Abstract

Introduction

Risk factors for osteoarthritis include raised BMI and female gender. Whether these two factors influenced synovial gene expression was investigated using a triangulation and modelling strategy which generated 12 datasets of gene expression in synovial tissue from three knee pathologies with matching BMI groups, obese and overweight, and gender distributions.

Methodology

Intra-operative synovial biopsies were immersed in RNAlater at 4oC before storage at -80oC. Total RNA was extracted using RNAeasy with gDNA removal. Following RT- PCR and quality assessment, cDNA was applied to Affymetrix Clariom D microarray gene chips. Bioinformatics analyses were performed. Linear models were prepared in limma with gender and BMI factors incorporated sequentially for each pathology comparison, generating 12 models of probes differentially expressed at FDR p<0.05 and Bayes number, B>0. Data analysis of differently expressed genes utilized Ingenuity Pathway Analysis and Cytoscape with Cluego and Cytohubba plug-ins.

Abstract

Introduction

It is increasingly evident that synovium may play a larger role in the aetiology of osteoarthritis. We compared gene expression in whole tissue synovial biopsies from end-stage knee osteoarthritis and knee trauma patients with that of their paired explant cultures to determine how accurately cultured cells represent holistic synovial function.

Methodology

Synovial tissue biopsies were taken from 16 arthroplasty patients and 8 tibial plateau fracture patients with no osteoarthritis. Pairs of whole tissue fragments were either immediately immersed in RNAlater Stabilisation Solution at 4o C before transfer to -80o C storage until RNA extraction; or weighed, minced and cultured at 500mg tissues/5ml media in a humidified incubator at 37oC, 5% CO2. After sub-culturing total RNA was extracted using RNAeasy Plus Mini Kit with gDNA removal. Following RT-PCR and quality assessment, cDNA was applied to Affymetrix Clariom D microarray gene chips. Bioinformatics analyses were performed.

Abstract

Introduction

Articular cartilage degradation is a defining feature of osteoarthritis. Synovium is a reactive tissue with synovial villae, neoangiogenesis and intimal hyperplasia common to many joint pathologies. The consequences of cartilage debris in osteoarthritis impacting the synovial intima is not well understood. We analysed the immunohistology of synovium from 16 patients with osteoarthritis and 17 patients undergoing knee surgery for non-arthritic pathologies. This data was integrated with imaging and functional scores to correlate synovitis in osteoarthritis.

Methodology

Formalin-fixed paraffin embedded synovial biopsy sections were cut in serial sequence and processed for routine staining (H&E or CD3, CD68, CD20, Vimentin, vWF and PCNA IHC) using standardised Dako monoclonal mouse anti-human antibodies. Digital images scanned at x20 were evaluated for fragments of cartilage and aggregates of inflammatory cells. Clinical data (gender, BMI, KL grade, WOMAC & SF-12 scores) was aligned with histopathological data.

High tibial osteotomy (HTO) is largely conducted as an inpatient procedure, imposing the significant cost of hospital admission. Here we examine patient-related outcomes and cost benefit of altering HTO to a day-case procedure.

A retrospective questionnaire was conducted with patients that underwent HTO in the one and half years prior to the change to day-case, and those in the one and half years afterwards. We considered pre-operative and post-operative KOOS scores, patient pain-scoring and whether they would, given their experience, undergo HTO as an outpatient again.

Thirty-three patients were included in the trial: twenty-three inpatient admissions and ten day-cases.

Average KOOS score improvement for inpatients was 24.68 and outpatients was 31.8. Outcomes such as post-operative pain and nausea/vomiting were found to be similar between groups. The outpatient group unanimously agreed that if undergoing HTO again they would desire to be treated in the outpatient setting again.

Currently HTO is conducted as an inpatient procedure in the majority of institutions. We have demonstrated that patients have comparable outcomes and agree that if undergoing HTO again they would wish to do so as day-case outpatient. This represents a significant cost saving for institutions conducting HTO as well as an improvement in service.

Ankle ligament injury is a common cause of injury to military recruits, and frequently implicated in failure to complete Royal Marines (RM) recruit training. A minority of patients at Commando Training Centre Royal Marines (CTCRM) with ankle ligament injury undergo arthroscopic ankle stabilisation surgery (Bostrum or Evans procedures). The decision to undertake surgery involves an assessment of functional benefit to the patient, medical and surgical risks, and cost-effectiveness. However, there is currently little data on the efficacy of surgery in enabling recruits to complete RM training.

To assess the number of RM recruits who completed recruit training following ankle stabilisation surgery and entered the trained strength.

A retrospective analysis of all patients at CTCRM who underwent surgery for ankle stabilisation was performed using healthcare records data. The primary outcome measure was completion of RM recruit training after surgery. 27 patients underwent surgical intervention for ankle instability between 2004 and 2015. Patients remaining in rehabilitation following their surgery were excluded, leaving 22 patients suitable for inclusion in the final analysis.

Of the eligible patients undergoing surgical intervention (n = 22), six patients – 27% – completed RM recruit training. Average time in rehabilitation 68 weeks. 10 patients underwent a Evans procedure and 9 underwent a brostum repair with 3 unknown.

Based on cumulative data spanning 11 years at CTCRM, operative intervention for ankle instability enables only a minority (27%) of patients to complete RM recruit training. Patients who undergo surgical intervention also undertake prolonged rehabilitation at a cost of £1850 per recruit per week. The poor rates of completing RM training following surgery, and the high costs of rehabilitation, have implications with regards to retaining recruits who sustain ankle injuries requiring surgical reconstruction.

Tibial plateau fractures classically present in a bimodal distribution associated with high energy mechanisms in the younger population and fragility fractures in the elderly populations as a result they are well suited for looking at the effect major trauma centre status. Military trauma surgeons in training should be exposed to as much young high-energy trauma as possible to equip them for operations.

Retrospective review of all tibial plateaus presenting to RVI 20 months before MTC status and 20 months following this.

61 patients pre, 66 post. Schatzker grade 1–4 were similar pre and post change. Post change there was an increase in Schatzker 5 (62%) and 6 (27%). High energy injuries were most common in younger males, cause was falls followed by RTAs.

MTC status has meant an increase in high energy tibial plateaus (Schatzker 5–6) These were predominantly seen in younger males with high-energy mechanisms. As this is likely to be replicated across all injuries, we would recommend military trauma surgeons have a significant period of time training in major trauma centres to ensure adequate exposure to young high energy trauma.

The PFC Sigma Cobalt Chrome Sigma (PFCSCC) was introduced in 2006, an update of the PFC Sigma designed to reduce backside wear. To help identify any significant early failures following its introduction, we prospectively identified all recipients over a one-year period. The patient's clinical, demographic and radiographic data, American Knee Society scores (AKSS), Oxford Knee scores (OKS) and SF-12 scores was recorded pre-operatively and at one, three and five years. 233 patients underwent 249 primary knee arthroplasties with the PFCSCC. Seventeen patients (19 knees) died and 29 patients (30 knees) were also lost to follow up at the five year point. The mean age was 66.6 (34–80) with 47.6% of the cohort being male. The mean five year follow-up was 1836 days (1530–2307). Five knees (2.2%) were revised for infection and three were revised for pain. The 5–year cumulative survival rate was 96.6% for any failure and 98.6% for aseptic failure. AKSS 32.6 (0–86.6) preoperatively, 80.7 (29–95) 5 years P < 0.001. OKS was 39.0 (22–53) preoperatively, 23.5 (4.7–42.3) 5 years P < 0.001. These results demonstrate a good early survivorship when compared to the old design PFC Sigma, however further follow-up to ten years is required.

Epidemiological data about tibial plateau and associated intra-articular proximal tibial fractures provides clinicians with an understanding of the range, variety, and patterns of injury. There are relatively few studies examining this injury group as a whole. We prospectively recorded all tibial plateau and intra-articular proximal tibial fractures occurring in our regional population of 545,000 adults (aged 15 years or older) in 2007–2008. We then compared our results with previous research from our institution in 2000. There were 173 fractures around the knee, 65 of these involved the tibial plateau. Median age was 59 years (IQR, 36.5–77.5 yrs). Tibial plateau fractures were more common in women (58.5%vs 41.5%). The median age of men was 37 years (IQr, 29–52 yrs) compared to women, 73 years (IQR, 57–82 yrs). Tibial plateau fractures accounted for 0.9% overall and 2.5% of lower limb fractures. Incidence was 1.2/10,000/yr (95% CI, 0.9–1.5). We have prospectively identified and described the epidemiological characteristics of tibial plateau fractures in adults from our region. We have identified a change to the epidemiology of these fractures over a relatively short time frame as the patients at risk age.

In June 2012 the Orthopaedic Speciality Advisory of the Joint Committee on Surgical Training defined ‘minimum indicative numbers’ that trainees would have to meet before completion of specialist training. It has been speculated that regions have varied in their ability to provide operative opportunities to their trainees. This study aims to test the hypothesis that there are regional differences in operative training experience. The eLogbook database was interrogated for cases over a 12 month period from 7 August 2013 to 5 August 2015. Within each region, the mean of the cases registered by orthopaedic trainees in each year of training during the study period was calculated and summed to give a representative surgical experience for the years ST3-8. First surgeon only cases were analysed for 11 index procedures in 30 T&O rotations. Considerable variation in training existed across rotations. In three index procedures, including DHS, no rotation achieved the minimum indicative number required. All rotations achieved the minimum indicative number of external fixator applications. This study proves the extent of the significant regional variation in surgical training in Trauma and Orthopaedics in the UK and raises concerns regarding the volume of operative training currently achieved.

Open fractures are uncommon in the UK sporting population, however because of their morbidity then are a significant patient group. Currently there is very little in the literature describing the epidemiology of open fracture in sport. We describe the epidemiology of sport related open fractures from one centre's adult patient population.

Retrospective analysis of a prospectively collected database recording all sport related open fracture s over a 15 year period in a standard population.

Over the 15 year period, there were 85 fractures in 84 patients. The mean age was 29.2 years (range 15–67). 70 (83%) were male and fourteen female (17%). The six most common sports were football (n=19, 22%), rugby (n=9, 11%), cycling (n=8, 9%), hockey (n=8, 9%); horse riding (n=6, 7%) and skiing (n=6, 7%). The top five anatomical locations were fingers phalanges, 35%; tibia-fibula 23%; foreman 14%; ankle 11% and metacarpals 5%. The mean injury severity score was 7.02. Forty five patients were grade 1; 28 patients were grade 2; 8 patients were grade 3a; and 4 were grade 3b according to the Gustilo-Anderson classification system. Seven patients (8%) required plastic surgical intervention for the treatment of these fractures. The types of flaps used were split skin graft (n=4), fasciocutaneous flaps (n=2); and adipofascial flap (n=1).

We looked at the epidemiology open fractures secondary to sport in one centre over a 15 year period. Football was the most common sport (22%) and within football, the most common site was the tibia and fibula. In contrast, within the cohort a whole the majority of fractures were upper limb, with the hand being the most common site. Whilst not common in sport, when they are sustained they are frequently occur on muddy sport fields or forest tracks and must be treated appropriately. A good understanding of the range and variety of injuries commonly sustained in different sports is important for clinicians and sports therapists.

Freezing cold injuries (Frostnip and Frostbite) (FCI) have become uncommon in UK military personnel relative to non-freezing cold injuries (NFCI, ‘trench foot’). However if unidentified or inappropriately treated they may lead to avoidable medical downgrading or discharge. JSP 539 recommends delay or avoidance of surgical debridement where possible.

An anonymised retrospective audit was performed of FMED7 medical reports of cases seen in the Institute of Naval Medicine Cold Injury Clinic (CIC) between July 2002 and January 2014 inclusive.

In all 149 FCI cases were identified, 71 affected hands only of which 34 were bilateral, and 58 affected feet only, 34 of those being bilateral. A total of 17 patients had injured both hands and feet, with 10 bilateral. Royal Marines accounted for half of these cases, with the Army making up a further third, and the Royal Navy and RAF making up the remainder. Most FCI were found to have occurred in Norway, with Marine ranks being most commonly affected. Ten cases underwent surgery: aspiration of blisters, debridement of tissue, or amputation. Seven of these procedures took place prematurely, which appeared detrimental to recovery. No patients required fasciotomy.

FCI are uncommon, but in arctic conditions their risk rises dramatically. The best treatment is conservative where possible following JSP 539 guidelines and consultation with CIC should occur at the earliest possibility. The Potential benefits of surgery must be weighed against problems of injured tissue healing and expert opinion should be obtained.

Current Department of Health guidelines state that medical personnel should be ‘bare below the elbows’. Critics of this policy have raised concerns over the impact of these dress regulations on the portrayed image and professionalism of doctors. However, the importance of the doctor's appearance in relation to other professional attributes is largely unknown. The purpose of this study was to determine the opinion of patients and their relatives on the importance of appearance and the style of clothing worn by orthopaedic doctors. The secondary aim was to establish how patients would prefer orthopaedic doctors to be dressed.

The study consisted of the administration of a survey questionnaire to 427 subjects attending the orthopaedic outpatient clinics in four hospitals across Scotland. Subjects were asked about the importance or otherwise of various aspects of the doctors’ appearance and responded using a modified Likert 5-point scale. Subjects’ rank preferences for four different styles of doctors’ clothing were also determined. The study was appropriately powered to identify a 0.5 difference in mean rank values with 0.90 power at a = 0.05.

The majority of respondents felt doctors’ appearance was important but not as important as compassion, politeness and knowledge. Only 50% felt that the style of doctors clothing mattered; what proved more important was an impression of cleanliness and good personal hygiene. In terms of how patients would prefer doctors to dress in clinic, the most popular choice proved to be the smart casual style of dress, which conforms with the ‘bare below the elbows’ dress code policy. Indeed, the smart casual clothing style was the highest ranked choice irrespective of patient age, gender, regional or socioeconomic background.

Some military personnel are having Femoral Acetabular Impingement (FAI) surgery. The use of the alpha angle (AA) to help assess the diagnosis is common. Currently there are no standardised values available across a asymptomatic pre-arthritic population.

Retrospective analysis of 200 consecutive individuals (400 hip joints) with ages 20 to 50, who had a CT performed between 1 Apr 2011 and 29 Nov 2011 due to abdominal pathology. The AA of Notzli was measured on the axial view.

The mean AA value was 53.5 (95%CI 1.30) for Right hips and 53.4 (95% CI 1.31) for the left. In age 20–30 Right 52.6 (95%CI 3.5) the Left 52.0 (95%CI 2.9), 31–40 Right 53.9 (95%CI 2.5) Left 53.4 (95%CI 3.1), 41–50 Right 53.8 (95% CI 1.9) Left 53.2 (95% CI 1.8). Mean male Right 52.9 (95% CI 1.5) Left 53.2 (95%CI 1.9) Female Right 52.5 (95% CI 1.5) Left 49.9 (95% CI 1.6). 144/400 (37%) of patients had angle >55 degrees.

Previous literature suggests an AA >55 degrees is diagnostic of FAI, we suggest that the AA is highly variable across age and sex and that >1/3rd of asymptomatic patients will have an AA that was previously regarded as abnormal.

Rugby union is the second commonest cause of sporting fracture in the UK. Yet little is known about patient outcome following such fractures.

All rugby union fractures sustained during 2007–2008 in the Lothian were prospectively recorded. Patients were contacted by telephone in February 2012 to ascertain their progress in returning to rugby.

There were 145 fractures in 143 patients, including 122 upper limb and 25 lower limb fractures. 117 fractures (81%) were followed at mean 50 months (range 44–56 months). 87% returned to rugby post injury, with 85% returning to rugby at the same level or higher. 77% returned by three months and 91% by six months. In upper limb fractures 86% returned by six months and 94% by six months. In lower limb fractures 42% returned by three months and 79% by six months. 32% had ongoing fracture related problems. 9% had impaired rugby ability secondary to fractures.

Most patients sustaining a fracture playing rugby union will return to rugby at a similar level. While one third of them will have persisting symptoms post-injury, for the majority this will not impair their rugby ability.

Alcohol remains a significant cause of disease in the UK population. Yet the effect of alcohol on fractures remains conflicting. We present a prospective analysis of self-reported alcohol consumption and the epidemiology of fractures sustained.

1950 patients over 13 years of age were prospectively interviewed after sustaining a fracture with basic epidemiological data, fracture data and average alcohol consumption recorded.

1621 (83%) of interviewees provided information on alcohol consumption. 10% admitted to drinking in excess of Scottish Health guidelines. 18.1% of males drunk to excess, compared to 4.7% of females (p<0.001). The five most frequent fractures were distal radius (20%), metacarpals (12%), ankle fractures (12%), neck of femur (10%), phallanges (10%). 48% of fractures were falls from standing height. Excess drinkers were more likely to sustain an AO grade C fractures than safe drinkers (18.1% compared to 11.2%, p<0.05). Excess drinkers sustained more open fractures than safe drinkers (5% compared to 1%, p<0.001). Excess drinkers were on average 5.66 years younger than safe drinkers at the time of injury (44.57 years compared to 50.23 years, p<0.05).

People reporting alcohol excess who have sustained a fracture are more likely to be younger and suffer more severe fractures than those drinking within current guidelines. Opportune targetting of patients consuming excess alcohol should be targetted at problem drinkers sustaining a fracture.

Introduction

Longstanding complex muliplanar foot deformities represent a significant challenge. The traditional surgical techniques involve excessive dissection and excision of large bony wedges or modifications of the triple fusion to correct the deformity. The majority of the reports in the literature present collective data on different deformity patterns and also mix paediatric and adult patients, even with multiple correction techniques. The aim of this study was to evaluate the clinical, radiological and functional outcomes of the gradual correction of a single common deformity pattern of equino-cavo-varus using a single correction technique of the V-osteotomy and the Ilizarov frame.

Material and methods

We present prospectively collected data on 40 feet in 35 adult patients with stiff longstanding equino-cavo-varus deformity. All patients had a V-osteotomy and gradual correction using an Ilizarov frame, with a mean follow-up of 20 months. We collected the American Orthopaedic Foot and Ankle Scocity score (AOFAS), the Foot and Ankle Disability Index (FADI) and a Visual Analogue Pain score (VAS) for all ptients preoperatively and between 1 and 2 years following frame removal.

The collective orthopaedic literature appears to highlight the Jones fracture of the fifth metatarsal, as being slow to heal, and having a high incidence of non-union. There remains a lot of confusion, throughout the orthopaedic literature, about the exact nature of this fracture.

The authors present the largest case series currently published of 117 patients who sustained a Jones fracture, demonstrating patient outcomes with different modalities of care.

All Medical notes from the Emergency Department are recorded on a database.

A computer program was use to search the Emergency department database of the Edinburgh Royal infirmary notes data base for the terms 5th metatarsal combined with a coding for referral to fracture clinic over a 6 years period from 2004–2010. The researchers went through the X-ray archive, identified and classified all 5th metatarsal fractures.

There were 117 patients in our series, refracture rate 7/117 6%. Average time to discharge 13 weeks (4–24). 18% of patients took longer than 18 weeks for their fracture to clinically heal. 34% were clinically healed at less than six weeks, with only 7% radiologically healed at six weeks. There was no significant difference in outcome between cast, moonboot, tubigrip or hard shoe in terms of outcome.

A large proportion of Jones fractures have delayed healing, patients who are clinically asymptomatic may not have radiological healing. Currently in our practice there is no uniform management of Jones fractures. We discuss the difference in healing rates for different management techniques.

The aim of this study was to compare the long-term outcome from total knee replacement (TKR) in young versus old patients in terms of pain and functional outcome.

We used our arthroplasty database which recorded prospectively pain and American Knee Society scores at regular intervals over ten years after TKR. The procedures used a modern, cemented resurfacing type cruciate retaining prosthesis. A cohort of young patients (< 55 years) were identified. A control group of patients ≥ 56 was identified, matching for ASA, body mass index and underlying condition. Change over time was analysed using a factorial repeated measures ANOVA test, which allowed for investigation of difference between groups.

40 Knees in 26 patients were identified. 2 patients died prior to follow up, 2 were revised within the study period. (1 for infection at 2 years and one for change of poly at 7 years) and a further 4 were lost to follow up. 7 knees could not be matched and were excluded. This left a study group of 24 young and 24 older knees.

Pain scores (p=0.025) and American Knee Society “Knee” (p<0.001) and “Function” (p<0.001) scores changed significantly over time. There were however no statistical differences over the 10 year period in pain (p=0.436) and knee performance (0.618) but overall function was higher throughout the period in the younger group (=0.004).

Knee replacement in younger patients produces similar outcomes in terms of pain and function compared with older patients and TKR should not be withheld purely on account of age.

Current literature comparing the effect on body mass index (BMI) on the outcome of total hip replacements (THR) is inconclusive. To describe the effect of BMI on THR over the first five years.

We prospectively recruited 1,617 patients undergoing primary THR for osteoarthritis and followed them up at 5 years, recording, dislocations, revisions, deep and superficial infections, Harris Hip scores (HHS) and SF-36. A multivariate analysis was performed to identify if BMI is an independent predictor of adverse outcome.

148 (9%) patients had a BMI >35. 6.8% of patients with a BMI >35 had a dislocation by 5 year post op compared with 3.2%BMI 30–34.9, 2% BMI 25–29.9 and 1.5% BMI<25 (p=0.03) Superficial infections 14.2% BMI >35, 4.4% BMI <25. In SF 36 scores only Mental Health and change in health had no significant differences with an increase in BMI having a statistically significant decrease in all other SF scores. HHS had a mean improvement of 45.1 at five years with an expected loss of 0.302 HHS points (95% CI0.440–0.163) per 1 point BMI increase. There was no significant difference in deep infections, mortality <3 months, revision rates or length of stay.

The most significant risk in increasing BMI is the dislocation rate, possibly representing increased technical difficulty in larger patients. Whilst increasing BMI has a reduced HHS and SF16 score, the overall benefit is still positive.
