Patient reported outcome measures (PROMs) after total knee arthroplasty (TKA) are typically used to assess longitudinal change in pain and function after total knee arthroplasty (TKA). The Patient's Knee Implant Performance (PKIP) score was developed to evaluate outcomes more broadly including function, stability, confidence, and satisfaction. Although validated in patients having a primary TKA, the PKIP has not been evaluated as an outcome measure for patients having revision TKA. This study examined patient outcomes at one year following primary and revision TKA measured using the PKIP, compared to Oxford Knee Scores.

A retrospective analysis of pre-operative and one-year post-operative outcomes was completed for 39 patients (21 female) who had primary (n=27) or revision (n=12) TKA with a single surgeon between 2017 and 2020. The mean age was 69.2±7.4 years, and mean weight 87.4± 5.1kg. The change over time and correlation between the self-reported outcome measures was evaluated.

There was a significant improvement in the PKIP overall score at the 12-month follow-up (32± 13 v 69± 15, p= <0.001), with no significant difference between groups (3.3 points, p=0.50). Among the PKIP sub-scores, there was a significant improvement in knee confidence (3.5±2 vs 7.7±2; p<0.001), stability (3.4±2 vs 7.4±3; p<0.001) and satisfaction (2.5±1.7 vs 6.6±3, p<0.001). Between group differences in PKIP sub-scores one year after surgery were small and non-significant. For all patients, the OKS and PKIP scores were moderately correlated before surgery (r=0.64, p=<0.05), and at 1 year after surgery (r=0.61, p= <0.001).

Significant improvements in knee confidence, stability, and satisfaction one year after TKA were identified from the PKIP responses, with no significant difference between primary and revision surgery. The moderate correlation with the OKS suggests these questionnaires measure difference constructs and may provide complementary outcome information in this patient cohort.

Clinical success of prostheses in joint arthroplasty is ultimately determined by survivorship and patient satisfaction. The purpose of this study was to compare (non-inferiority) a new morphometric designed stem for total hip arthroplasty (THA) against an established comparator.

A prospective randomised multi-centre study of 144 primary cementless THA performed by nine experienced orthopaedic surgeons was completed (70 received a fully coated collarless tapered stem and 74 received a morphometric designed proximally coated tapered stem). PROMs and blood serum markers were assessed preoperatively and at intervals up to 2-years postoperatively. In addition, measures of femoral stem fit, fill and subsidence at 2-years post-operatively were measured from radiographs by three observers, with an intra-class correlation coefficient of 0.918. A mixed effects model was employed to compare the two prosthesis over the study period. A p-value <0.05 was considered statistically significant.

Demographics, Dorr types and blood serum markers were similar between groups. Both stems demonstrated a significant improvement in PROMs between the pre- and post-operative measurements, with no difference at any timepoint (p > 0.05). The fully coated tapered collarless femoral stem had a non-significantly higher intra-operative femoral fracture rate (5.8% vs 1.4%, p = 0.24), with all patients treated with cable fixation and partial weight bearing. The mean subsidence at 2-years was 2.5mm +/− 2.3mm for the morphometric stem and 2.4mm +/− 1.8mm for the fully coated tapered collarless femoral stem (p = 0.879). There was one outlier in each group with increased subsidence (fully coated tapered collarless femoral stem 6.9mm, morphometric wedge stem 7.4mm), with both patients reporting thigh pain at 2 years.

When compared with an established stem, the newer designed morphometric wedge stem performed well with comparable radiological and PROM outcomes at 2 year follow up. Continued follow-up is required for long term benchmarking.

This study aimed to analyze the effect of two different techniques of cement application: cement on bone surface (CoB) versus cement on bone surface and implant surface (CoBaI) on the short-term effect of radiolucent lines (RLL) in primary fully cemented total knee arthroplasties (TKA) with patella resurfacing.

379 fully cemented TKAs (318 patients) were included in this monocentric study. Preoperative and postoperative at week 4 and 12 month after surgery all patients had a clinical and radiological examination and were administered the Oxford Knee Score (OKS). Cement was applied in two different ways among the two study groups: cement on bone surface (CoB group) or cement on bone surface and implant surface (CoBaI group). The evaluation of the presence of RLL or osteolysis was done as previously described using the updated Knee Society Radiographic Evaluation System.

The mean OKS and range of motion improved significantly in both groups at the 4-week and 12-month follow-up, with no significant difference between the groups (CoB vs. CoBaI). RLL were present in 4.7% in the whole study population and were significantly higher in the CoBaI group (10.5%) at the 4-week follow-up. At the 12-month follow-up RLL were seen in 29.8% of the TKAs in the CoBaI group, whereas the incidence was lower in the CoB group (24.0% (n.s.)). There were two revisions in each group. None of these due to aseptic loosening.

Our study indicated that the application of bone cement on bone surface only might be more beneficial than onto the bone surface and onto the implant surface as well in respect to the short-term presence of RLL in fully cemented primary TKA.

The long-term results will be of interest, especially in respect to aseptic loosening and might guide future directions of bone cement applications in TKA.

Total Knee Arthroplasty (TKA) is an effective surgery performed for knee osteoarthritis. Despite this success up to 20% of patients are less than fully satisfied. Rotating platform (RP) bearings have demonstrated significant reduction in rates of wear in laboratory studies but thus far have failed to demonstrate a clinical difference compared to fixed bearing (FB) arthroplasty in patients. This may be due to studies limited by small sample size and single centre design. Additionally, no studies have utilised modern patient-reported outcome measures (PROMs) such as PKIP to investigate differences between these bearings.

A non-randomised, prospective, multi-centre study was conducted across centres in Australia, United Kingdom and the United States from November 2012 to May 2015. Knee prosthesis included were the RP and FB models of the PFC Sigma Knee (Depuy, Synthes) and the ATTUNE Knee (Depuy, Synthes). Data was collected on pre-arthroplasty interventions, demographics and PROMs. PROMs included were the KOOS, OKS, EQ53DL and PKIP. Participants were followed-up for 2 years.

A total of 1,718 patients were recruited across 27 centres. 959 participants underwent a FB TKA, 759 participants underwent a RP TKA. No significant difference was found between RP and FB in change from baseline at 2 years across any parameter in all PROMs investigated at 2 years. Body Mass Index pre-operatively was the biggest indicator of change from baseline at 2 years.

Our data demonstrates similar improvements in the short-term in both RP and TKA across all PROMs. Patients reported similar scores in satisfaction, confidence, mobility and stability in the PKIP PROM for both RP and FB groups. We have not found any evidence to suggest that RP TKA is disadvantageous at 2 years, and this is reassuring for any surgeon looking to utilise this bearing type in practice.

Our data does not demonstrate a clinical advantage in RP TKA over FB TKA at 2 years in any PROM.

Hip resurfacing arthroplasty (HRA) is a bone conserving alternative to total hip arthroplasty. We present the early 1 and 2-year clinical and radiographical follow-up of a novel ceramic-on-ceramic (CoC) HRA in a multi-centric Australian cohort.

Patient undergoing HRA between September 2018 and April 2021 were prospectively included. Patient-reported outcome measures (PROMS) in the form of the Forgotten Joint Score (FJS), HOOS Jr, WOMAC, Oxford Hip Score (OHS) and UCLA Activity Score were collected preoperatively and at 1- and 2-years post-operation. Serial radiographs were assessed for migration, component alignment, evidence of osteolysis/loosening and heterotopic ossification formation.

209 patients were identified of which 106 reached 2-year follow-up. Of these, 187 completed PROMS at 1 year and 90 at 2 years. There was significant improvement in HOOS (p< 0.001) and OHS (p< 0.001) between the pre-operative, 1-year and 2-years outcomes. Patients also reported improved pain (p<0.001), function (p<0.001) and reduced stiffness (p<0.001) as measured by the WOMAC score. Patients had improved activity scores on the UCLA Active Score (p<0.001) with 53% reporting return to impact activity at 2 years. FJS at 1 and 2-years were not significantly different (p=0.38). There was no migration, osteolysis or loosening of any of the implants. The mean acetabular cup inclination angle was 41.3° and the femoral component shaft angle was 137°. No fractures were reported over the 2-year follow-up with only 1 patient reporting a sciatic nerve palsy.

There was early return to impact activities in more than half our patients at 2 years with no early clinical or radiological complications related to the implant. Longer term follow-up with increased patient numbers are required to restore surgeon confidence in HRA and expand the use of this novel product.

In conclusion, CoC resurfacing at 2-years post-operation demonstrate promising results with satisfactory outcomes in all recorded PROMS.

Aims

National joint registries under-report revisions for periprosthetic joint infection (PJI). We aimed to validate PJI reporting to the Australian Orthopaedic Association National Joint Arthroplasty Registry (AOANJRR) and the factors associated with its accuracy. We then applied these data to refine estimates of the total national burden of PJI.

Methods

A total of 561 Australian cases of confirmed PJI were captured by a large, prospective observational study, and matched to data available for the same patients through the AOANJRR.

Aims

A proximal femur fracture (PFF) is a common orthopaedic presentation, with an incidence of over 25,000 cases reported in the Australian and New Zealand Hip Fracture Registry (ANZHFR) in 2018. Hip fractures are known to have high mortality. The purpose of this study was to determine the utility of the Clinical Frailty Scale (CFS) in predicting 30-day and one-year mortality after a PFF in older patients.

Methods

A retrospective review of all fragility hip fractures who met the inclusion/exclusion criteria of the ANZHFR between 2017 and 2018 was undertaken at a single large volume tertiary hospital. There were 509 patients included in the study with one-year follow-up obtained in 502 cases. The CFS was applied retrospectively to patients according to their documented pre-morbid function and patients were stratified into five groups according to their frailty score. The groups were compared using t-test, analysis of variance (ANOVA), and the chi-squared test. The discriminative ability of the CFS to predict mortality was then compared with American Society of Anaesthesiologists (ASA) classification and the patient’s chronological age.

In a society whereby the incidence of obesity is increasing and medico-legal implications of treatment failure are more frequently ending with the consulting doctor, clarity is required as to any restrictions placed on common orthopaedic implants by manufacturing companies. The aim of this study was to identify any restrictions placed on the commonly used femoral stem implants in total hip replacement (THR) surgery, by the manufacturers, based on patient weight. The United Kingdom (UK) National Joint Registry (NJR) was used to identify the five most commonly used cemented and uncemented femoral stem implants during 2012. The manufacturing companies responsible for these implants were asked to provide details of any weight restrictions placed on these implants. The Corail size 6 stem is the only implant to have a weight restriction (60Kg). All other stems, both cemented and uncemented, were free of any restrictions. Fatigue fracture of the femoral stem has been well documented in the literature, particularly involving the high nitrogen stainless steel cemented femoral stems and to a lesser extent the cemented cobalt chrome and uncemented femoral stems. In all cases excessive patient weight leading to increased cantilever bending of the femoral stem was thought to be a major factor contributing to the failure mechanism. From the current literature there is clearly an association between excessive patient weight and fatigue failure of the femoral stem. We suggest avoiding, where possible, the insertion of small stems (particularly cemented stems) and large offset stems (particularly those with a modular neck) in overweight patients.

Periprosthetic femoral fractures are a challenging problem to manage. In the literature various constructs have been designed and tested, most requiring cables for proximal fixation. The Synthes Locking Attachment Plate (LAP) has been designed to achieve proximal fixation without the use of cables.

The aim of this study was to biomechanically evaluate the LAP construct in comparison to a Cable plate construct, for the fixation of periprosthetic femoral fractures after cemented total hip arthroplasty (THA).

Twelve synthetic femora were tested in axial compression, lateral bending and torsion to determine initial stiffness, and stiffness following fixation of a simulated midshaft fracture with and without a bone gap. Two different fracture fixation constructs (six per group) were assessed. Each construct incorporated a broad curved LCP with bi-cortical locking screws for distal fixation. In the Cable construct, 2 cables and 2 uni-cortical locking screws were used for proximal fixation. In the LAP construct, the cables were replaced by a LAP with 4 bicortical locking screws. Axial, lateral bending and torsional stiffness were assessed using intact specimen values as a baseline. Axial load to failure was also measured. The LAP construct was significantly stiffer than the cable construct under axial load with a bone gap (simulating a comminuted fracture) (p=0.01). There were no significant differences between the two constructs in any of the other modalities tested. Loading to failure resulted in no significant differences between constructs, in either initial stiffness or peak load.

In conclusion the LAP construct enables bi-cortical screw fixation around a prosthesis. Compared to cables, this was stiffer when there was a bone gap and thus should offer improved proximal fixation of Vancouver B1 proximal femoral fractures in cemented THA.

Introduction

Periprosthetic femur fractures are a serious complication after hip replacement surgery. In an aging population these fractures are becoming more and more common. Open reduction and plate osteosynthesis is one of the available treatment options.

Objective

To investigate hip stem stability and cement mantle integrity under cyclic loading conditions after plate fixation with screws perforating the cement in the proximal fragment.

We have prospectively followed up 191 consecutive primary total hip replacements utilising a collarless polished tapered (CPT) femoral stem, implanted in 175 patients between November 1992 and November 1995.

At a mean follow-up of 15.9 years (range 14 – 17.5) 86 patients (95 hips) were still alive (25 men and 61 women) and available for routine follow up. Clinical outcome was determined from a combination of the Harris (HHS) and Oxford (OHS) hip scores. Radiological assessment was with antero-posterior radiographs of both hips and a lateral radiograph of the operated hip. The radiographs were evaluated using well-recognised assessment techniques.

There was no loss to follow up, with clinical data available on all 95 hips. Five patients were too frail to undergo radiographic assessment, therefore radiological assessment was performed on 90 hips (95%). At the latest follow-up, the mean HHS was 78 (range 28 – 100) and the mean OHS was 36 (range 15 – 48). Stems subsided within the cement mantle, with a mean total subsidence of 2.1mm (range 0.4 – 24). Higher grades of heterotopic bone formation were significantly associated with males (p<0.001) and hypertrophic osteoarthritis (p<0.001). Acetabular wear was associated with increased weight (p<0.001) and male sex (p=0.005). Amongst the cohort, only 1 stem (1.1%) has been revised due to aseptic loosening. This patient required reaming of their canal prior to implantation, as a result of a previous femoral osteotomy. The rate of stem revision for any cause was 7.4% (7 stems), of which 4.2% (4 stems) resulted from infection following revision of the acetabular component. Twenty patients (21.1%) required some sort of revision procedure; all except 3 of these resulted from failure of the acetabular component. Cemented cups had a significantly lower revision burden (2.7%) than Harris Galante uncemented components (21.8%) (p<0.001).

The CPT stem continues to provide excellent radiological and clinical outcomes at 15 years following implantation. Its results are consistent with other polished tapered stem designs. Cup failure remains a problem and is related in part to inadequate bearings and biological abnormalities.

Periprosthetic femur fractures are severe complications after hip arthroplasty. There is a high re-operation rate due to malunion, refracture and stem loosening. Fixation is more rigid when screws are used for proximal fixation of the plate instead of cables. Screws penetrating the cement mantle may damage it and induce loosening of the prosthesis stem.

Hypotheses

The usage of larger diameter drills can prevent cement damage during screw insertion. There is only little loss in pull-out resistance using larger drills. A metal rod (diameter: 13 mm) was cemented into a transparent plastic tube (diameter: 25 mm), leaving a homogeneous cement layer of 6mm. Drills of different diameters (4.3 mm, 4.3 mm + tapping, 4.5 mm, 4.8 mm) were used to implant uni- and bicortical locking screws (all 5mm outer and 4.4mm core diameter) into the cement layer. Locking head screws (LHS: Synthes, Switzerland), periprosthetic locking screws (PPLS: Synthes, Switzerland) and NCB mulitidirectional locking screws (NCB: Zimmer, USA) were used.

The onset of cracks was visually monitored during drilling, tapping and screw implantation. Pull-out resistance was measured on each screw. No crack appeared after implantation of any unicortical screw. No cracks appeared after drilling for bicortical screws. Cracks appeared after tapping or inserting bicortical screws (62.5% of the cases). Increasing the drill diameter reduces the risk of cement mantle cracks (to 25%). Bicortical screws had the highest pull-out resistance (median 3015N compared to 1250N for unicortical screws). Screws with a flat tip, smaller flute or double thread showed higher pullout forces.

Unicortical screws can be implanted without damaging the cement. Bicortical screws have higher pull out resistance but bear the risk of cement mantle damage.

For insertion of bicortical screws a 4.5 mm drill should be considered instead the usual 4.3 mm one. New screws should be developed for unicortical fixation of periprosthetic fractures combining favorable design properties. Further studies should follow to investigate crack formation and loosening after cyclic loading.

Introduction and aim

One of the suggested advantages of hip resurfacing arthroplasty is that it offers the possibility of straightforward revision. As the femoral canal is not violated during the resurfacing procedure, it is postulated that the outcome of revision surgery should be equivalent to that of a primary hip arthroplasty. The purpose of this study was to investigate whether revision was as straightforward and as successful as has been suggested.

Method

14 patients underwent revision surgery due to the failure of the femoral component of their Birmingham hip resurfacing. The femoral component was revised to a cemented Zimmer CPT stem, with a large modular metal head (MMT). The acetabular component was found to be well fixed and was left in situ. Radiographs were studied to review any change in offset. These patients were matched with a group who had undergone a hybrid total hip arthroplasty as a primary procedure, using the same bearing. The Oxford and Harris Hip scores were used to measure outcome.

We reviewed 142 consecutive primary hip arthroplasties using the Exeter Universal femoral stem implanted between 1988 and 1993 into 123 patients. 74 patients with 88 hips survived to 10 years or more and were reviewed with a mean 12 years 8 months. There was no loss to follow-up and the fate of all stems is known.

Our stem revision rate for aseptic loosening and osteolysis was 1.1% (1 stem); stem revision for any cause was 2.2% (2 stems); and re-operation for any cause was 21.6% (19 hips), all but 2 of which were due to cup failure.

All but one stem subsided within the cement mantle to an average of 1.5mm at final follow-up (0 to 8mm). One stem was revised for deep infection and one was revised for excessive periarticular osteolysis. One further stem had subsided excessively (8mm) and demonstrated lucent lines at the stem-cement and cement-bone interfaces. This was classified as a radiological failure and is awaiting revision. 28% of stems had cement mantle defects, which were associated with increased subsidence (p=0.01), but were not associated with endosteal lysis or stem failure.

Periarticular osteolysis was significantly related with the degree of polyethylene wear (p<0.001), which was in turn associated with younger age patients (p=0.01) and males (p<0.001).

The Exeter metal backed cups were a catastrophic failure with 34% revised (11 cups) for loosening. The Harris Galante cups failed with excessive wear and osteolysis, with failure to revision of 18%. Only 1 cemented Elite cup was revised for loosening and osteolysis (4%).

The Exeter Universal stem implanted outside the originator centre has excellent medium term results.

Introduction

The success of total hip replacement in the young has consistently been worse both radiologically and clinically when compared to the standard hip replacement population.

Methods

We describe the clinical and radiological outcome of 58 consecutive polished tapered stems (PTS) in 47 patients with a minimum of 10 years follow-up (mean 12 years 6 months) and compared this to our cohort of standard patients. There were 22 CPT stems and 36 Exeter stems.

Objectives

Overlap between the distal tibia and fibula has always been quoted to be positive. If the value is not positive then an injury to the syndesmosis is thought to exist. Our null hypothesis is that it is a normal variant in the adult population.

Methods

We looked at axial CT scans of the ankle in 325 patients for the presence of overlap between the distal tibia and fibula. Where we thought this was possible we reconstructed the images to represent a plain film radiograph which we were able to rotate and view in multiple planes to confirm the assessment.

Introduction: Since the introduction of locked implants new possibilities in the treatment of proximal humeral fractures have evolved. Despite the success using locked plates recent publications report the cutting of screws through the humeral head in up to 30% of the cases. The distribution of the bone strength in the humeral head is not linear. Can polyaxial screw positioning in areas with higher bone strength reduce the “cutout” rate? Which effect has an inferomedial screw if the medial hinge is not restored?

Methods: 4 groups were formed from 31 fresh frozen proximal humeral cadavers. A polyaxial proximal humeral locking plate was used to perform the tests. A standardised unstable intraarticular fracture was created. Main Outcome measure was the load and cycle where at least one screw was cutting through the cartilage.

Results: Polyaxial screw placement in areas of incresead bone strength compared to random screw placement had no effect on the cutout behaviour (p=0.7). Increased screw number (3 vs. 5) significantly increases the resistance against cutout (p< 0.04). An inferomedial screw significantly increases the resistance to develop a cutout compared to the control groups(p=0.03 and p< 0.05).

Discussion: The placement of an inferomedial screw significantly increases the resistence to develop a cutout in proximal humeral fractures without a medial hinge independently of the total screw number. In addition we could also show that the number of screws in the humeral head has a significant effect on cutout resistance in a human cadaver setup.

Background: A radiological audit of the local use of the Dynamic Hip Screw in extracapsular proximal femur fractures. Study aim: to identify cases of mechanical failure and revision, to determine predictors of fixation failure.

Methods: A retrospective radiological review of 567 consecutive cases at Western Australian tertiary hospitals over a 3 year period (2002 – 2004) using the Picture Archive Computer System (PACS).

Results: Female: male ratio was 2.79: 1. Evan’s classification: 418 fractures stable (73.7%), 149 unstable (26.3%). Failure of fixation occurred in 14 cases (2.5%); ten due to hip screw cut out (1.8%) and four due to plate pull off (0.8%). All cases of cut out had a significantly higher mean tip apex distance (TAD) (31 vs 20mm, P < 0.001) and an unstable fracture configuration; 8 of 10 had a poor reduction. Bivariate logistical regression revealed TAD of 25mm or more to be most predictive of cut out; followed by mean TAD, superior anterior and inferior posterior screw placement, unstable fracture configuration and poor reduction. Unassociated factors included gender, age, American Society of Anesthesiologists’ score, plate angle and length, operation time and surgeon level. A three-variable model found TAD of 25mm or more and unstable fracture configuration to be predictive, but not poor reduction. Cases with a TAD of 25mm or more with unstable fracture configuration and a poor reduction had a 21.6% chance of cut out (8 of 29).

Conclusions: This is the first multifactorial multivariate analysis of a single implant sliding hip screw series. Compared with the literature, the rate of failure is low. Possible reasons include appropriate choice of implant for fracture type, improved performance with use of a single model of implant, and low exclusion rates due to the use of PACS.
