Introduction

Female gender, old age (men >60y and women > 55y), severe acetabular dysplasia, poor proximal femoral bone geometry, large (>1cm) femoral head cysts, limb-length discrepancy (> 2cm) and small prosthetic head size (less than 50mm for men and less than 46mm for women) are risk factors for hip resurfacing arthroplasty (HRA).

Purpose

To present clinical and radiographic results of HRA in patients having risk factors.

Patients and methods: A total of 39 HRA was inserted in 33 patients (11 men and 22 women). Birmingham hip resurfacing (Smith & Nephew, UK) was used in 9 hips and Adept (Finsbury, UK) was used in 30 hips. Among the 30 hips inserted Adept, 11 cups were fixed with rim screws. The mean age of the patients at the time of operation was 52 years. The mean weight and height of the male and female patients were 70.4kg and 167cm, 58.5kg and 154.4cm, respectively. The median head size of the male and female patients was 50mm and 42mm, respectively. Preoperative diagnosis was primary osteoarthritis in 6 hips and secondary osteoarthritis due to aceatbular dysplasia (DDH) in 33 hips. Risk factors of HRA were listed for each patient. The Harris hip score and visual analogue pain scale (VAS) were measures of clinical outcome. Radiographic review was performed retrospectively. MRI and CT images were acquired in 29 hips and 2 hips, respectively, at a mean of 4.8 years after HRA to find periprosthetic soft tissue abnormality such as a psedotumor. Kaplan-Meier method was used to calculate implant survivorship.

Introduction

Highly cross-linked polyethylene (HXLPE) was developed to reduce the wear of articular-bearing surfaces in total hip arthroplasty (THA). This study aimed to compare the mean linear wear of HXLPE with a 22.225 mm diameter zirconia head with that of conventional polyethylene (CPE) with a 22.225 mm diameter ortron head.

Materials and Methods

A prospective cohort study performed on 93 patients (113 hips) who had undergone primary cemented THAs at our hospital between January 2001 and December 2003. The subject population included 85 females and 8 males with a mean age of 58.0 years (22 to 78) at the time of surgery. The mean follow-up period was 10.2 years (9 to 12). We randomly used two types of implants: the HXLPE cup with a 22.225 mm diameter zirconia head (Kyocera Medical, Osaka, Japan) in 60 hips (HXLPE group), and the CPE cup with a 22.225 mm diameter ortron head (DePuy International, Leeds, UK) in 53 hips (CPE group). Linear wear (penatration) by computer-assisted method with PolyWare software (Draftware Inc, Indiana, USA) was measured at 10 years. Anteroposterior radiographs were evaluated for osteolysis or component loosening defined by the criteria of Hodgkinson et al. Analysis of covariance using the general linear models procedure was carried out to determine the linear wear rate difference between the groups after adjusting for variables (age at surgery, sex, body mass index, vertical distance, horizontal distance, cup inclination, and cup anteversion) as covariates. The differences were considered significant when the p value was <0.05.

Stair stepping motion is important in daily living, similar to gait. In this study, we did a Kinematic Analysis of total knee arthroplasty during stair-stepping. A total of 20 patients implanted with Bi-Surface 5PS were assessed. The Bi-Surface knee is a posterior-cruciate substitute prosthesis with a unique ball-and-socket joint in the mid-posterior portion of the femoral and tibial components. This joint functions as a posterior stabilizing cam mechanism and as a load-bearing surface in flexion. Patients were examined during stair-stepping motion using a 2-dimensional to 3-dimensional registration technique. The kinematic pattern in step up was a medial pivot, in which the level of anteroposterior translation was very small. In step down, the kinematic pattern was neither pivot shift nor rollback. From minimum to maximum flexion, anterior femoral translation occurred slightly. However, it became clear in this study that the joint's stability during stair-stepping was affected by the design of the femorotibial joint rather than Post/Cam engagement.

Introduction:

The diagnosis of implant-associated infections is challenging, and the conventional culturing of periprosthetic tissue has been the gold standard for diagnosis of implant-associated infections. However, conventional diagnostic tests are inaccurate because the pathogenesis of implant-associated infection is related to microorganisms growing in biofilms. We compared culture of samples obtained by sonication of explanted implants to dislodge adherent bacteria from implants with conventional culture of periprosthetic tissue. The purpose of this study is to evaluate the results of sonication that is microbiological diagnostic method for implant-associated infections.

Materials and Methods:

Between January 2013 and April 2013, a total of 19 consecutive patients underwent the removal of implants at our institution. There were 15 women and 4 men with a mean age of 71 years (32 to 90) at the time of the operation. Implants were removed because of aseptic loosening in 9 patients, infection in 6 patients, necrosis in 2 patients, dislocation in 1 patient and implant fracture in 1 patient. Removed implants, including 17 joint prostheses and 2 fracture fixation devices, were subjected to sonication in a BactoSonic (BANDELIN, Germany). Preoperative bacterial culture, intraoperative conventional culture of periprosthetic tissue, intraoperative culture of sonicate-fluid, and pathological examination were assessed.

Total Knee Arthroplasty (TKA) has been widely performed and successful clinical outcomes have been achieved for the patients with knee osteoarthritis which is generally known to cause ADL problem. Clinical and radiographic evaluations are commonly used when evaluating postoperative outcomes, among which kinetic analysis and gait analysis are considered essential to investigate the more detailed effect of the treatment. There is a controversy whether performing TKA on both knees simultaneously is appropriate in treating patients with bilateral knee osteoarthritis, in terms of the speed and effectiveness of gait recovery. In this study, we reviewed the significance of performing simultaneous bilateral TKA, by the results of preoperative and postoperative gait analysis.

Materials and Methods: Total of eight patients, all female and diagnosed of bilateral knee osteoarthritis, were treated with TKA and reviewed. Mean age was 70 years old (60–74). For prosthesis, we used Scorpio NRG PS, and ADVANCE, with cementation for all. No patella was replaced. Some had unilateral TKA, and some were treated bilaterally as needed. We examined distance factors (step length and step width), gait velocity, and gait barycentric factors (single-support phase and Ratio of center of gravity maximum values). We performed the analysis preoperatively, postoperatively at 1 month, 3 months, and 6 months. We used the floor pressure gauge (NITTA CORPORATION) and the three-dimensional motion analysis device (DITECT Co. Ltd) for the analysis.

Results: During the six-month follow-ups, six cases were unilateral TKA and two were treated bilaterally. Increase in step length was seen in the unilateral cases, and it decreased in the bilateral cases. Step width decreased in five cases, two cases showed no change, and increased in one case. Gait velocity had increased in all cases. Single-support phase was close to 1 for all the cases. Ratio of center of gravity maximum values, which indicates the movement of centroid during ambulation, the ratio went up for unilateral cases while it showed no change in the bilateral cases.

Discussion: Quantitative studies of gait analysis have reported that gait condition had improved after TKA. However, some reported that the gait impairment had remained. Unilateral TKA group showed gait restoration, whereas gait abnormality in either leg was seen in the bilateral group. Gait analysis is effective in determining whether surgeons should perform unilateral TKA or bilateral TKA to the patients with bilateral knee osteoarthritis. Among the gait analysis factors, we consider that Ratio of center of gravity maximum values shows effectively the improvement of the treated knee, gait, and the condition of contralateral knee.

We have previously developed a radiographic technique, the oblique posterior condylar view, for assessment of the posterior aspect of the femoral condyles after total knee arthroplasty. The purpose of this study was to confirm the validity of this radiographic view based upon intra-operative findings at revision total knee arthroplasty. Lateral and oblique posterior condylar views were performed for 11 knees prior to revision total knee arthroplasty, and radiolucent lines or osteolysis of the posterior aspect of the femoral condyles were identified. These findings were compared with the intra-operative appearance of the posterior aspects of the femoral condyles. Statistical analysis showed that sensitivity and efficacy were significantly better for the oblique posterior condylar than the lateral view. This method can, therefore, be considered as suitable for routine follow-up radiographs of the femoral component and in the pre-operative planning of revision surgery.

The use of standing radiographs to determine correction angles for high tibial osteotomy is not appropriate because the relative angle of the articular surfaces (condylar-plateau angle) in the weight-bearing knee changes after the osteotomy. This may give unpredictable results postoperatively. We found that the condylar-plateau angle in postoperative standing films is very similar to that seen in non-weight-bearing supine views, and suggest that these latter radiographs be used for pre-operative planning. We describe our early results, using a special osteotomy jig, in 140 knees.
