Muller-Weiss disease is an uncommon condition with unclear etiology and no gold standard treatment. The question arises; which joints to fuse? Although no consensuses prevail, one must postulate fusion should include those affected. Consequently, to establish an algorithm for its surgical management we set out to study clinical and radiographic features with use of SPECT-CT and a literature review.

57 consecutive feet presenting with Muller-Weiss disease analysed; 15 men, 25 women, age 22–84. Condition bilateral in 17, left side 16, right in 7 patients. Specific history and examination by senior author. Radiographic series and SPECT-CT obtained with surgery performed on significantly symptomatic feet. Measurements of Meary-Tomeno angles, anteroposterior thickness of navicular at the midpoint of each naviculo-cuneiform, alongside the medial extrusion distance and percentage of compression in each case performed. Poor correlation between Meary's angle and 1) degree of compression at naviculo-cuneiform joints, 2) degree of extrusion 3) compression vs extrusion using R2 coefficient of determination (invalidating Maceira et al. classification). In unilateral cases, extrusion significantly greater on affected side 94.7% (P< 0.001 Fisher exact test). Degree of extrusion significantly greater in bilateral than unilateral cases (p=0.004 unpaired T test). Valgus hindfoot and Meary's negative most common pattern with no correlation between heel alignment and Meary's R2 = 0.003. SPECT-CT useful to determine subtalar involvement in ‘stage 2 disease.’

Following review of cases and published literature we propose the following classification for Muller-Weiss disease with treatment algorithm. 3 Stage delineation; Stage 1 (Normal hindfoot alignment); 1A. Talonavicular disease only - Isolated Talonavicular arthrodesis 1B. Talonavicular + Subtalar; double medial or triple arthrodesis. Stage 2. Talonavicular + Naviculocuneiform; 2A. Adequate bone stock - Talo-naviculo-cuneiform arthrodesis, 2B. Inadequate bone stock +- subtalar disease; Talo-naviculo-cuneiform arthrodesis with tricortical bone graft (Mayich). Stage 3; Asymmetric ankle varus. Pantalar arthrodesis Double/triple/TNC/TAR arthrodesis with hindfoot re-alignment.

Aim

To determine mortality and outcomes of patients diagnosed with fracture-related infections (FRIs).

Method

FRI patients treated at a trauma centre between 2001 and 2020 were analysed. The primary outcome was 1-year mortality; mortality associations with FRI organism, depth of involvement, and temporality were investigated with multivariable survival analysis. Healthcare-associated and serological outcomes were reported as secondary outcomes.

Introduction

Management of open fractures is challenging and requires a multidisciplinary team approach. Gustilo Anderson Type IIIB fractures reportedly have a higher infection rate (up to 52%) and up to 16% amputation rate. This study aims to evaluate outcomes of using Adjuvant Local Antibiotic Hydroxyapatite Bio-Composite in management of Open Gustilo-Anderson IIIB fractures.

Materials and Methods

We reviewed a prospective data of 80 patients who presented with Gustilo Anderson Type IIIB Open Fracture to a single ortho-plastic centre. Only patients who were managed with single-stage “Fix and Flap” along with intra-operative Adjuvant Local Antibiotic Bio-Composite were included.

Abstract

Objectives

Our objective was to determine the incidence of post-operative COVID-19 infection within the first two weeks following treatment at the COVID-free site. During the COVID-19 pandemic our hospital saw one of the highest rates of COVID-19 infection in the United Kingdom. As a result, our trauma services were relocated to a vacant private elective hospital to provide a ‘COVID-free’ setting for trauma patients. Patients admitted to the COVID-free site were screened for coronavirus infection and only treated at that site if asymptomatic. Those with symptoms were treated at the ‘COVID’ site.

Method

We collected data at the COVID-free site during the first seven weeks of its’ establishment. Inclusion criteria were all patients presenting for operative management of limb trauma; however, fragility hip fractures were excluded. All patients were screened for symptoms of COVID-19 infection at their 2-week follow-up appointment.

Rotator cuff tears are the most common cause of shoulder disability, affecting 10% of the population under 60 and 40% of those aged 70 and above. Massive irreparable rotator cuff tears account for 30% of all tears and their management continues to be an orthopaedic challenge. Traditional surgical techniques, that is, tendon transfers are performed to restore shoulder motion, however, they result in varying outcomes of stability and complications. Superior capsular reconstruction (SCR) is a novel technique that has shown promise in restoring shoulder function, albeit in limited studies. To date, there has been no biomechanical comparison between these techniques. This study aims to compare three surgical techniques (SCR, latissimus dorsi tendon transfer and lower trapezius tendon transfer) for irreparable rotator cuff tears with respect to intact cuff control using a clinically relevant biomechanical outcome of rotational motion.

Eight fresh-frozen shoulder specimens with intact rotator cuffs were tested. After dissection of subcutaneous tissue and muscles, each specimen was mounted on a custom shoulder testing apparatus and physiologic loads were applied using a pulley setup. Under 2.2 Nm torque loading maximum internal and external rotation was measured at 0 and 60 degrees of glenohumeral abduction. Repeat testing was conducted after the creation of the cuff tear and subsequent to the three repair techniques. Repeated measures analysis with paired t-test comparisons using Sidak correction was performed to compare the rotational range of motion following each repair technique with respect to each specimen's intact control. P-values of 0.05 were considered significant.

At 0° abduction, internal rotation increased after the tear (intact: 39.6 ± 13.6° vs. tear: 80.5 ± 47.7°, p=0.019). Internal rotation was higher following SCR (52.7 ± 12.9°, intact - SCR 95% CI: −25.28°,-0.95°, p=0.034), trapezius transfer (74.2 ± 25.3°, intact – trapezius transfer: 95% CI: −71.1°, 1.81°, p=0.064), and latissimus transfer (83.5 ± 52.1°, intact – latissimus transfer: 95% CI: −118.3°, 30.5°, p=0.400) than in intact controls. However, internal rotation post SCR yielded the narrowest estimate range close to intact controls. At 60° abduction, internal rotation increased after the tear (intact: 38.7 ± 14.4° vs. tear: 49.5 ± 13°, p=0.005). Internal rotation post SCR did not differ significantly from intact controls (SCR: 49.3 ± 10.1°, intact – SCR: 95% CI: −28°, 6.91°, p=0.38). Trapezius transfer showed a trend toward significantly higher internal rotation (65.7 ± 21.1°, intact – trapezius transfer: 95% CI: −55.7°, 1.7°, p=0.067), while latissimus transfer yielded widely variable rotation angle (65.7 ± 38°, intact – latissimus transfer: 95% CI: −85.9°, 31.9°, p=0.68). There were no significant differences in external rotation for any technique at 0° or 60° abduction.

Preliminary evaluation in this cadaveric biomechanical study provides positive evidence in support of use of SCR as a less morbid surgical option than tendon transfers. The cadaveric nature of this study limits the understanding of the motion to post-operative timepoint and the results herein are relevant for otherwise normal shoulders only. Further clinical evaluation is warranted to understand the long-term outcomes related to shoulder function and stability post SCR.

Aim

To evaluate the clinical outcome of a new absorbable, gentamycin loaded calcium sulfate/hydroxyapatite biocomposite (CERAMENT/G) as cavity filler after debridement and removal of infected metalwork in chronic osteomyelitis.

Methods

We report the retrospective study of prospectively collected data from 36 patients with chronic osteomyelitis from implant infection. Treatment included a single stage protocol with removal of the metalwork, debridement augmented with application of CERAMENT/G, stabilization, culture-specific antibiotics and primary skin closure or flap. The biocomposite was used for dead space filling after resection of Cierny-Mader (C-M) stage III and IV chronic osteomyelitis. Data were collected on patient age, comorbidities, operation details, microbiology, postoperative complications and type of fixation or plastic surgery. Primary measure of outcome was recurrence rate.

Introduction

Orthopaedic and trauma surgeons not infrequently encounter the hallucal interphalangeal joint sesamoid (HIPJS) in irreducible traumatic dislocations. However, patients with the classic triad of plantar keratoma beneath a hyperextended interphalangeal (IP) joint associated with stiffness of the first metatarsophalangeal joint tend to present to podiatrists rather than orthopaedic surgeons.

Methods

We present our experience with the HIPJS following first metatarsophalangeal joint (MTP1) arthrodesis in 18 feet of 16 women, aged 42 to 70 years old. Where CT scan was available, volume of the HIPJS was determined using Vitrea Software.

Background

This study analyzes position of the peroneal tendons and status of the superior peroneal retinaculum (SPR) whenever a lateral malleolar bony flake fracture occurs.

Methods

Twenty-four patients had a lateral malleolar bony fleck on anteroposterior ankle radiographs, either in isolation or associated with other hindfoot injuries. We studied size of the bony flecks, presence or absence of peroneal tendon dislocation and pathoanatomy on CT scans.

Unicompartmental knee replacement (UKR) is associated with higher revision rates than total knee replacement and it has been suggested that surgeons should receive specific training for this prosthesis. We investigated the outcome of all UKR in a district general hospital over ten years.

All patients who had received UKR from 2003 to 2013 were identified from theatre records, as were all revision knee arthroplasties. We contacted all patients (or their GP) with no known revision to ascertain UKR status. A life table analysis was used for three categories: all surgeons and types of UKR, Oxford UKR only and Oxford UKR by surgeons with specific training.

There were 319 UKR (one loss to follow up), four types of prosthesis, 21 failures and a 5-year cumulative survival rate of 91.54%. There were 310 Oxford UKR with 17 failures and 5-year survival 93.56%. Surgeons with training in use of Oxford UKR completed 242 replacements with 10 failures and 5-year survival of 95.68%.

In comparison with results for Oxford UKR in 11th annual NJR report, our results are satisfactory and support continued use of this prosthesis in a non-specialist centre. Our established programme of surveillance will monitor the survival of UKR in our hospital.

Introduction:

Although dislocation of the peroneal tendons (PT's) in association with calcaneal fractures has been described over 25 years ago, it frequently passes unrecognised by radiologists and orthopaedic surgeons. This retrospective study aims to determine the prevalence of PT dislocation in association with calcaneal fractures at a single institution and describe systematic steps to avoid missed diagnosis at each stage of management.

Methods:

CT scans of all patients with calcaneal fractures from the Picture Archiving and Communications System (PACS) from 2010 were systematically reviewed. The senior author and a Musculoskeletal Radiologist analysed the images for concomitant dislocation or subluxation of the PT's, utilizing criteria as defined by Ho et al. Further to this we included patients who sustained calcaneal fractures with associated PT dislocation prior to June 2010 and were referred either for primary open reduction or later with post-traumatic osteoarthrosis of the subtalar joint.

The last decade has seen a rise in the use of the gamma nail for managing inter-trochanteric and subtrochanteric hip fractures. Patients with multiple co-morbidities are under high anaesthetic risk of mortality and are usually not suitable for general or regional anaesthesia. However, there can be a strong case for fixing these fractures despite these risks. Apart from aiming to return patients to their pre-morbid mobility, other advantages include pain relief and reducing the complications of being bed bound (e.g. pressure ulcers, psychosocial factors). While operative use of local anaesthesia and sedation has been documented for insertion of extra-medullary femoral implants such as the sliding hip screw, currently no literature is present for the insertion of the gamma nail.

We studied intra-operative and post-operative outcomes of three patients aged between 64 and 83 with right inter-trochanteric hip fractures and American Society of Anesthesiologists (ASA) scores of 4 or more. Consent for each case was obtained after discussion with the patient and family, or conducted with the patient's best interests in mind. All patients received a short unlocked gamma nail, and were operated on within 24 hours of admission. Each patient underwent local injections of Bupivacaine or Lignocaine or both, with Epinephrine, and with one patient receiving nerve block of the fascia iliaca. Each patient received a combination of sedatives under the discretion of the anaesthetist including Midazolam, Ketamine, Propofol, Fentanyl, and/or Haloperidol. Operating time ranged from 30–90 minutes. Patients were managed post-operatively with analgesia based on the WHO pain ladder and physiotherapy.

Our results showed no intra-operative complications in any of the cases. All patients noted improvement in their pain and comfort post-operatively without complications of the operation site. Two patients achieved their pre-morbid level of mobility after undergoing physiotherapy and were subsequently discharged from the orthopaedic team. One patient with ongoing pre-operative medical complications died 5 days after the operation.

This study provides a glimpse into the use of local anaesthetic and sedation on high operative risk patients, and this may be a viable alternative to extra-medullary implants or non-operation. Further research is needed to quantify the risks and benefits of operating within this patient group.

Background

Gram stain microscopy is a routinely requested investigation in the evaluation of septic arthritis in both paediatric and adult patients. Recent evidence suggests that gram stain microscopy has poor diagnostic accuracy in adults with a sensitivity of only 45%, however the diagnostic accuracy remains unknown in children. We sought to establish the diagnostic utility of gram stain microscopy in the diagnosis of septic arthritis in children.

Methods

We conducted a retrospective review of all patients of 16 years and under that underwent aspiration and washout of suspected septic joints in theatre from March 2005 to February 2011. Theatre data were cross referenced with microbiology results and analysed by simple descriptive methods in Excel.

Aligning the tibial tray is a critical step in total knee arthroplasty (TKA). Malalignment, (especially in varus) has been associated with failure and revision surgery. While the link between varus malalignment and failure has been attributed to increased medial compartmental loading and generation of shear stress, quantitative biomechanical evidence to directly support this mechanism is incomplete. We therefore constructed and validated a finite element model of knee arthroplasty to test the hypothesis that varus malalignment of the tibial tray would increase the risk of tray subsidence.

INTRODUCTION

Intramedullary nail fixation has been used for successful treatment of long bone fracture such as humerus, tibia and femur. We look at the experience of our trauma unit in treating long bone fracture using the AO approved Expert femoral/tibial nail and proximal femoral nail antirotation (PFNA). We look at the union and complication rates in patients treated with AO approved nailing system for pertrochanteric, femoral and tibial shaft fracture.

METHODS

We carried out retrospective case notes review of patients that underwent femoral and tibial nailing during the period of study- October 2007 to August 2009. All patients were treated using the AO approved nailing system. We identified all trauma patients that underwent femoral and tibial nailing through the trauma register. Further information was then obtained by going through medical notes and reviewing all followed-up X-rays stored within the online radiology system.

On 7 July 2005, four bombs were detonated on the London transport system. Three of these bombs exploded almost simultaneously at 08:50h affecting the underground tube network at Aldgate, King's Cross and Edgware Road stations. The fourth bomb exploded at 09:47h on a double-decker bus in Tavistock Square. There were 54 deaths in total at the scenes and over 700 injured.

194 patients were brought to the Royal London Hospital. 167 were assessed in a designated minor injuries unit and discharged on the same day. 27 patients were admitted of whom 7 required ITU care, 1 died in theatre and 1 died post-operatively. The median Injurity Severity Score (ISS) in this group of patients was 6 (range 0-48) and the mean ISS was 12. The general pattern of injury in the critically ill patients was of mangled lower limbs and multiple, severely contaminated fragment wounds. Hepatitis B prophylaxis was administered to those patients with wounds contaminated by foreign biological material. 11 primary limb amputations were performed in 7 patients. 9 limb fasciotomies, 5 laparotomies and 1 sternotomy were carried out. 3 patients had blast lung injury. All patients who underwent primary amputations and debridement received further regular inspections in theatre. These inspections formed the majority of our theatre work. Under no circumstance was initial reconstructive surgery attempted. Delayed primary closure and split skin grafting of all wounds was completed by the end of the second week. There have been no sepsis-related deaths.

Our experience at The Royal London has allowed us to revisit the principles of blast wound management in a peacetime setting. A number of lessons were learned regarding communication and resource allocation. A multi-disciplinary approach with the successful execution of a major incident plan is the key to managing an event of this magnitude.

We therefore constructed a finite element model of knee arthroplasty to test the hypothesis that varus malalignment of the tibial tray would increase the risk of tray subsidence.

Methods: Cadaver Testing: Fresh human knees (N = 4) were CT scanned and implanted with a TKA cruciate-retaining tibial tray (Triathlon CR. Stryker Orthopaedics). The specimens were subjected to ISO-recommended knee wear simulation loading for up to 100,000 cycles. Micromotion sensors were mounted between the tray and underlying bone to measure micromotion. In two of the specimens, the application of vertical load was shifted medially to generate a load distribution ratio of 55:45 (medial: lateral) to represent neutral varus-valgus alignment. In the remaining two specimens, a load distribution ratio of 75:25 was generated to represent varus alignment.

Finite element analysis: qCT scans of the tested knees were segmented using MIMICS (Materialise, Belgium). Material properties of bone were spatially assigned after converting bone density to elastic modulus. A finite element model of the tibia implanted with a tibial tray was constructed (Abaqus 6.8, Simulia, Dassault Systèmes). Boundary conditions were applied to simulate experimental mounting conditions and the tray was subjected to a single load cycle representing that applied during cadaver loading.

Results: The two cadaver specimens tested at 55:45 medial:lateral (M:L) force distribution survived the 100,000 cycle test, while both cadaver specimens tested at 75:25 M:L force distribution failed. The finite element model generated distinct differences in compressive strain distribution patterns in the proximal tibia. A threshold of 2000 microstrain was used for fatigue damage in bone under cyclic loading. Both specimens loaded under 75:25 M:L distribution demonstrated substantially larger cortical bone volumes in the proximal tibial cortex that were greater than this fatigue threshold.

Discussion and Conclusion: We validated a finite element model of tibial loading after TKA. Local compressive strains directly correlated with subsidence and failure in cadaver testing. A significantly greater volume of proximal tibial cortical bone was compressed to a strain greater than the fatigue threshold in the varus alignment group, indicating an increased risk for fatigue damage. This model is extremely valuable in studying the effect of surgical alignment, loading, and activity on damage to proximal bone.

Percutaneous fluoroscopically asseisted iliosacral screw insertion has become one of the most popular methods of stabilisation of the posterior aspect of the vertically unstable pelvis. Screw malpositioning rates range from 0 to 10%. Screw misplacement can cause injury to the iliac and gluteal vessels, L4 to s1 nerve roots and sympathetic chain.

We performed two radiographic studies on dry human bones to seek safe radiographic landmarks for insertion of iliosacral screws.

Part 1: Two parallel linear densities are always present on lateral plain radiographs of the lumbosacral spine and pelvis. Using wire markers on pelvic bones, we accurtely define the origins of these “pelvic lines”. Steel wires of different lengths were placed along the iliopectineal and arcuate lines of the pelvis. The shorter wire stopped at the anterior limit of the sacroiliac joint. The longer wire extended further along the entire course of the medial border of the ilium to the iliac crest posteriorly. We demonstrate that each “pelvic line” represents the sharp bony ridge that forms the anterosuperior limit for insertion of the iliosacral screws.

Part 2: In a second experiment on dry pelvis, we inserted balloons filled with radio-opaque contrast medium into the spinal canal of the sacrum and exiting through the anterior and posterior sacral foramina on either side. Plain lateral radiographs and CT scan with reformatted images were obtained. We present a previously undescribed radiological sign on plain lateral radiographs of the lumbosacral spine. The inferior and posterior boundaries of the “acorn sign” are delineated. Together, the “pelvic lines” and “acorn sign” provide accurate landmarks for the safe insertion of iliosacral screws. Iliosacral screws should be contained within this “acorn sign” to avoid injury to the nerve roots and below the “pelvic lines” to safeguard the iliac vessels and lumbosacral trunk.

Using a comparative, longitudinal study design, we studied the effects of early weight bearing and ankle mobilisation following acute repair of ruptured Achilles tendon.

Patients in Group 1 (22 males and 4 females; mean age 44.7 years [range 31–69], 11 right- and 15 left-sided ruptures) were immobilised with their ankle in gravity equinus, and encouraged to fully weight bear. They received a single cast change at 2 weeks, when the ankle was accommodated in an anterior splint, allowing full plantarflexion but not dorsiflexion above neutral.

Patients in Group 2 (23 males and 4 females; mean age 43.8 years [range 30–67], 11 right- and 16 left-sided ruptures) were immobilised in full equinus. They received a cast change at 2 and 4 weeks, when the ankle was immobilised in a plantigrade position. They were advised to weight bear 4 weeks after the operation.

Patients in Group 1 attended less outpatient visits and completely discarded their crutches at an average of 2.5 weeks after the operation. Group 2 discarded their crutches at an average of 5.7 weeks after from the operation (p=0.013). At ultrasound scan, the average thickness of the repaired tendon was 12.1 mm (SD 2), with no difference in the thickness of the ruptured tendon regardless of the method of post-operative management. There was no significant difference in isometric strength between the two groups of patients. A greater proportion of patients in Group 1 were satisfied with the results of surgery (p=0.04).

Early weight bearing with the ankle plantigrade is not detrimental to the outcome of repair following rupture of the Achilles tendon, and shortens the time needed for rehabilitation. However, strength deficit and muscle atrophy are not prevented.

Two hundred and thirty six posterior stabilized total knee arthroplasties were performed consecutively. Twenty seven patellar clunk syndromes were identified in 25 patients. Insall-Salvati ratio, position of joint line, postoperative patellar height and anterior-posterior position of tibial tray were measured. We found that post-operative low-lying patella (p< 0.001) and anterior placement of tibial tray (p=0.011) was associated with patellar clunk syndrome. Thirteen patients had bilateral total knee replacements of the same prosthesis (5 bilateral AMK and 8 bilateral IB) but unilateral patellar clunk syndrome. The non-clunk sides were used as control for comparison with the clunk sides. The congruency and tilting of the patellar button in the skyline view were documented. We observed that the congruency of the patella button was less satisfactory in the clunk side (p=0.019).
