips. 13.5% (n=25) had re-revisions at a mean of 1.2 years (range 0.1–3.1 years). Infection (32%), dislocation/subluxation (24%), and aseptic loosening (24%) were the commonest indications for re-revision. The 4-year survival rate was 83.8% (95% CI=76.7%–88.9%). Multiple revision indications (Hazard Ratio (HR)=2.78; 95% CI=1.03–7.49) and incomplete revision procedures (HR=5.76; 95% CI=1.28–25.9) increased the risk of re-revision

Although the overall risk of revision for pseudotumour in non-MoM THRs is low, the risk is increasing and is significantly higher in ceramic-on-ceramic and large head metal-on-polyethylene THR. These revisions have a high early failure rate.


Outcomes following metal-on-metal hip replacement (MoMHR) revision surgery for adverse reactions to metal debris (ARMD) have been poor, and inferior compared with non-ARMD revisions. Subsequently, surgeons and worldwide authorities widely recommended early revision for ARMD, with a lower surgical threshold adopted. However, the impact of early surgery for ARMD is unknown. We compared the rates of adverse outcomes following MoMHR revision surgery in matched ARMD and non-ARMD patients.

We performed a retrospective observational study using data from the National Joint Registry for England, Wales, Northern Ireland and the Isle of Man. All MoMHR patients subsequently undergoing revision surgery for any indication between August 2008 and August 2014 were eligible. ARMD and non-ARMD revisions were matched one-to-one for multiple potential confounding factors using propensity scores. Adverse outcomes following revision surgery (intra-operative complications, mortality, re-revision surgery) were compared between matched groups using regression analysis.

In 2,576 matched MoMHR revisions (ARMD=1,288 and non-ARMD=1,288), intra-operative complications were similar between ARMD (2.4%) and non-ARMD (2.5%) revisions (odds ratio=0.97, 95% CI=0.59–1.60; p=0.899). All-cause mortality rates were lower following ARMD revision compared with non-ARMD revision (hazard ratio (HR)=0.43, 95% CI=0.22–0.86; p=0.018). All-cause re-revision rates were lower following ARMD revision compared with non-ARMD revision (HR=0.52, 95% CI=0.36–0.75; p<0.001). Compared with ARMD revision (5-years=94.3%), MoMHR revisions for infection (5-years=81.2%) and dislocation/subluxation (5-years=81.9%) had the lowest implant survival rates.

Contrary to previous observations, MoMHRs revised for ARMD have approximately half the risk of re-revision and death compared to non-ARMD revisions. We suspect worldwide regulatory authorities have positively influenced outcomes following ARMD revision by widely recommending that surgeons exercise a lower revision threshold.

Our findings suggest the threshold for ARMD revision surgery need not be lowered further. The high risk of failure following MoMHR revision for infection and dislocation is concerning.

Recent studies have demonstrated that implant-specific blood metal ion thresholds exist in unilateral and bilateral metal-on-metal (MoM) hip arthroplasty patients, with these thresholds being most effective for identifying patients at low-risk of adverse reactions to metal debris (ARMD). We investigated whether these new blood metal ion thresholds could effectively identify patients at risk of ARMD in an external cohort of MoM hip arthroplasty patients.

We performed a validation study involving 803 MoM hip arthroplasties implanted in 710 patients at three European centres (323=unilateral Birmingham Hip Resurfacing (BHR); 93=bilateral BHR; 294=unilateral Corail-Pinnacle). All patients underwent whole blood metal ion sampling. Patients were divided into those with ARMD (revised for ARMD or ARMD on imaging; n=75), and those without ARMD (n=635). Previously devised implant-specific blood metal ion thresholds (cobalt=2.15μg/l for unilateral BHR; maximum cobalt or chromium=5.5μg/l for bilateral BHR; cobalt=3.57μg/l for unilateral Corail-Pinnacle) were applied to the validation cohort, with receiver operating characteristic curve analysis used to establish the discriminatory characteristics for each respective threshold.

The area under the curve, sensitivity, specificity, positive predictive value and negative predictive value for distinguishing between patients with and without ARMD for each implant-specific threshold were respectively: unilateral BHR=89.4% (95% CI=82.8%-96.0%), 78.9%, 86.7%, 44.1%, 96.9%; bilateral BHR=89.2% (95% CI=81.3%-97.1%), 70.6%, 86.8%, 54.5%, 93.0%; unilateral Corail-Pinnacle=76.9% (95% CI=63.9%-90.0%), 65.0%, 85.4%, 24.5%, 97.1%. The 7μg/l UK MHRA threshold missed significantly more patients with ARMD compared with the implant-specific thresholds (4.9% vs. 2.8%; p=0.0003).

This external multi-centre validation study has confirmed that MoM hip arthroplasty patients with blood metal ion levels below newly devised implant-specific thresholds have a low-risk of ARMD. Compared to implant-specific thresholds, the currently proposed fixed MHRA threshold missed more patients with ARMD.

We recommend using implant-specific thresholds over fixed thresholds when managing MoM hip arthroplasty patients.


Recent studies have reported on non-metal-on-metal hip arthroplasty (non-MoMHA) patients requiring revision surgery for adverse reactions to metal debris (ARMD). Although the outcomes following revision surgery for ARMD in MoMHA patients are known to generally be poor, little evidence exists regarding outcomes following non-MoMHA revision surgery performed for ARMD. We determined the outcomes following non-MoMHA revision surgery performed for ARMD, and identified predictors of re-revision.

We performed a retrospective observational study using data from the National Joint Registry for England, Wales, Northern Ireland and the Isle of Man. All primary non-MoMHA patients who subsequently underwent revision surgery for ARMD between 2008–2014 were included (n=185). Outcome measures following ARMD revision were intraoperative complications, mortality, and re-revision surgery. Predictors of re-revision surgery were identified using Cox regression analysis.

Intra-operative complications occurred in 6.0% (n=11) of ARMD revisions. The cumulative 4-year patient survival rate was 98.2% (95% CI=92.9–99.5%). Re-revision surgery was performed in 13.5% (n=25) of hips at a mean time of 1.2 years (range 0.1–3.1 years) following ARMD revision. Infection (32%), dislocation/subluxation (24%), and aseptic loosening (24%) were the commonest re-revision indications. The cumulative 4-year implant survival rate was 83.8% (95% CI=76.7%-88.9%). Significant predictors of re-revision were: multiple revision indications (Hazard Ratio (HR)=2.78; 95% CI=1.03–7.49; p=0.043), incomplete revision procedures (including modular component exchange only) (HR=5.76; 95% CI=1.28–25.9; p=0.022), and ceramic-on-polyethylene revision bearings (HR=3.08; 95% CI=1.01–9.36; p=0.047).

Non-MoMHA patients undergoing ARMD revision have a high short-term risk of re-revision. Infection, dislocation/subluxation, and aseptic loosening were the commonest re-revision indications. Furthermore, important and potentially modifiable predictors of future re-revision were identified.

Although the poor prognostic factors identified require validation in future studies, our findings may be used to counsel patients about the risks associated with ARMD revision surgery, and guide decisions about the reconstructive procedure.

Aim

Advocates of Debridement-Antibiotics-and-Implant-Retention (DAIR) in hip peri-prosthetic joint infection (PJI) argue that a procedure not disturbing a sound prosthesis-bone interface is likely to lead to better survival and functional outcome compared to revision. However, no evidence supports this. This case-control study's aims were to compare outcome of DAIRs for infected 1° total hip arthroplasty (THA) with outcomes following 1° THA and 2-stage revisions of infected 1° THAs.

Method

We retrospectively reviewed all DAIRs, performed for confirmed infected 1° THR (DAIR-Group, n=80), in our unit between 1997–2013. Data recorded included patient demographics, medical history, type of surgery and organism identified. Outcome measures included complications, mortality, implant survivorship and functional outcome using the Oxford Hip Score (OHS). Outcome was compared with 2 control groups matched for gender and age; a cohort of 1° THA (1°-THA-Group, n=120) and a cohort of 2-stage revisions for infection (2-Stage-Revision-Group, n=66).

Introduction

The impact of pseudotumours associated with metal-on-metal hip resurfacings (MoMHRs) within the second decade is unknown. We investigated: (1) the incidence and risk factors for all-cause and pseudotumour revision following MoMHR at 15-years follow-up, and (2) whether risk factors were gender specific.

Patients and methods

This single-centre prospective cohort study included 1429 MoMHRs (1216 patients; 40% female) implanted between 1999–2009. All patients were contacted in 2010 and 2012 as per national recommendations. Patients with hip problems and/or suboptimal Oxford Hip Scores (<41/48) underwent cross-sectional imaging and blood metal ion sampling. Revisions were performed as indicated with diagnoses confirmed from operative and histopathological findings. Multi-variate Cox proportional hazard models assessed the association of predictor variables on time to all-cause and pseudotumour revision.

Introduction

This study aimed to: (1) compare published follow-up guidelines for metal-on-metal (MoM) hip patients and analyse protocols in relation to current evidence, and (2) assess the financial implications of these guidelines.

Methods

Follow-up guidance for MoM hips from five national authorities (MHRA in the UK; EFORT; United States FDA; Therapeutic Goods Administration of Australia; Health Canada) were contrasted and critically appraised. Using National Joint Registry (NJR) data (67,363 MoM hips implanted) the cost of annual surveillance for all MoM hips recorded in the NJR was calculated for each protocol.

There is a large amount of evidence available about the relative merits of unicompartmental and total knee arthroplasty (UKA and TKA). Based on the same evidence, different people draw different conclusions and as a result, there is great variability in the usage of UKA.

The revision rate of UKA is much higher than TKA and so some surgeons conclude that UKA should not be performed. Other surgeons believe that the main reason for the high revision rate is that UKA is easy to revise and, therefore, the threshold for revision is low. They also believe that UKA has many advantages over TKA such as a faster recovery, lower morbidity and mortality and better function. They therefore conclude that UKA should be undertaken whenever appropriate.

The solution to this argument is to minimise the revision rate of UKA, thereby addressing the main disadvantage of UKA. The evidence suggests that this will be achieved if surgeons use UKA for at least 20% of their knee arthroplasties and use implants that are appropriate for these broad indications.

Anatomical variations in hip joint anatomy are associated with both the presence and location of tibiofemoral osteoarthritis (OA).

Introduction

Variations in hip joint anatomy can alter the moment-generating capacity of the hip abductor muscles, possibly leading to changes in the magnitude and direction of ground reaction force and altered loading at the knee. Through analysis of full-limb anteroposterior radiographs, this study explored the hypothesis that knees with lateral and medial knee OA demonstrate hip geometry that differs from that of control knees without OA.

Purpose

According to the National Institute for Clinical Excellence (NICE) a risk assessment for venous thromboembolism (VTE) should be conducted on all patients undergoing elective orthopaedic surgery. We looked at the patient outcome undergoing elective Ilizarov surgery in terms of symptomatic VTE occurring during or after frame management.

Methods

We performed a retrospective chart review of all adult Ilizarov cases performed by a single surgeon between 2000–2011. Patient mortality was confirmed using the Demographics Batch Service.

Unicompartmental Knee Replacement (UKR) is associated with fewer complications, faster recovery and better function than Total Knee Replacement (TKR). However, joint registries demonstrate a higher revision rate in UKR, limiting its use. Currently most UKRs are cemented and performed using a minimally invasive technique. In joint registries, common reasons for revision include aseptic loosening and pain. These problems could potentially be addressed by using cementless implants, which may provide more reliable fixation.

The objectives of this study were to compare the quality of fixation (determined by the incidence and appearance of radiolucencies), and clinical outcomes of cemented and cementless UKR at five years.

A randomised controlled trial was established with 63 knees (62 patients) randomised to either cemented (32 patients) or cementless UKR (30 patients). Fixation was assessed with fluoroscopic radiographs aligned to the bone-implant interface at one and five years. Outcome scores were collected pre-operatively and at one, two and five years, including Oxford Knee Score (OKS), American Knee Society Score, objective and functional (AKSS-O/F) and Tegner Activity Scale (TAS), expressed as absolute scores and 0–5 year change (δ) scores.

Four patients died during the study period. There were no revisions. Mean operative time was 11 minutes shorter in the cementless group (p=0.029). At five years, there was no significant difference in any outcome measure except AKSS-F and δAKSS-F which were significantly better in the cementless group (both p=0.003). There were no femoral radiolucencies in either group. There were significantly more tibial radiolucencies in the cemented group (20/30 vs 2/27, p< 0.001). There were nine complete radiolucencies in the cemented group and none in the cementless group (p< 0.001).

Cementless fixation provides improved fixation at five years compared to cemented fixation in UKR, maintaining equivalent or superior clinical outcomes with a shorter operative time and no increase in complications.

Introduction

Solid or cystic pseudotumour is a potentially destructive complication of metal on metal (MoM) couples, usually needing revision surgery. However, complete clearance of the pseudotumour is unlikely at times. This prospective case-controlled study reports cases which had recurrence after revision surgery for pseudotumour related to metal on metal hip couples.

Methods

A total of 37 hips (33 MoM hip resurfacing and four big head MoM total hip arthroplasty (THA)) were revised for pseudotumour during the last 10 years. The patient demographics, time to revision, cup orientation, operative and histological findings were recorded for this cohort. Patients were divided into two groups - group R (needing re-revision for disease progression) and group C (control - no evidence of disease progression). Oxford hip scores (OHS, 0–48, 48 best outcome) were used to assess clinical outcome. The diagnosis of disease progression was based on recurrence of clinical symptoms, cross-sectional imaging, operative and histological findings.

Indications for Unicompartmental Knee Arthroplasty (UKA) vary between units. Some authors have suggested, and many surgeons believe, that medial UKA should only be performed in patients who localise their pain to the medial joint line. This is despite research showing a poor correlation between patient-reported location of pain and radiological or operative findings in osteoarthritis. The aim of this study is to determine the effect of patient-reported pre-operative pain location and functional outcome of UKA at one and five years.

Pre-operative pain location data were collected for 406 knees (380 patients) undergoing Oxford medial UKA. Oxford Knee Score, American Knee Society Scores and Tegner activity scale were recorded preoperatively and at follow-up. 272/406 (67%) had pure medial pain, 25/406 (6%) had pure anterior knee pain and 109/406 (27%) had mixed or generalised pain. None had pure lateral pain. The primary outcome interval is one year; 132/406 patients had attained five years by the time of analysis and their five year data is presented.

At one and five years, each group had improved significantly by each measure (mean δOKS 15.6 (SD 8.9) at year one, 16.3 (9.3) at year five). There was no difference between the groups, nor between patients with and without anterior knee pain or isolated medial pain.

We have found no correlation between preoperative pain location and outcome. We conclude that localised medial pain should not be a prerequisite to UKA and that it may be performed in patients with generalised or anterior knee pain.

Introduction

The peri-operative analgesic management of patients having either Total Knee Replacement (TKR) or Unicompartmental Knee Replacement (UKR) is an area that continues to have prominence, driven in part by the desire to reduce hospital stay, while maintaining high patient satisfaction. This is particularly relevant in the current climate of healthcare cost savings. We evaluated the role of “top up” intra-articular local anaesthetic injection after identifying that an appreciable number of patients in the unit suffered “breakthrough pain” on the first post-op day, when the effects of local analgesia are wearing off.

Method

43 patients, who were scheduled to have a cemented Oxford UKR, were prospectively recruited and randomised. All patients had the same initial anaesthetic regime of general anaesthesia, femoral nerve block and intra-operative intra-articular infiltration of the cocktail. All patients had a 16G multi-holed epidural catheter placed intra-articularly prior to wound closure. Patients had the same operative technique, post operative rehabilitation and rescue analgesia.

An independent observer recorded post-operative pain scores using a visual analogue score (1–10) every 6 hours and any rescue analgesia that was required. On the morning after surgery, 22 patients, (Group I), received 20 mls of 0.5% bupivicaine through the catheter whilst 21 patients, (Group II), had 20 mls of normal saline by the same observer, (who was blinded to the contents of the solution being injected), after which the catheter was removed.

Introduction

Since the introduction of 3rd generation Metal-on-Metal-Hip-Resurfacing-Arthroplasty (MoMHRA), thousands of such prostheses have been implanted worldwide in younger patients with end-stage hip osteoarthritis. However, no independent centre has reported their medium-to-long term outcome. The aim of this study is to report the ten year survival and outcome of the Birmingham Hip Resurfacing (BHR), the most commonly used MoMHRA worldwide.

Methods

Since 1999, 648 BHRs were implanted in 555 patients, the majority of which were male (326). The mean age at surgery was 52.1years (range: 17–82), with primary OA as most common indication (85%). Mean follow up was 7.1years (range: 1–11). The Oxford Hip Score (OHS) and UCLA questionnaires were sent to all patients. Implant survival was established, with revision as the end point. Sub-analysis was performed by gender, femoral component size (small: <45mm, standard: 46–52, large: >53mm) and age at surgery (young:<50yrs, old:>50yrs).

INTRODUCTION

Femoral neck narrowing (NN) following Metal-on-Metal Hip Resurfacing Arthroplasty (MoMHRA) is a well-recognised clinical phenomenon. The incidence of resurfaced hips with NN > 10% is reported to be up to 27%. Its pathogenesis is thought to be multi-factorial secondary to stress shielding, impingement, osteolysis secondary to wear/ion particles and as a result of reduced vascularity and pressure effect on cancellous bone secondary to the presence of a soft-tissue mass around the resurfaced hip. Recognised risk factors for its development include: female gender and the presence of a pseudotumour. Serum Chromium (Cr) and Cobalt (Co) are recognised surrogate markers of in-vivo wear of MoMHRA. The aims of this study were to establish whether NN is associated with increased wear.

METHODS

A cohort of 214 patients with unilateral MoMHRA (139M: 75F) was included in this study. Primary osteoarthritis was the diagnosis leading to surgery for the majority of patients (208). The average age at surgery was 54.1 years old (13–73). Six different implants were used; BHR (116), Conserve plus (92), Recap (2), ASR (2), Adept (1) and Cormet (1). The average femoral component size was 49.2mm (range: 38–59). The average follow up was 4.3 years (range: 2–10). Patients were subdivided into 3 groups as per implant size. Small size component group had implants <45mm, average size group had components 45–51mm and large component size group had components >51mm.

All patients had Prosthesis-Junction-Ratio (PJR) measured from postoperative (PJRpost) and at latest follow up (PJRfollow) radiographs. Measurements were made using the method described by Lilikakis1.

Metal ion levels (Cr/Co) were measured at last follow-up for all patients. Cr level >5.1g/ml and Co levels >4.4 g/ml were considered high2 and patients with such levels formed the high ion group.

Introduction

The options for the treatment of the young active patient with unicompartmental symptomatic osteoarthritis and pre-existing Anterior Cruciate Ligament (ACL) deficiency are limited. Patients with ACL deficiency and end-stage medial compartment osteoarthritis are usually young and active. The Oxford Unicompartmental Knee Replacement (UKA) is a well established treatment option in the management of symptomatic end-stage medial compartmental osteoarthritis, but a functionally intact ACL is a pre-requisite for its satisfactory outcome. If absent, high failure rates have been reported, primarily due to tibial loosening. Previously, we have reported results on a consecutive series of 15 such patients in whom the ACL was reconstructed and patients underwent a staged or simultaneous UKA. The aim of the current study is to provide an update on the clinical and radiological outcomes of a large, consecutive cohort of patients with ACL reconstruction and UKA for the treatment of end-stage medial compartment osteoarthritis and to evaluate, particularly, the outcome of those patients under 50.

Methods

This study presents a consecutive series of 52 patients with ACL reconstruction and Oxford UKA performed over the past 10 years (mean follow-up 3.4 years). The mean age was 51 years (range: 36–67). Procedures were either carried out as Simultaneous (n=34) or Staged (n=18). Changes in clinical outcomes were measured using the Oxford Knee Score (OKS), the change in OKS (OKS=Post-op − Pre-op) and the American Knee Society Score (AKSS). Fluoroscopy assisted radiographs were taken at each review to assess for evidence of loosening, radiolucency progression, (if present), and component subsidence.

INTRODUCTION

Studies have suggested that there is a reduction in head-neck-ratio (HNR) associated with MoMHRA. A reduction in HNR at operation would decrease range of movement and increase impingement risk. Impingement could lead to 20 edge loading, increasing wear. Serum ion levels of Chromium (Cr) and Cobalt (Co) are surrogate markers of wear. Although acetabular component orientation has been shown to contribute to wear and PT development, the role of a decrease in HNR has only been highlighted in PT development. This study aimed to measure changes in HNR that occur at resurfacing and determine any gender- and component size-specific differences. In addition it aimed to determine whether changes in HNR could be associated with increased wear.

METHODS

84 patients (56M: 28F) with unilateral MoMHRA were included. The mean age at surgery was 57 years. The mean femoral component was 49mm. Components were considered small if <45mm, average if between 45–50mm and large if >50mm. Three designs were implanted; BHR, C+ and Recap. The average follow up was 4 years. All patients had Cr/Co levels measured at follow up. Patients were considered to have high ions if Cr and Co levels were 5.1ppb and 4.4ppb respectively.

Pre-operative HNR (HNRpre) and the post-operative HNR (HNRpost) were made from the respective pelvic radiographs. Assuming a 2mm thick cartilage layer, the HNR based on the diameter of the articular cartilage pre-operatively (HNRart) was calculated.

Introduction

Radiolucencies beneath the tibial component are well recognized in knee arthroplasty; the aetiology and significance are poorly understood. Non-progressive narrow radiolucencies with a sclerotic margin are thought not to be indicative of loosening. Factors which decrease the incidence of radiolucencies include cementless fixation and the use of pulse lavage. Leg/component alignment or BMI do not influence radiolucency. We are not aware of any studies that have looked at the effect of load type on radiolucency.

The Oxford domed lateral tibial component was introduced to decrease the bearing dislocation rate that was unacceptably high with the flat tibial tray. However, the introduction of the domed tibial component alters the forces transmitted through the implant-cement-bone interface. As the Oxford UKR uses a fully congruent mobile bearing, the forces transmitted through the interface with a flat tray are compressive, except for the effect of friction. However, with the domed tibial component shear forces are introduced. The aim of this study was to assess the prevalence of radiolucency beneath the previous flat design and the new domed tibial tray.

Patients and methods

A consecutive series of 248 cemented lateral UKRs (1999–2009) at a single institution were assessed. The first 55 were with a flat tibia and the subsequent 193 with a domed component. One year post-op radiographs were assessed, by two observers, for the presence (full or partial) and distribution of radiolucency. The distribution and thickness of each radiolucency. Cases were excluded for missing or poorly aligned radiographs.

INTRODUCTION

Although simulation studies have shown superior wear properties of metal-on-metal articulations, increased concern exists regarding the excess in-vivo wear of a small number of Metal-on-Metal-Hip-Resurfacing (MoMHRA) implants. Serum ion levels of Chromium (Cr) and Cobalt (Co) are surrogate markers of wear. Risk factors associated with increased wear include female gender, small components, dysplasia, cup orientation outside safe zone and femoral head downsize during surgery with an associated decrease in Head-Neck-Ratio (HNR). However, these factors are interlinked. This study aims to identify the factors that are most important for subsequent wear of MoMHRA, by performing a multivariate analysis.

METHODS

206 patients (124M: 82F) with unilateral MoMHRA were included in this study. The average follow up was 3.3 years. All patients had Cr/Co levels measured at follow up. Inclination and anteversion of each cup were measured using EBRA. Cups were analysed as being within or outside the previously defined optimum-zone. HNR measurements were made from pre-operative (HNRpre) and post-operative (HNRpost) radiographs. The immediate changes in HNR (downsize/upsize of femoral head) as a result of the operation were expressed as:

Multivariate linear regression modelling was used to explore the association between measures of ions with the following predictor variables (gender, age, diagnosis, femoral component size, orientation of the acetabular component, head/neck ratio and position of femoral stem). Analyses were carried out separately for each outcome (Cr and Co). Classification and Regression Tree (CART) models were fitted as a complimentary approach to regression modelling.

INTRODUCTION

The introduction of hard-on-hard bearings and the consequences of increased wear due to edge-loading have renewed interest in the importance of acetabular component orientation for implant survival and functional outcome following hip arthroplasty. Some studies have shown increased dislocation risk when the cup is mal-oriented which has led to the identification of a safe-zone1. The aims of this prospective, multi-centered study of primary total hip arthroplasty (THA) were to: 1. Identify factors that influence cup orientation and 2. Describe the effect of cup orientation on clinical outcome.

METHODS

In a prospective study involving seven UK centers, patients undergoing primary THA between January 1999 and January 2002 were recruited. All patients underwent detailed assessment pre-operatively as well as post-op. Assessment included data on patient demographics, clinical outcome, complications and further surgery/revision. 681 primary THAs had adequate radiographs for inclusion. 590 hips received cemented cups. The primary functional outcome measure of the study was the change between pre-operative and at latest follow up OHS (OHS). Secondary outcome measures included dislocation rate and revision surgery. EBRA was used to determine acetabular inclination and version.

The influence of patient's gender, BMI, surgeon's grade and approach on cup orientation was examined. Four different zones tested as possibly ± (Lewinnek Zone, Callanan's described zone and zones ± 5 and ±10 about the study's mean inclination and anteversion) for a reduced dislocation risk and an optimal functional outcome.

Introduction

Traditional TKR designs exhibit abnormal and unpredictable kinematics: with posterior subluxation in extension and anterior slide with flexion. These can contribute to restricted knee flexion and reduced quadriceps efficiency. Newer designs attempt to provide “guided motion” with the aim of mimicking normal knee kinematics. The Journey (Smith & Nephew) BCS TKR incorporates both an anterior and a posterior cam/post mechanism while Triathlon PS TKR (Stryker) incorporates a posterior cam/post mechanism. This study compares the in-vivo kinematics of these two designs and compares it with normal knee.

Methods

Knee kinematics of 10 patients with Journey-BCS TKR and 11 patients with Triathlon PS TKR; all with excellent clinical outcome (average age: 65) were analysed. Patients underwent fluoroscopic assessment of the knee during a step-up and deep knee bend exercise. 2D fluoroscopic images were recorded. Data was analysed for patella tendon angle (PTA) and contact points using a 3D model fitting technique. This data was compared to normal knee kinematics (n=20).

INTRODUCTION

Establishing a full-thickness cartilage in the lateral compartment and functionally intact ACL is vital before proceeding with unicompartmental knee replacement (UKR). The aim of this study is to assess whether MRI is a useful adjunct in predicting suitability for UKR, as compared to standard and stress radiographs.

METHODS

We identified 50 patients with a knee found suitable for UKR based on their standard and stress radiographs (full-thickness cartilage on lateral side). These patients underwent an additional cartilage-specific MRI scan to identify the status of ACL and the lateral compartment. The final decision regarding the suitability for UKR was based on the intra-operative observation.

STUDY PURPOSES

To evaluate the role of “top up” intra-articular local anaesthetic injection in patients who have had UKR.

METHOD AND RESULTS

43 patients scheduled to have a cemented Oxford UKR were prospectively recruited and randomised. All patients had the same initial anaesthetic regime of general anaesthesia, femoral nerve block and intra-operative intra-articular infiltration. All patients had a multi-holed epidural catheter placed intra-articularly prior to wound closure. Patients had the same operative technique, post operative rehabilitation and rescue analgesia.

An independent, blinded observer recorded post-operative pain scores using a visual analogue score every 6 hours and any rescue analgesia. On the morning after surgery, 22 patients, (Group I), received 20 mls of 0.5% bupivicaine through the catheter whilst 21, (Group II), patients had 20 mls of normal saline by the same observer, after which the catheter was removed.

No statistical difference was found in pain scores on the day of operation between the groups. However, patients in Group I had a significantly better pain score initially post top up and at 6 hours (2.4 (0-8) vs 5.7 (2-9), p<0.001). This cohort of patients required less rescue analgesia (p<0.001). In addition, Group I had statistically significant higher patient satisfaction outcome scores after the infiltration, (p<0.001).

OBJECTIVE

To examine the short term patient assessed functional results of the Journey BCS ¯(Smith & Nephew) and Triathlon ¯(Stryker Orthopaedics, Mahwah, NJ) total knee replacements when compared to the Scorpio ¯(Stryker) total knee replacement using a multi-surgeon case control design in a single centre.

METHODS

From September 2006 to August 2008 a total of 135 Journey and 97 Triathlon total knee replacements (TKR) were performed. 105 patients with Journey and 90 patients with Triathlon implants were available for follow-up at a minimum of 1 year, with an average of 2 years. Age and sex matched controls were obtained from our pool of patients who had had Scorpio TKR's. The same surgeons using the same approach operated on patients in both groups. All implants were posterior stabilised and all underwent patella resurfacing. All patients were seen pre-operatively and followed up post operatively in a physiotherapist led joint review clinic to assess range of motion (ROM) as well as function using the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), Oxford Knee Score (OKS) and the High Activity Arthroplasty Score (HAAS - used post operatively only).

Introduction

The results of the mobile bearing Oxford unicompartmental knee replacement (UKR) in the lateral compartment have been disappointing with a five year survival of 82%. Therefore, it is recommended that mobile bearings should not be used for lateral UKR. This low survivorship is primarily due to high dislocation rate, all occurring in the first year. A detailed analysis of the causes of bearing dislocation confirmed the elevated lateral tibial joint line to be a contributory factor. A new surgical technique was therefore introduced in which care was taken neither to remove too much bone from distal femur nor to over tighten the knee and thus ensure that the tibial joint line was not elevated. Other modifications to the technique were also introduced including use of a domed tibial component.

Aim

The aim of this study is to compare the outcome of these iterations: the original series [series I], Series II with improved surgical technique and the domed tibial component [Series III].

Introduction

Obesity has been considered a relative contra-indication in unicompartmental knee arthroplasty (UKA) due to fear of high wear rates, loosening and tibial collapse. The aim of this study was to investigate the impact of high body mass index (BMI) on ten-year survivorship and five-year functional outcome after Oxford UKA, a fully congruous mobile bearing design with large contact area and low wear rate.

Methods

This prospective study examines a consecutive series of 595 knees (mean age 66 years, range: 33-88) undergoing Oxford UKA with a minimum 5-year follow-up. Patients were divided into three groups; Group I (Normal body weight), BMI <25 (n=171), Group II (overweight), BMI 25- 30 (n=264), and Group III (Obese), BMI ≥30 (n=160). The survivorship and functional outcome (as assessed by change in Oxford Knee Score [DeltaOKS]) and Knee Society Score (KSS) for all three groups were compared.

INTRODUCTION

Mobile bearing unicompartmental knee replacement (UKR) is an accepted treatment for patients with isolated medial unicompartmental knee osteoarthritis (OA) with a full thickness cartilage loss. The aim of this study was to determine if this recommendation was correct and if the procedure could be used for partial-thickness cartilage loss.

METHODS

1053 Oxford medial UKRs were studied prospectively. The knees were divided into two groups; partial-thickness cartilage loss (PTCL) group and the full thickness-cartilage loss (FTCL) group. The primary outcome measure was the total Oxford Knee Score (OKS, 0 to 48) at the time of final follow up. The groups were also compared for the change in OKS (?OKS) and the proportion of patients that were considered to have benefited substantially from surgery (?OKS >5).

Purpose of Study

To assess the incidence of radiolucency in cemented and cementless Oxford unicompartmental knee replacement at two years.

Introduction

Most unicompartmental knee replacements (UKRs) employ cement for fixation of the prosthetic components. The information in the literature about the relative merits of cemented and cementless UKR is contradictory, with some favouring cementless fixation and others favouring cemented fixation. In addition, there is concern about the radiolucency that frequently develops beneath the tibial component with cemented fixation. The exact cause of the occurrence of radiolucency is unknown but it has been hypothesised that it may suggest suboptimal fixation.

Introduction

Kozinn and Scott have made recommendations about contra-indications for unicompartmental knee replacement (UKR). They suggest that patients younger than 60, weight > 82 kilograms, patients with exposed bone in patella-femoral compartment or patients who are physically active/perform heavy labour should not be offered a UKR. In addition, chondrocalcinosis is a contra-indication. These strict selection criteria are based on the experience with fixed bearing UKAs and are more intuitive than evidence based. The Oxford UKR has a fully congruous mobile bearing and has been shown to have minimal wear. Over the past 25 years, the Oxford Group has followed a standardised protocol for patient selection for UKR. We ignore patella-femoral joint pathology, chondrocalcinosis, patient's age, weight and activity level when deciding the suitability for UKR.

Methods

Using the standardised indications, more than 1100 Oxford UKRs have been performed to date over the last 10 years. These patients are assessed pre-operatively and at regular intervals post-operatively in a dedicated research clinic. We present the results of these consecutive cases. Patients were classified into two groups: group I (satisfy Kozinn-Scott recommendations) and group II (outside recommendations).

Introduction: Metal-on-metal-hip-resurfacing-arthroplasty (MoMHRA) has been associated with the development of inflammatory pseudotumours(IP), especially in females. IPs have been linked to wear debris, which can be related to metal-ion blood levels. Acetabular component position has been shown to influence wear. We have identified an optimum component orientation minimising IP risk around an inclination/anteversion of 40°/20°±10°. Our aim was to see if this optimal position results in lower metal ions and to identify the boundary of an optimal placement zone for low wear.

Methods: A cohort of 104 patients(60M:44F) with unilateral MoMHRA was studied. Blood tests were obtained at a mean follow up of 3.9 years and serum Co/Cr levels were measured(ICPMS). High metal ion concentrations were defined as Co> 4.1ppb and Cr> 5.2ppb. Radiographic cup inclination/anteversion were measured using EBRA. The differences in ion levels between different orientation zones were investigated. Three orientation zones were defined centered on 40°/20°: Z1-within ±5°, Z2-outside ±5°/within ±10° and Z3-within ±10°.

Results: There was a wide range of cup placements. Females had significantly (p< 0.001) smaller components(mean:51, 44–60) than males(mean: 56, 52–64). Cr levels, but not Co, were higher in females(p=0.002) and those with small femoral components(< 50mm, p =0.03). Patients with cups within Z1 (n=13) had significantly lower Co(p=0.005) and Cr(p=0.001). Males with cups within Z3(n=27) had lower ion levels in comparison to those outside, which were significantly lower for Co(p=0.049) but not Cr(p=0.084). Females had similar ion levels within and out of Z3(Cr/Co: p=0.83/0.84). Co levels were significantly lower in Z1(n=13) in comparison to Z2(n=33)(p=0.048) but Cr levels were not different (p=0.06).

Discussion: MoMHRA cups placed within ±5° of the optimum(40°/20°) had significantly lower metal ions indicating lower wear within this narrow zone. This safe zone, could extend to ±10° for males only. The narrower safe zone coupled with smaller components implanted are possible factors contributing to the increased IP incidence seen in females.

Introduction: Metal on metal hip resurfacing arthroplasty(MoMHRA) is an alternative option to THR in the treatment of young adults with OA. A recognised MoMHRA complication is the development of an inflammatory pseudotumour(IP). Diagnosis is made with the aid of US and/or MRI. To-date, no radiographic indication of the presence of IP has been identified. Neck thinning is a recognised phenomenon in MoMHRA hips not associated with any adverse clinical events. Its pathogenesis is considered multi-factorial. Our aim was to establish whether excessive neck narrowing is associated with the presence of a pseudotumour.

Methods: Twenty-seven hips (26 patients) with IP confirmed clinically, radiologically, intra-operatively and histologically were matched for sex, age, pre-operative diagnosis, component size and follow-up with an asymptomatic MoMHRA cohort (Control n=60). For all patients, prosthesis-neck-ratio(PNR) was measured on plain AP pelvic radiographs post-operatively and at follow-up as previously described and validated.

Results: All IP patients (4M:23F) and all (12M:48F) but two controls had a posterior approach at the time of MoMHRA. Post-operatively, there was no difference in the PNR between the two groups (p=0.19). At an average follow up of 3.5 years (range:0.7–8.3), IP patients(mean 1.26, 1.10–1.79) had a significantly higher (p< 0.0001) PNR in comparison to their controls(mean 1.14, 1.03–1.35). Greater neck narrowing occurred in both genders. IP necks had narrowed by an average of 8% (range:3–23). The degree of neck narrowing was correlated with length of survival of implant (p=0.001).

Discussion: This study shows a strong association between IP and neck narrowing. Processes such as impingement and increased wear are considered to be involved in the pathogenesis of both IP and neck narrowing. Furthermore, the presence of an IP, could lead to altered vascularity via a mass effect and further contribute to neck narrowing. Neck narrowing in symptomatic MOMHRA patients should alert surgeons of the possible presence of IP.

Metal on metal hip resurfacing (MMHR) is a popular procedure for the treatment of osteoarthritis in young patients. Several centres have observed masses, arising from around these devices, we call these inflammatory pseudotumours. They are locally invasive and may cause massive soft tissue destruction. The aim of this study was to determine the incidence and risk factors for pseudotumours that are serious enough to require revision surgery.

In out unit, 1,419 MMHRs were performed between June 1999 and November 2008. All revisions were identified, including all cases revised for pseudotumour. Pseudotumour diagnosis was made by histological examination of samples from revision. A Kaplan-Meier survival analysis was performed, Cox regression analysis was used to estimate the independent effects of different factors.

The revision rate for pseudotumour increased with time and was 4% (95% CI: 2.2% to 5.8%) at eight years. Female gender was a strong risk factor: at eight years the revision rate for pseudotumours in men was 0.5% (95% CI 0% to 1.1%), in women over 40 it was 6% (95% CI 2.3% to 10.1%) and in women under 40 it was 25% (95% CI 7.3% to 42.9%) (p< 0.001). Other factors associated with an increase in revision rate were, small components (p=0.003) and dysplasia (p=0.019), whereas implant type was not (p=0.156).

We recommend that resurfacings are undertaken with caution in women, especially those younger than 40 years of age, but they remain a good option in men. Further work is required to understand the patho-aetiology of pseudotumours so that this severe complication can be avoided.

Introduction: The results of the mobile bearing Oxford partial knee replacement (PKR) in the lateral compartment have been disappointing with a five year survival of 82%. Bearing dislocation is a particular concern, and to address this issue a new domed implant was introduced with a modified surgical technique. The aim of this study was to compare the risk of dislocation between a domed and flat lateral PKR.

Methods: Separate kinematic models were generated for the domed and flat bearings. The femoral component, tibial tray and bearing were aligned in a neutral position; the flat bearing was positioned centrally on the tibial tray and 2 mm from the side wall; the domed bearing was placed concentrically on the domed tibial tray. Dislocation in the Posterior (A-P), Lateral (M-L) and Medial against the tray wall (L-M-wall) were investigated. For each dislocation the tibial tray was restrained in all degrees of freedom (DOF) and the femoral component was restrained in five DOF; A-P and M-L displacements; A-P, M-L and Superior-Inferior (S-I) rotations. The bearing was restrained from rotating about the S-I axis for each dislocation. For the L-M-wall dislocation the underside of the bearing was held in contact with the tibial tray wall such that the lowest S-I displacement of the femoral component was achieved. The least amount of distraction required for bearing dislocation to occur was calculated for the seven bearing sizes available. The effect of medial-lateral positioning of the femur on dislocation was investigated.

Results: The minimum femur distraction to cause A-P flat and domed bearing dislocation ranged from 4.68mm to 3.91mm and 6.29mm to 5.59mm respectively as the bearing thickness increased from 3.5mm to 9.5mm. The minimum femur distraction to cause L-M-wall flat and domed bearing dislocation ranged from 3.42mm to 4.16mm and 4.55mm to 5.44mm respectively as the bearing thickness increased from 3.5 mm to 9.5 mm. The femur distraction required for L-M-wall bearing dislocation increased from 4.55mm to 6.3mm with a 2 mm medial movement of the femoral component. A 4 mm lateral movement of the femoral component decreased the distraction from 4.55mm to 2.35mm.

Discussion: A domed bearing can lead to an increased femoral distraction of between 25% and 37%, significantly reducing the likelihood of dislocation. This may be significant during everyday activities and demonstrates that the new domed design should reduce the incidence of bearing dislocation by increasing the amount of entrapment; our current series of 200 patients has no dislocations. Increasing the thickness of the bearing has a small effect on the distraction required to allow bearing dislocation. The medial-lateral placement of the femoral component has a pronounced effect on the femoral distraction required for bearing dislocation over the tray wall; medial placement of the femoral component is advisable.

Introduction: Partial Knee Replacement (PKR) is an appealing alternative to Total Knee Replacement (TKR) when the patient has isolated compartment osteoarthritis (OA). In nearly all cases there is a radiolucency observed between the tibial tray wall and the boney interface. The reasons why radiolucencies appear are unknown, but the bone will adapt to its altered mechanical environment by bone remodelling in accordance with ‘Wollf’s Law’. The aim of this study was to investigate the mechanical environment of the tibia bone adjacent to the tray wall, following cemented and cementless PKR, in order to determine whether this region of bone resorbs.

Methods: A validated finite element (FE) model of a cadaver tibia implanted with an Oxford PKR was used in this study. Kinematic data from fluoroscopy measurements during a step-up activity were used to determine the relative tibio-femoral positioning for the Oxford PKR model. Load data were adapted from the in-vivo measured loads using an instrumented implant during a step-up activity. The standard operating protocol was simulated for the Oxford PKR FE models, with the tibial tray implanted in a neutral position. The tibia was sectioned around the tray. Zone 7 was defined as parallel to the vertical tray wall, corresponding to the region on screened x-rays where radiolucencies are observed. It was assumed that the bone in the implanted tibia will attempt to normalise its stress-strain patterns locally to its equilibrium state, the intact tibia, for the same loading conditions. Forty patients (20 cemented, 20 cementless) who had undergone PKR were randomly selected from a database, and their screened x-rays assessed for radiolucency in region 7.

Results: The SED in region 7 was 80% lower in the cemented and cementless tibia, compared to the intact tibia (Figure 2). The maximum tensile stress was 63% lower in the cemented and cementless tibia, compared to the intact tibia. The corresponding maximum compressive stress was 52% lower. Radiolucency was observed in all forty radiographs in region 7.

Discussion: After implantation with a cemented or cementless PKR the bone strains and SED in region 7 are reduced. This reduction may provide the signal for adaptive bone remodelling and bone will be resorbed from this region, decreasing the volume and increasing the SED. Bone resorption will continue until the equilibrium state is reached. If a ‘lazy’ zone between 35% and 50% of the remodelling signal is considered, bone resorption will still occur due to the large decrease in SED for this region. For region 7 to return its SED to the equilibrium state, its volume will need to be reduced by 80%. This is likely to be the reason why a radiolucency is observed clinically in this region in almost every case, whether a cemented or cementless implant is used.

Introduction: Metal-on-metal hip resurfacing arthroplasty (MoMHRA) has gained popularity as an alternative to THR for younger patients with osteoarthritis. A growing concern has been the association of MoMHRA with the development of inflammatory pseudotumours (IP), especially in women. These have been linked to metal-on-metal wear, which can be related to metal ion concentrations. Elevated metal wear debris levels may result from impingement, rim contact and edge loading. Head-neck ratio (HNR) is a predetermining factor for range of movement and impingement. Neck thinning is a recognised phenomenon post-MoMHRA and we have found an association of IP with increased neck thinning based on a case control study. Our aims were to identify HNR changes a hip undergoes when resurfaced and at follow up; and whether greater neck thinning at follow-up could be associated with the presence of elevated metal ions.

Methods: A cohort of 91 patients (57M:34F) with unilateral MoMHRAs were included in this study. Blood tests were obtained at a mean follow up of 3.9 years (range 1.7–7 years) and serum (Co:Cr) ion levels were measured (ICPMS). High metal ion concentrations were defined as Co> 4.1ppb and Cr> 5.2ppb. For all patients, head-neck ratio (HNR) was measured on plain anterio-posterior pelvic radiographs pre-operatively, immediately post-operatively and at follow-up.

Results: Female patients had significantly bigger HNR pre-op (mean=1.35, range:1.22–1.64) compared to males(mean=1.22, range:1.05–1.38) (p< 0.01). Immediately post-op, female HNRs (mean: 1.26, range: 1.14–1.34) were not different to male patients(mean=1.24, range=1.11–1.38) (p=0.11). At follow-up HNR was once again significantly bigger (p< 0.01) in females (mean=1.35, range: 1.21–1.49), compare to males (mean=1.27, range:1.11–1.38). HNR alterations with operation (p=0.00) and at follow-up (p< 0.01) were significantly bigger in female patients. Furthermore, there was a significant correlation between high ion levels and HNR change at follow-up for both Co (p=0.02) and Cr (p< 0.01).

Conclusion: This study identified gender-specific changes in HNR that resurfaced hips undergo, not previously documented. Female hips have greater HNR pre-operatively, compared to male hips, and appear to be biomechanically disadvantaged when resurfaced. A decrease in HNR with resurfacing could result in impingement and lead to processes, known to be more prevalent in females, such as neck thinning, increased wear and IP development. In addition, we highlight a correlation between high ion levels and greater neck thinning at follow-up. Increased neck thinning in symptomatic MoMHRA hips could be secondary to increased wear and should be investigated further radiologically for the presence of IP.

IPs can be cystic (predominantly posterio-laterally located), solid (mostly anteriorly located) or mixed in nature. Diagnosis is made with the aid of US and/or MRI. To-date, no radiographic aid in the diagnosis of IP has been identified. Neck thinning is a recognised phenomenon following MoMHRA, occurring in up to 90% of resurfaced hips, which has not been associated with any adverse clinical events. Its pathogenesis is considered multi-factorial secondary to stress shielding, impingement, pressure effect on cancellous femoral neck, bone necrosis secondary to femoral preparation and altered vascularity/AVN. Our aim was to establish whether neck thinning is associated with the presence of a pseudotumour.

Methods: Thirty-one hips (30 patients) with IP confirmed clinically, radiologically, intra-operatively and histologically were matched for sex, age, pre-operative diagnosis, component size and follow-up with an asymptomatic MoMHRA cohort without pseudotumour (Control n=60). Radiological and operative findings at the time of revision of all IP patients were reviewed regarding location of pseudotumour; 4 different locations were defined: anteriorly-extending, posteriorly-extending, anteriorly & posteriorly-extending and within joint only. For all patients, prosthesis-neck ratio (PNR) at follow-up was measured on plain AP pelvic radiographs as previously described and validated.

Results: All IP patients (6M:24F) and all (12M:48F) but two controls had a posterior approach at the time of MoMHRA. Mean femoral component size was 46 mm for both groups. At an average follow up of 3.5 years (0.7–8.3), IP patients (mean 1.26, 1.10–1.79) had a significantly higher (p< 0.0001) PNR in comparison to their controls (mean 1.14, 1.03–1.35). Greater neck thinning had occurred in both IP-males (p< 0.001) and IP-females (p=0.002) in comparison to their controls. Location of IP and hence nature did not appear to have an effect on the degree of neck thinning.

Discussion: This study shows that IP patients had significantly narrower femoral necks at follow-up. Processes, such as impingement and increased wear that are thought to contribute to the process of neck narrowing are also thought to be factors in IP development. Furthermore, the presence of an IP, could lead to altered vascularity via a mass effect and further contribute to neck narrowing. Interestingly, nature of IP did not have a significantly affect PNR. Although one cannot be certain whether neck narrowing is a consequence or a contributing factor for IP development, their association is significant. Surgeons should consider the possibility of pseudotumour in symptomatic MoMHRA patients with neck narrowing.

Introduction: Metal-on-metal hip resurfacing arthroplasty (MoMHRA) has gained popularity as an alternative to THR for younger patients with osteoarthritis. A growing concern has been the association of MoMHRA with the development of inflammatory pseudotumours (IP), especially in women. These have been linked to metal-on-metal wear, which can be related to metal ion concentrations. Although cup orientation has been shown to influence wear, the optimum cup position has not been clearly defined. We have identified an optimal cup orientation to minimise IP risk, based on a case controlled study, for inclination/anteversion within ±10° of 40°/20°. Our aim was to see if this optimal position results in lower metal ions, and to identify the boundary of an optimal placement zone for low wear.

Methods: A cohort of 104 patients (60M: 44F) with unilateral MoMHRA was included in this study. Blood tests were obtained at a mean follow up of 3.9 years (range 1.7–7 years) and serum Co and Cr ion levels were measured (ICPMS). High metal ion concentrations were defined as Co> 4.1ppb and Cr> 5.2ppb. Radiographic cup inclination and anteversion were measured using EBRA. The differences in ion levels between different cup orientation zones were investigated. Three orientation zones were defined centered on the target orientation of 40°/20°: Z1 within ±5°, Z2 outside ±5°/within ±10° and Z3: within ±10°.

Results: There was a wide range of cup placements; mean inclination/anteversion were 46.3°(21.5°–64.6°)/15°(2.7°–35.6°). Cr levels, but not Co, were higher in female patients (p=0.002) and those with small femoral components (< 50mm, p =0.03).

For the whole cohort, there was no significant difference in ion levels (Cr: p=0.092. Co=0.075) between cups positioned within Z3 (n=58) versus those outside (n=46 mean). Male patients with cups within Z3 (n=27) had lower ion levels in comparison to those outside Z3, which were significantly lower for Co (p=0.049) but not Cr (p=0.084). Female patients had similar levels within and out of Z3 for both ions (Cr: p=0.83, Co: p=0.84). However, patients with cups within Z1 (n=13) had significantly lower Co (p=0.005) and Cr (p=0.001) than those outside Z1 (n=95). Interestingly, Co levels were significantly lower in Z1 (n=13) in comparison to Z2 (n=33) (p=0.048) but Cr levels were not different (p=0.06).

Discussion: MoMHRA cups placed with ±5° of the ideal position of 40°/20°gave rise to significantly lower metal ions indicating lower wear within this narrow zone, in both sexes. This safe zone, could be extended to ±10° for male patients only. Gender specific factors, such as pelvic anatomy and joint flexibility, could be responsible for the narrower ‘safe’ zone seen in females. The narrower safe zone coupled with smaller components implanted are factors contributing to higher ion levels and hence the increased incidence of IP seen in females.

Introduction: The development of Inflammatory Pseudotumour (IP) is a recognised complication following Metal on Metal Hip Resurfacing Arthroplasty (MoMHRA), thought to occur secondary to wear and elevated ion levels. Studies have shown that acetabular component orientation influences the wear of metal-on-metal hip replacement bearings. The aims of this study were to investigate the significance of cup orientation in the development of IP, and to identify a ‘safe-zone’ for cup placement with lower-risk for IP development.

Methods: Twenty six patients (n=27 hips) with IP confirmed radiologically, intra-operatively and histologically were matched for sex, age, pre-operative diagnosis, component size and follow-up with a cohort of asymptomatic MoMHRA patients (Control n=58). Radiographic acetabular anteversion and inclination were measured using EBRA. We calculated the distance in degree space of each acetabular component from the optimum position of 40° inclination and 20° anteversion, recommended by the designers, and thus compared acetabular component position between the two groups. Three different zones were tested as possibly optimum for acetabular placement. These were Lewinneck’s Zone (LZ) (inclination/anteversion; 30–50°/5–25°), and two zones defined by ±5° (Zone 1) or ± 10° (Zone 2) about the suggested target of 40°/20°. An optimal placement zone was determined based on a significant difference in IP incidence between components in the zone versus those outside.

Results: There was a wide range in cup orientations; mean inclination and anteversion were similar in the two groups: IP 47.5° (10.1°–80.6°)/14.1° (4.1°–33.6°) Vs Control 46.1° (28.8°–59.8°)/15.6° (4.3°–32.9°). Acetabular components in the IP group were significantly further away from the optimum position of 40°/20° in comparison to the controls (p=0.023). There was no difference in IP incidence between cups positioned within (IP:13/27, Control:35/58) or out of LZ (p=0.09) and within (IP: 2/27, Control: 10/58) or out of Zone 1 (p=0.156). Cups placed in Zone 2 (IP:6/27, Control:27/58) had significantly lower IP incidence versus those outside this zone (p=0.01). The odd’s ratio of developing IP when the cup is positioned out-of Zone 2 was 3.7.

Discussion: This study highlights the importance of ace-tabular component orientation in IP development. On the whole, patients with pseudotumour had acetabular components that were further away from the optimum position in comparison to the controls. However, a small number of IP patients had well-placed components implying that additional factors, possibly patient and/or gender specific, are involved in the development of pseudotumour. Furthermore, we defined an optimum, ‘safe-zone’ of ±10° around the cup position of 40°/20°. Patients with acetabular components outside this safe zone have an increased risk of IP development.

The results of mobile bearing Oxford unicompartmental knee replacement (UKR) in the lateral compartment have been disappointing (five-year survival: 82%). Therefore, it is recommended that mobile bearings should not be used for lateral UKR. This low survivorship is primarily due to a high dislocation rate. A detailed analysis confirmed the elevated lateral tibial joint line to be a contributory factor to bearing dislocation. A new surgical technique was therefore introduced in which care was taken neither to remove too much bone from the distal femur nor to over tighten the knee and therefore ensure that the tibial joint line was not elevated. Other modifications included use of a domed tibial component.

The aim of this study is to compare the outcome of these iterations: the original series (series I), those with improved surgical technique (series II) and the domed tibial component (series III). The primary outcome measure was bearing dislocation at one year. One year was chosen as all the dislocations in the first series occurred within a year. In the original series (n=53), implanted using a standard open approach, there were six dislocations in the first year, the average flexion 110°, and 95% had no/mild pain on activity. In the second series (n=65), there were 3 dislocations, the average flexion was 117°, and 80% had no/mild pain on activity.

In the third series with the modified technique and a convex domed tibial plateau, there was one dislocation, average flexion was 125° and 94% had no/mild pain on activity. At four years the cumulative primary dislocation rates were 10%, 5% and 0% respectively, and were significantly different (p=0.04).

The improved surgical technique and implant design has reduced dislocation rate to an acceptable level so a mobile bearing can now be recommended for lateral UKR.

About ten years ago we introduced sophisticated instrumentation and an increased range of component sizes for the Oxford unicompartmental knee replacement (UKR) to facilitate a minimally invasive surgical (MIS) approach. The device is now routinely implanted through an incision from the medial pole of the patella to the tibial tuberosity. This has resulted in a more rapid recovery and an improved functional result. As the access to the knee is limited there is a concern that the long term results may be compromised. The aim of this study was to determine the 10 year survival.

A prospective follow up of all Phase 3 minimally invasive Oxford UKR implanted by two senior authors (DWM & CAFD) has been undertaken. So far 1015 UKRs have been implanted for anteromedial osteoarthritis. All patients received a cemented implant through a MIS approach and were followed up prospectively by an independent observer. The data was collected prospectively regarding pre-operative status, complications and clinical as well as functional outcome at predetermined intervals.

The average age of patients was 66.4 years (range: 33 – 88) with mean Oxford Knee Score 41 (SD: 7.9) at the time of last follow up, Knee Society Score (objective) of 84 (SD: 13) and Knee Society Score (functional) of 83 (SD: 21). At ten years the survival of this cohort is 96%. There were 22 revisions including 7 for progression of arthritis, 5 for infection, 5 for bearing dislocation, 4 for unexplained pain and one for rupture of ACL secondary to trauma.

We conclude that the Oxford Knee can be implanted reliably through a minimally invasive approach, giving excellent long term results.

Introduction: Hoffa’s fat pad (HFP) of the knee is affected by a variety of tumours and tumour-like conditions. HFP can be affected by diffuse or solitary, focal disease. Solitary tumours are relatively uncommon but with widespread uptake of Magnetic Resonance Imaging Scans (MRI) an increasing number of Hoffa’s fat pad tumours (HFP) are being recognized.

Methods: This paper reports a consecutive series of 20 cases of solitary symptomatic HFP tumours referred to Oxford bone and soft tissue tumour service between 1999 and 2008. The commonest presenting symptom was anterior knee pain. All patients underwent open excision after diagnostic magnetic resonance imaging (MRI).

Results: Histology revealed varied diagnoses with the commonest being pigmented villonodular synovitis (PVNS) and ganglia. American Knee Society scores improved from 76 pre-operatively to 96 post-operatively with an improvement in functional scores from 92 to 100. In one patient, MRI identified the cause of hypo-phosphataemic osteomalacia as an HFP phosphaturic mesenchyma tumour despite the lack of local symptoms.

Discussion In conclusion the majority of solitary HFP tumours are benign and may be either cystic or solid. MRI and plain radiographs are the imaging of choice. The definitive treatment of both cystic and solid tumours should be selective arthrotomy and excision biopsy. Arthroscopic resection is not advised, as complete excision is not always possible. None of the 20 patients in this series had a malignant tumour but this has been reported in the literature. Calcification on plain radiographs may indicate a malignant lesion. All patients in our series reported substantial improvement in symptoms following open tumour resection.

Metal on Metal Hip Resurfacing Arthroplasty (MoMHRA) has gained popularity due to its perceived advantages of bone conservation and relative ease of revision to a conventional THR if it fails. Known MoMHRA-associated complications include femoral neck fracture, avascular necrosis/collapse of the femoral head/neck, aseptic loosening and soft tissue responses such as ALVAL and pseudotumours. This study’s aim was to assess the functional outcome of failed MoMHRA revised to THR and compare it with a matched cohort of primary THRs.

Method: We have revised 53 MoMHRA cases to THR; the reasons for revision were femoral neck fracture (Fracture Group, n=21), soft tissue reaction (Pseudotumour Group, n=16) and other causes (Other Group, n=16: loosening, AVN and infection). Average followup was 2.9 years. These MoMHRA revisions were compared with 103 matched controls from a primary THR cohort; matched for age, gender and length of followup. We compared, using the MannWhitney U test, operative time (OT, measured in minutes), and Oxford Hip Score (OHS) between the revised MoMHRA groups and their individual controls. We also compared, using the Kruskal Wallis test, UCLA Activity Score in the revised MoMHRA groups.

Results: There were no differences between the Fracture Group (mean OT 99.6, SD: 30.4; mean OHS 19.8, SD:9.2) and its controls (mean OT 95.9, SD: 31.8; mean OHS 17.3, SD: 7.5) nor between the Other Group (mean OT 129.4, SD: 36.7; mean OHS 22.2, SD: 9.4) and its controls (mean OT 104.4, SD: 39.2; mean OHS 20.3, SD: 10.1) in terms of OT and OHS. The Pseudotumour Group had significantly longer OT (mean 161.6, SD: 24.5, p< 0.001) and worse outcome (mean OHS 39.1, SD: 9.3, p< 0.001) than its controls (mean OT 113.1, SD: 51.7; mean OHS 20.0, SD: 9.2). In the Fracture Group, there were 3 infections requiring revisions. For the Pseudotumour Group, there were 3 recurrent dislocations, 1 femoral artery stenosis and 3 femoral nerve palsies. In the Other Group, there were 2 periprosthetic fractures. There was significant difference (p< 0.001) in UCLA scores between the MoMHRA groups. The Pseudotumour Group had the lowest mean UCLA score of 3.8 (SD: 1.89). The Fracture Group (mean: 7.0, S.D. 2.0) and the Other Group (mean: 6.7, S.D. 2.1) had similar UCLA scores.

Discussion: The results demonstrate that outcome after revision of MoMHRA is dependent upon the indication for revision. Patients revised for soft tissue reactions had significantly worse outcome. Patients with soft tissue reactions are more likely to experience complications and require further surgical intervention. The pseudotumour associated revisions were associated with a significantly prolonged OT. The overall complication rate for the study groups was quite high, with 11 (21%) revised MoMHRA cases experiencing a complication. The Pseudotumour Group had a higher complication rate (37%).

Introduction: Most unicompartmental knee replacements (UKRs) employ cement for fixation of the prosthetic components to the bone. The information in the literature about the relative merits of cemented and cementless UKR is contradictory, with some favouring cementless fixation while others favouring cemented fixation. There is concern about the radiolucency which frequently develops around the tibial component with cemented fixations. The exact cause of the occurrence of radiolucency is unknown but according to some, it may suggest suboptimal fixation.

Method: Following ethical approval, 62 patients with medial OA were randomised to receive either cemented (n=31) or cementless components (n=31). All patients underwent identical surgical procedure with either a cemented or cementless Oxford UKR. Patients were assessed clinically and radiologically. The x-rays were taken with an image intensifier (I.I.). The position of the I.I. was adjusted until it was perfectly aligned with the tibial bone-implant interface thereby allowing accurate assessment of presence and extent of the radiolucency.

Results: The patients in the two groups were well matched. There was no significant difference in the clinical scores between the two groups. The mean OKS for the cemented group was 40 (± 8.3) and 42 (± 4.6) for cementless group. Narrow radiolucent lines were seen at the bone-implant interfaces of 75% of the cemented tibial components; partial in 43% and complete in 32%. In the cementless implants, partial radiolucencies were seen in 7% and complete radiolucencies in none. The differences are statistically highly significant (p< 0.0001) and imply satisfactory bone ingrowth into the cementless implants.

Conclusions: The method of fixation influences the incidence of radiolucency. With identical designs, no patient with cementless components developed any complete radiolucency. The observation raises the question as to whether cementless rather than cemented components should be routinely used for UKR.

Purpose: The purpose of this study was to report the mid-term survival results of Oxford UKAs in patients of 50 years of age or less, using (1) revision surgery and (2) Oxford Knee Scores (OKS) as outcome measures.

Method: A literature review identified studies of Oxford mobile bearing UKAs containing individuals 1) 50 years old or less with 2) medial osteoarthritis and 3) 2 years or longer follow-up. Authors were approached to participate in a multi-centre survival analysis by submitting all their patients, 50 years of age or less, who received a medial UKA for osteoarthritis. Patients who had died, been lost to follow-up or who underwent revision were identified. OKS were established for all patients with surviving implants.

Results: Seven centres submitted 107 patients. The mean age was 47 years (range 32–50). The average follow-up was 4 years (range 1–25). Forty-seven patients had follow-up into their fifth year or longer. The cumulative 7-year survival using revision as the endpoint was 96% (CI 8). The mean post-operative OKS for surviving implants was 38 (CI 2) out of a possible 48.

Conclusion: While early survival rates and function are encouraging, long-term follow-up is required before concluding UKA is a viable treatment option in young patients with unicompartmental knee arthritis.

This retrospective study is aimed at assessing the functional outcome of failed MoMHRA revised to THR and comparing it with a matched cohort of primary THRs.

Method: Since 1999 we have revised 53 MoMHRA to THR. The reasons for revision were femoral neck fracture (Group A, n=21), pseudotumour (Group B, n=16) and other causes (Group C, n=16: loosening, avascular necrosis and infection). Average follow-up was 3 years months (1.2–7.3). These revisions were compared with 106 primary THRs which were age, gender and follow-up matched with the revision group in a ratio of 2:1.

Results: The mean Oxford Hip Score (OHS) was 20.1 (12–51) for group A, 39.1 (14– 56) for group B, 22.8 (12–39) for group C and 17.8 (12–45) for primary THR group. In group A, there were three infections requiring further revisions. In group B, there were three recurrent dislocations, three patients with femoral nerve palsy and one femoral artery stenosis. In group C, there were no complications. The differences in clinical and functional outcome between group B and the remaining groups as well as the difference in the outcome between group B and control group were statistically significant (p < 0.05).

Conclusions: THR for failed MoMHRA was associated with significantly more complications, operation time and need for blood transfusion for the pseudotumour group. In addition, the revisions secondary to pseudotumour also had significantly worse functional outcome when compared to other MoMHRA revisions or primary THR.

Purpose: This study was designed to establish the poly-ethylene wear rates in the Oxford medial unicompert-mental knee replacement.

Introduction: The Oxford meniscal bearing knee was introduced as a design to reduce polyethylene wear. There has been one previous retrieval study of the Oxford UKA, which reported very low wear rates in some specimens, but abnormal patterns of wear in others, including impingement. There has been no further investigation of these abnormal wear patterns.

Methods: Forty-seven bearings were retrieved from patients who had received a medial Oxford UKA for anteromedial osteoarthritis of the knee, none of which had previously been studied. Mean time to revision was 8.4 years (SD 4.1) and 20 had been implanted for over 10 years. The macroscopic pattern of polyethylene wear and the linear penetration (dial gauge measurement) was recorded for each bearing.

Results: The mean linear penetration rate (LPR) was 0.07mm/year. The patterns of wear fell into 4 categories, each with a different LPR; 1) No abnormal macroscopic appearance, n=16 (LPR = 0.01mm/year), 2) Abnormal macroscopic wear with extra-articular impingement, n=16 (LPR = 0.05mm/year), 3) Abnormal macroscopic wear with intra-articular impingement, n=6 (LPR = 0.10mm/year), 4) Abnormal macroscopic wear with impingement and signs of incongruous articulation, n=9 (LPR = 0.14mm/year). The differences in LPR were statistically significant (p< 0.05).

Conclusion: The results show that very low polyethylene wear rates are possible if the device functions normally. However if the bearing displays abnormal function (extra-articular, intra-articular impingement or incongruous articulation) wear rates increase significantly.

Introduction: Despite the satisfactory short-term implant survivorship of MoM hip resurfacing arthroplasty, symptomatic abnormal periprosthetic soft-tissue masses relating to the hip joint, ‘pseudotumours’, are being increasingly reported. These were found be locally destructive, requiring revision surgery in 75% of patients. Asymptomatic pseudotumours have not been previously investigated.

Methods: The aims were: (1) to investigate the prevalence of asymptomatic pseudotumours; and (2) to investigate their potential association with the level of metal ions. A total of 160 hips in 123 patients with a mean age 56 years (range 33–73) were evaluated at a mean follow-up of 61 months (range 13–88). Radiographs and OHS were assessed. Patients with a cystic or solid mass detected on the ultrasound/MRI had an aspiration or biopsy performed. Cobalt and chromium levels were analysed using Inductively-Coupled Plasma Spectrometer.

Results: Pseudotumours were found in 6 patients (5F: 1M). In 80% of bilateral cases, it was found in both sides. Histological examination showed extensive necrosis and diffuse lymphocyte infiltration. The presence of pseudotumour was associated with higher serum cobalt (9.2 μg/L vs. 1.9μg/L, p< 0.001) and chromium levels (12.0μg/L vs. 2.1μg/L, p< 0.001); higher hip aspirate cobalt (1182 μg/L vs. 86.2μg/L, p=0.003) and chromium levels (883μg/L vs. 114.8μg/L, p=0.006); and with inferior OHS (23 vs. 14 p=0.08).

Discussion: The prevalence of asymptomatic pseudotumour (5%) was higher than previously reported for the symptomatic pseudotumours (1%). There was a sixfold elevation of serum and a twelve-fold elevation of hip aspirate levels of cobalt and chromium in patients with pseudotumours. This suggests that pseudotumours may be a biological consequence of the large amount of metal debris generated in vivo. The association between pseudotumour and elevated metal ion levels might theoretically be explained by either systemic hypersensitivity responses to metal ions or local cytotoxic effects due to a high level of metal ions.

Introduction: Radiolucent lines (RLL) underneath the tibial component are common findings following the Oxford Uni-compartmental Knee Arthroplasty (OUKA)[1]. Many theories have been proposed to explain the cause of RLL, such as poor cementing, osteonecrosis, micromotion, and thermal necrosis, however, the true aetiology and clinical significance remain unclear. We undertook a retrospective study analysing the association between RLL and pre-operative, intra-operative factors, as well as clinical outcome scores.

Method: One hundred and sixty-one knees which had undergone primary Phase 3 medial Oxford OUKA were included in the study. Fluoroscopic radiography films were assessed at five years post-operatively for areas of tibial RLL. The presence of RLL was compared to

patients’ pre-operative demographics for age, weight, height, BMI,

intraoperative variables such as the operating surgeon (n=2), insert and component sizes, and

clinical assessment criteria including pre-operative and five-year post-operative Oxford knee (OKS) and Tegner (TS) scores.

Results: Of the 161 knees in the study, 126 (78%) were found to have tibial RLL. No statistical difference was found between knees with RLL and those without in terms of preoperative demographics, intra-operative factors, or clinical assessment criteria.

Discussion: No clear relationship between RLL, preoperative demographics, and intra-operative factors has been identified in this study. We conclude that tibial RLL following OUKA is a common finding but do not seem to affect medium term clinical outcome.

Introduction: Although resurfacing hip replacements are widely used there are few little independent outcome data to support this. The aim of this study was to report the 5 year clinical outcome and 7 year survival of an independent series.

Method: 610 Birmingham hip resurfacings were implanted in 532 patients with an average age of 51.8 years (range 16.5–81.6 years) and were followed for between 2 to 8 years; 120 of this series had minimum five year follow-up. Two patients were lost. There were 23 revisions, giving an overall survival of 95% (95% CI 85–99%) at seven years. Fractured neck of femur (n=13) was the most common reason for revision, followed by aseptic loosening (n=4). There were also 3 patients who had failures that were possibly related to metal debris. At a minimum of 5 year follow-up 93% had excellent or good outcome according to the Harris Hip Score. The mean Oxford Hip Score was 16.1 points (SD 7.7) and the mean UCLA activity score was 6.6 points (SD 1.9). There were no patients with definite evidence of radiographic loosening or greater than 10% of neck narrowing.

Discussion: The results demonstrate that with the Birmingham Hip Resurfacing, implanted using the extended posterior approach, the five year survival is similar or better to the reported survival rates for cemented and hybrid THR’s in young patients.

Conclusions: Considering these patients are young and active these results are good and support the use of resurfacing. However, further study is needed to address the early failures; particularly those related to fracture and metal debris.

Introduction: We report on a group of 20 metal-onmetal resurfaced hips (17 patients) presenting with a soft tissue mass associated with various symptoms; these masses we termed pseudotumours.

Methods: All patients underwent plane radiography; CT, MRI and ultrasound investigations were also performed for some patients. Where samples were available histology was performed. Metal ion levels were measured in six patients and one patient had the metal ion levels in the joint fluid measured.

Results: All patients in this series were female. Presentation was variable; the most common symptom was pain or discomfort in the hip region. Other symptoms included spontaneous dislocation, nerve palsy, a noticeable mass or a rash. In all cases a soft tissue mass was present in the region of the hip, this was either solid or cystic. The common histological features were extensive necrosis and lymphocytic infiltration. The blood cobalt and chromium levels varied considerably between the six patients that had these measurements. The median blood chromium level was 3.8 μg/L (range 0.8 to 23 μg/L) and that for cobalt was 11.5 μg/L (range 2.1 to 15 μg/L). The synovial fluid sample taken from a single joint contained much higher metal levels, 701 μg/L for chromium and 329 μg/L for cobalt. Twelve of the 20 cases have so far required revision to a conventional hip replacement.

Discussion: This complication is best imaged with ultrasound, and is not detected by normal xray. We estimate that about 1% of patients develop a pseudotumour in the first five postoperative years. The cause of these pseudotumours is unknown and is probably multifactorial, further work is required to define this; they may be manifestations of a metal sensitivity response. We are concerned that with time the incidence of these pseudo-tumours will increase.

Statement of purpose: The aim of this study is to evaluate different designs of unicompartmental knee replacement (UKR) by comparing the peak von Mises and contact stresses in polyethylene (PE) bearings over a step-up activity.

Summary of Methods: A validated finite element (FE) model was used in this study. Three UKR designs were modelled: a spherical femoral component with a spherical PE bearing (fully-congruent), a poly-radial femoral component with a concave PE bearing (semi-congruent), and a spherical femoral component with a flat bearing (non-congruent).

Kinematic data from in-vivo fluoroscopy measurements during a step-up activity was used to determine the relative tibial-femoral position as a function of knee flexion angle for each model. Medial and lateral force distribution was adapted from loads measured in-vivo with an instrumented implant during a step-up activity. The affect that varying the bearing thickness has on the stresses in the bearing was investigated. In addition, varus-valgus mal-alignment was investigated by rotating the femoral component through 10 degrees.

Summary of Results: Only the fully congruent bearing experienced peak von Mises and contact stresses below the PE lower fatigue limit (17MPa) for the step-up activity (fully congruent PE peak contact stress, 5MPa). The highest PE contact stresses were observed for the semi-congruent and non-congruent designs, which experienced approximately 3 times the PE lower fatigue limit. Peak PE von Mises stresses for the semi-congruent and non-congruent designs were similar, peaking at approximately 25MPa. Peak PE von Mises stresses were ameliorated with increased bearing thickness. Varus-valgus mal-alignment had little effect on the peak stresses in the three UKR designs.

Statement of Conclusions: Fully congruent articulating surfaces significantly reduce the peak contact stresses and von Mises stresses in the bearing. The FE model demonstrates that fully congruent bearings as thin as 2.5mm can be used without increasing the contact stresses significantly. Fully congruent designs can use thinner bearings and enable greater bone preservation.

Introduction: The results of the mobile bearing Oxford unicompartmental knee replacement (UKR) in the lateral compartment have been disappointing with a five year survival of 82%. Therefore, it is recommended that mobile bearings should not be used for lateral UKR. This low survivorship is primarily due to high dislocation rate, all occurring in the first year. A detailed analysis of the causes of bearing dislocation confirmed the elevated lateral tibial joint line to be a contributory factor. A new surgical technique was therefore introduced in which care was taken neither to remove too much bone from the distal femur nor to over tighten the knee and therefore ensure that the tibial joint line was not elevated. Other modifications to the technique were also introduced including use of a domed tibial component.

Aim: The aim of this study is to compare the outcome of these iterations: the original series [series I], Series II with improved surgical technique and the domed tibial component [Series III].

Method: The primary assessment of outcome was bearing dislocation at one year. One year was chosen as all the dislocations in the first series occurred within a year. In series I, there were 53 knees, in series II 65 knees and in series III 60 knees, all with a minimum of one year follow up.

Results: In series I, there were 6 bearing dislocations (11%) and the average range of movement (ROM) was 110°. In the second series, there were 2 dislocations (3%) and the average ROM was 118°. In the third series, there were no primary dislocations and the average ROM was 125°.

Conclusions: The improved surgical technique and implant design has reduced the dislocation rate to an acceptable level so a mobile bearing can now be recommended for lateral UKR.

We are concerned that with time the incidence of these pseudotumours will increase.

The purpose of this study was to determine if a single physiotherapy intervention would enable patients to kneel following Unicompartmental knee arthroplasty (UKA).

Kneeling is an important functional activity that is frequently not performed after knee arthroplasty, thus affecting a patient’s ability to carry out basic tasks of everyday life. There is however no clinical reason why patients should not kneel and many with proposed knee surgery ask about the possibility of kneeling after their operation.

Sixty adults participated in a prospective randomised controlled trial with blinded assessments. At 6 weeks post-operatively UKA patients were randomised to either the Routine care group where no advice on kneeling was given or to the Kneeling intervention group where participants were taught and given advice on how to kneel and were encouraged to do so. They were re-assessed at 1 year. The primary outcome measure was Question 7 of the Oxford Knee Score which asks the question “Could you kneel down and get up again afterwards?”

Pre-operatively there was no difference in the kneeling ability of the two groups. At 1 year the difference in kneeling ability between the two groups was highly significant (p< 0.05). Spearman’s correlation coefficient showed no significant association between a change in score of Question 7 at 1 year and the following factors; scar position, numbness, range of flexion, arthritic involvement of other joints and pain. Linear regression analysis also confirmed that these factors were not successful in predicting a change in kneeling ability.

This study showed that the single factor predictive of kneeling ability was the physiotherapy intervention provided at 6 weeks post-operatively and it is suggested that kneeling should be incorporated into patient’s post-operative rehabilitation programmes.

Introduction: Treatment options for the young active patient with isolated symptomatic medial compartment OA and pre-existing ACL deficiency are limited. Implant longevity and activity levels may preclude TKA, whilst HTO and unicompartmentasl knee arythroplasty (UKA) are unreliable due to ligamentous instability. UKAs tend to fail because of wear or tibial loosening resulting from eccentric loading. Combined UKA and ACL reconstruction may therefore be a solution.

Method: Fifteen patients with combined ACL reconstruction and Oxford UKA (ACLR group), were matched (age, gender and follow-up period) with 15 patients with Oxford UKA with intact ACL (ACLI group). Prospectively collected clinical and x-ray data from the last follow-up (minimum 3 years, range: 3 – 5) were compared. Ten patients from each group also underwent in-vivo kinematic assessment using a standardised protocol.

Results: At the last follow-up, the clinical outcome for the two groups were similar. One ACLR patient needed revision due to infection. Radiological assessment did not show any significant difference between relative component positions and none of the patients had pathological radiolucencies suggestive of component loosening. Kinematic assessment showed posterior placement of the femur on tibia in extension for the ACLR group, which corrected with further flexion.

Conclusions: The short-term clinical results of combined ACL reconstruction and UKA are excellent. Lack of pathological radiolucencies and near normal knee kinematics suggest that early tibial loosening due to eccentric loading is unlikely.

Aim: The aim of this study was to asses the accuracy of skyline radiographs in the assessment of the patellofemoral joint, when compared to open intraoperative assessment.

Methods: Eighty nine patients undergoing knee replacement surgery were included in the study. Skyline radiographs were obtained preoperatively. These radiographs were assessed and graded by an experienced musculoskeletal radiologist using the Altman and Ahlbäck classifications. The grades were calculated for both the medial and lateral facets of the PFJ. Intraoperative assessment of the Patellofemoral joint was undertaken at the time of surgery. The damage was graded using the modified Collins classification (0: Normal, 1: Superficial damage, 2: Partial thickness cartilage loss, 3: Focal Full thickness cartilage loss < 2cm2, 4: Extensive full thickness cartilage loss < 2cm2). Data was obtained for the Medial Facet, Lateral Facet and Trochlea.

Results: Spearman’s rank correlation coefficient between the radiographic and macroscopic changes within the lateral PFJ were poor with both the Altman 0.22 (p=0.0350) and Ahlbäck 0.24 (p=0.018). The correlation of the medial PFJ was slightly better with a coefficient for Altman 0.42 (P< 0.0001) and Ahlbäck 0.34 (P> 0.001).

Conclusion: In conclusion skyline radiographs provide a poor to moderate preoperative assessment of the degree of osteoarthritis within the patella-femoral joint. This has significant implications for establishing radiographic criteria for planning patella-femoral joint replacement.

Purpose: The aim of this study is to compare the long-term survival results of TKA in patients under the age of 60, using

revision surgery and

poor functional outcome as the end-points.

Method: From our knee database we identified a cohort of 60 total knee replacements that had been performed over 15 years previously. We identified those who had died, those who had been revised and established the Oxford Knee Score (OKS) for all those still surviving.

Results Using the following endpoint criteria the cumulative 15-year survival was (A) revision surgery alone = 78% (CI 12), (B) revision surgery or an OKS less than or equal to 24 (50% of total OKS) = 63% (CI 13), and (C) revision surgery or moderate pain = 48% (CI 14).

Conclusion The functional survival of TKA in patients under the age of 60 decreases in the second decade following implantation with a significant number of prostheses failing the patient due to knee pain

Introduction: The information in the literature about the relative merits of cemented and cementless unicompartmental knee replacement (UKR) is contradictory, with some favouring cementless fixation while others favouring cemented fixation. Cemented fixations give good survivorship but there is concern about the radiolucency which frequently develops around the tibial component. The exact cause of the occurrence of radiolucency is unknown but according to some, it may suggest suboptimal fixation.

Method: Sixty-two knees (31 in each group) were randomised to receive either cemented or cementless UKR components. The components were similar except that the cementless had a porous titanium and hydroxyappatite (HA) coating. Patients were prospectively assessed by an independent observer pre-operatively and annually thereafter. The clinical assessment included Oxford Knee Score, Knee Society Scores and Tegner activity score. Fluoroscopically aligned radiographs were assessed for thickness and extent of radiolucency under the tibial implant.

Results: At one year there were no differences in the clinical outcome between the groups and there were no loose components. No radiolucencies thicker than 1mm were seen. At one year none of the cementless tibias and 30% of the cemented tibias had complete radiolucencies. One out of 31 cementless (3%) and 12 out of 31 cemented (39%) had partial radiolucencies. This difference between these two groups was high significant (p< 0 0001).

Conclusions: This study clearly demonstrates that the incidence of radiolucency beneath the tibial component is influenced by component design and method of fixation. With identical designs of tibial component none of the cementless components developed complete radiolucences whereas 30% of the cemented components did. We conclude that HA achieves better bone integration than cement.

Introduction: The Oxford unicompartmental knee replacement (UKR) use in the lateral compartment has been associated with a reduced flexion range and increased medial compartment pain than seen with its medial counterpart due to, in part, the inadequacy of a flat tibial tray replacing the domed anatomy of the lateral tibia. A new design incorporating a domed tibial component and a biconcave meniscal bearing has been developed to overcome these problems.

This study reports a clinical comparison of new and old establishing whether this modified implant has maintained the established normal kinematic profile of the Oxford UKR.

Method: Patients undergoing lateral UKR for OA were recruited for the study. Fifty one patients who underwent UKR with the domed design were compared to 60 patients who had lateral UKR with a flat inferior bearing surface. Kinematic evaluation was performed on 3 equal subgroups (n = 20); Group 1-Normal volunteer knees, Group 2-Flat Oxford Lateral UKR’s and Group 3-Domed Oxford Lateral UKR’s. The sagittal plane kinematics of each knee was assessed using videofluoroscopic analysis whilst performing a step up and deep knee bend activity. The fluoroscopic images were recorded digitally, corrected for distortion using a global correction method and analysed using specially developed software to identify the anatomical landmarks needed to determine the Patella Tendon Angle (PTA) (the angle the patella tendon and the tibial axis).

Knee kinematics were assessed by analysing the movement of the femur relative to the tibia using the PTA.

Results: PTA/KFA values, for both devices, from extension to flexion did not show any significant difference in PTA values in comparison to the normals as measured by a 3-way ANOVA. The Domed implant achieved higher maximal active flexion during the lunge exercise than those with a flat implant. Only 33% of the flat UKR’s achieved KFA of 130° or more under load whilst performing a lunge, compared with 75% of domed UKR’s and 90% of normal knees. No flat UKR achieved a KFA of 140° or more, yet 50% of all domed UKR’s did, as did 60% of all normal knees.

Conclusions: There was no significant difference in sagittal plane kinematics of the domed and flat Oxford UKR’s. Both designs had favorable kinematic profiles closely resembling that of the normal knee, suggesting normal function of the cruciate mechanism. The domed knees had a greater range of motion under load compared to the flats, approaching levels seen with the normal knee, suggesting that limited flexion for the flat plateau results from over tightening in high flexion and that this is corrected with the domed plateau. Problems with the second generation of lateral Oxford UKA have been rectified by a new bi-concave bearing without losing bearing stability and normal kinematics.

Introduction: Metal-on-metal hip resurfacing arthroplasty (HRA) is currently recommended for younger, active patients with high expectations but information about outcomes is limited. Reports concentrate on wear, fracture rates and radiographic appearance, rather than function. Studies that report function do not describe rehabilitation protocols. This data is important to discussions about likely outcomes and restrictions prior to consent and to fully evaluate HRA.

Methods: Consecutive Conserve HRA operations were reviewed 1 year post-surgery. Function was assessed using 3 validated questionnaires; the OHS (Oxford Hip Score), HOOS (Hip Disability and Osteoarthritis Outcome Score) and UCLA Activity Scale. Complications, pain, ROM, muscle strength, single leg stand, walking and stair climbing ability were recorded.

Results: 125 HRA were reviewed (68 right, 57 left hips) in 120 patients (71 male, 49 female) of mean age 56 years. 86.7% recorded no complications, but 20% had pain at 3 months. The median OHS was 15, median UCLA 7 (active) and mean HOOS 82.78%. Operated hip flexors, extensors and abductors were weaker (p=0.000) and hip flexion ROM a mean 94.46 ± 12.71 (55–120) degrees. For 25% walking was limited, 7.6% needed a stick and 10% a stair rail. The OHS correlated with HOOS pain subscale (r=0.812, p=0.000), flexion ROM (r=0.426, p=0.000), hip extensor (r=0.359, p=0.000) and abductor (r=0.424, p=0.000) strength. Pain at 3 months correlated with the HOOS pain subscale (p=0.000, r= 0.503).

Discussion: Although outcomes were generally good with few complications, high levels of function and activity 25% had poor outcomes; with pain, restricted hip flexion, decreased strength, limited walking and functional problems, particularly putting on socks. Pain present at 3 months was associated with pain and worse function at 1 year. It is postulated this sub-optimal recovery may be related to current rehabilitation protocols adopted from THA and not tailored to HRA.

Background: The Oxford unicompartmental knee replacement (UKR) use in the lateral compartment has been associated with a reduced flexion range, increased medial compartment pain and a higher dislocation rate than seen with its medial counterpart due to the inadequacy of a flat tibial tray replacing the domed anatomy of the lateral tibia. A new design incorporating a domed tibial component and a biconcave meniscal bearing has been developed to overcome these problems. This current study was designed to establish whether this modi-fied ‘domed’ implant has maintained the established normal kinematic profile of the Oxford UKR.

Methods: The study population consisted of 60 participants from three equal groups; Group 1- Normal volunteer knees (n = 20), Group 2 – Flat Oxford Lateral UKR’s (n = 20) and Group 3 – Domed Oxford Lateral UKR’s (n = 20). The sagittal plane kinematics of each involved knee was assessed continuously using videofluoroscopic analysis. A standardised protocol of step-up and deep lunge was used to assess loadbearing range of motion during which the patella tendon angle (PTA) was measured as a function of the knee flexion angle (KFA).

Results: PTA/KFA values compared at 10 degree KFA increments from maximal extension to maximal flexion for all 3 groups did not demonstrate any statistically significant difference in PTA values between any group as measured by a 3-way ANOVA. The Domed implant achieved higher maximal active flexion during the lunge exercise than those with a Flat implant. Only 33% of the Flat UKR’s achieved KFA of 130 degrees or more under load whilst performing a lunge, compared with 75% of domed UKR’s and 90% of normal knees. No Flat UKR achieved a KFA of 140 degrees or more, yet 50% of all domed UKR’s did, as also did 60% of all normal knees.

Conclusions: There is no significant difference in the sagittal plane kinematics of the domed and flat Oxford UKR’s. Both implant designs have a favourable kinematic profile closely resembling the normal knee. The domed knees though do have a greater range of motion under load as compared to the flats, approaching levels seen with the normal knee.

Purpose of study: We report the results of a prospective case series of 10 patients who developed tumour-like masses following resurfacing arthroplasty

Method: Ten subjects were referred to the tumour service at the Nuffield Orthopaedic Centre with symptomatic masses around the hip, all had previously received a resurfacing arthroplasty.

We report the clinical, radiographic and histologic features of these cases.

Results: MRI and ultrasound scanning was preformed, which demonstrated masses with solid and cystic components.

Biopsy was performed and subsequent histological examination revealed a profound plasma-cell lymphocytic response associated with metal wear debris.

There were no infections in this series.

Three subjects required revision surgery.

Conclusion: Over 50,000 resurfacing arthroplasties have been implanted worldwide over the past ten years. Although the early clinical results are encouraging little is known about the long term consequences of large head metal on metal bearing surfaces. Despite this, these devices are being widely marketed and are often implanted in younger patients. Resurfacing arthroplasties are associated with high serum and urine metal ion concentrations, metal particles have also been shown to migrate along the lymphatic system. In addition, there is now evidence that high local metal ion concentrations can induce haempoietic cancers.

This study suggests that resurfacing arthoplasty can also induce a local hypersensitivity reaction in response to metal wear debris. It therefore raises new concerns regarding the long-term safety of this procedure.

Introduction: Restoration of predictable and normal knee kinematics after a TKR can improve the patient’s function. Traditional designs exhibit grossly abnormal kinematics with the femur subluxing posteriorly in extension and a paradoxical forward slide in flexion. In addition, the kinematics are very variable. Newer designs were intended to overcome these problems, owing to their ability to provide ‘guided motion’ of the components. The medial pivot knee uses a specifically designed articulating surface constraining the femoral component to externally rotate about an axis through the medial compartment.

This study assesses the functional in vivo kinematics of Advanced Medial Pivot (AMP) TKR and compares it to kinematics of the normal knee.

Methods: Thirteen patients with pre-operative diagnosis of primary osteoarthritis, who had undergone a knee replacement with the AMP knee at least one-year prior were recruited in this study. All had an excellent clinical outcome (as assessed by AKSS) and underwent fluoro-scopic analysis whilst performing a step up activity. Knee kinematics were assessed by analysing the movement of the femur relative to the tibia using the Patella Tendon Angle (PTA) through the range of knee flexion. This data was compared to that of thirteen normal knees.

Results: The PTA for the normal knee has a linear relationship with knee flexion. The PTA is 14 degrees in full extension and decreases to -10 degrees at 100 degrees knee flexion during a step-up exercise. Between extension and 60 degrees of knee flexion, no significant difference was found between the PTA for the normal knee and for the AMP. The PTA for AMP is significantly higher for values of knee flexion exceeding 60 degrees. The standard deviation for different values of knee flex-ion is similar to that seen in the normal knee.

Conclusions: In extension, the PTA is near normal but in flexion PTA is higher than normal suggesting that the femur is too anterior. The variability of the kinematics for AMP TKR is similar to that of the normal knee and is better than that of most other knee designs that we have studied in the past, indicating that it is a stable TKR.

Introduction: The cam-post mechanism of Posterior Stabilized Total Knee Arthroplasty (PS-TKA) should provide a constraint that limits anterior translation of the femur on the tibia in flexion and thereby ensure femoral roll-back with progressive knee flexion. In a previous fluoroscopic study we showed that the sagittal plane kinematics of a PCL substituting TKA (Scorpio PS) was abnormal in flexion, suggesting inefficiency of the cam-post mechanism. We also assessed the movement of the femur relative to the tibia using the Patella Tendon Angle (PTA) through the range of knee flexion (0 to 90 degrees). The aim of the current study was to investigate in greater detail why the cam-post mechanism was ineffective by assessing the contact point movement and the distance between the cam and post.

Method: Twelve patients with Scorpio PS TKA underwent fluoroscopic assessment of the knee during a step up exercise and a weight bearing deep knee bend. The image distortion was corrected using a global correction method and the data was analysed using a 3D model fitting technique. Having determined the component position, the minimum distance between cam and post were determined. The femoro-tibial contact positions of the medial and lateral condyles were determined relative to the mid-coronal plane of the tibial component. The PTA was calculated by measuring the angle subtended by patella tendon with the tibial axis and was plotted against knee flexion angle (KFA).

Results: The relationship between PTA and KFA was abnormal relative to the normal knee. Between extension and 60 degrees flexion there was forward movement of both medial (11 mm) and lateral (5 mm) femoral condyles. Thereafter, both condyles moved back (10 mm). The cam-post mechanism failed to engage in one case while in others it engaged between 70 to 100 degrees.

Conclusions: The 3D analysis has confirmed the preliminary findings of the previous study using the PTA and KFA relationship. Despite the cam engaging in flexion normal knee kinematics were not restored. The femoral roll-back is inadequate and starts to occur at least 20 degrees before the cam and post engage.

The purpose of the study: to determine if Patient height-and gender could be used to predict component size With a minimally invasiveapproach for unicompartmental knee replacement.

Material and methods: One hundred x-rays of patients (44 men, 56 women), who had undergone Oxford UKR, were reviewed. The preoperative radiographs were assessed for component size using the standard template. The postoperative x-rays were reviewed to determine-whether the ideal component size had been used or if not what could be the most appropriate. Patient’s height was recorded. The proportion of patients for whom an appropriate size could be selected by either template or height measurements was calculated.

Current templating system accurately predicted the ideal size in 67%. In no case was the size incorrect by more than one size. The following size bands were set according to height. For men: size small in patients less than160 cm, medium less than 170 cm and large less than 180cm. For women: size small in patients less than 165 cm, medium less than 175cm and large less than 185 cm. Height accurately predicted the ideal size in75%. In no case was the assessment of component size incorrect by more than one size.

As the Oxford femoral component is spherical, its size is not critical and it is acceptable to use one size too large or too small. Both height and templating safely predicted an acceptable size in all cases and predicted the ideal size in about 70% > Conclusion: Gender specific height should be used to predict the component size in situations were templating is difficult as in digital x-rays orsuperimposition of the two femoral condyles, and non-standardised x-raymagnification.

The indications for unicompartmental knee arthroplasty (UKA) remain controversial; in particular the threshold of disease in the patellofemoral compartment is debated. Whilst some authorities ignore the condition of the patellofemoral joint, others consider pre-existing patellofemoral osteoarthritis (PFOA) a contra-indication to UKA. The aim of this study was to determine the influence of PFOA on the outcome of medial UKA.

This prospective study involved one hundred consecutive patients who had undergone cemented medial Oxford UKA (phase 3), via a minimally invasive approach, at least one year previously. Patients were divided into two groups according to the presence or absence of full thickness cartilage loss (FTCL) on the patella or trochlea at operation. A pre-operative skyline radiograph was graded using the Altman score, by an independent Musculoskeletal Radiologist. Outcome was evaluated with the Knee Society Score (AKSS) and the Oxford Knee Score (OKS, maximum 48). Groups were compared for differences in knee score and Altman grade using a one way ANOVA. Repeat analysis was performed using the presence of anterior knee pain (AKP) as the group defining variable.

There were 28 patients with FTCL, and both groups were well matched for age, gender and activity levels. Analysis showed no significant difference in post operative knee scores between groups with either the presence of FTCL or the presence of AKP pre-operatively as a factor. There was no significant difference in Altman grade between groups.

Intra-operative evidence of PFOA in patients with medial compartment osteoarthritis does not prejudice the outcome of UKA. Even the inclusion of patients with symptomatic AKP, without necessarily having PFOA, does not affect the outcome after UKA. These short results are encouraging, but longer follow up is required.
