Introduction

In total knee arthroplasty, the aim is to relieve pain and provide a stable, functional knee. Sagittal stability is crucial in enabling a patient to return to functional activities. Knee implants with a medial pivot (MP) design are thought to more accurately reproduce the mechanics of the native joint, and potentially confer greater antero-posterior stability through the range of flexion than some other implant designs.

Aim

This study aims to compare the sagittal stability of four different total knee arthroplasty implant designs. Method

Comparison was made between four different implant designs: medial pivot (MP), two different types of cruciate retaining (CR1 and CR 2) and deep dish (DD). A cohort of 30 Medial Pivot (MP) knees were compared with matched patients from each of the other designs, 10 in each group. Patients were matched for age, body mass index and time to follow up.

Clinical examination was carried out by an orthopaedic surgeon blinded to implant type, and sagittal stability was tested using a KT1000 knee arthrometer, applying 67N of force at 30˚ and 90˚.

Purpose

Femoral component malrotation is a common cause for persisting symptoms and revision following total knee arthroplasty (TKA). There is ongoing debate about the most appropriate use of femoral landmarks to determine rotation. The Sulcus Line (SL, See Figure 1) is a three-dimensional curve produced from multiple points along the trochlear groove. Whiteside's Line, also known as the anteroposterior axis (APA), is derived from single anterior and posterior points. The purposes of the three studies presented are to i) assess the SL in a large clinical series, ii) demonstrate the effect of parallax error on rotational landmarks, and iii) assess the accuracy of a device which transfers a geometrically corrected SL onto the distal cut surface of the femur.

Methods

The first study assessed the SL using a large, single surgeon series of consecutive patients (n=200) undergoing primary TKA. The postoperative CT scans of patients were examined to determine the final rotational alignment of the femoral component. In the second study measurements were taken in a series of 3DCT reconstructions of osteoarthritic knees (n=44) comparing the rotational landmarks measured along either the mechanical axis or the coronal axis of the trochlear groove. The third study assessed the accuracy of a novel trochlear alignment guide (TAG) using cadavers (n=10)

We have assessed the comparative function of young patients with hip or knee replacement using a high level function score.

A high level function score was developed for assessing running, walking, stair climbing and recreational activity. This was used on a series of patients after a total of 153 joint replacements including 99 THR and 54 TKR.

The groups were comparable with respect to age (average 57 years), gender and follow-up. Comparison between the groups showed that there was no significant difference in walking, stair-climbing and recreational activity. Patients with THR scored better on running ability and overall total score. Other differences noted included lower scores in bilateral joint replaced patients and posterior cruciate sacrificed knees.

Young patients undergoing hip or knee replacement have similar functional outcomes using a high activity function score. There were trends towards superior running ability and overall scores in the THR group.

Our aim was to assess the clinical and radiological medium-term outcome of a series of 153 consecutive total hip arthroplasties performed by two surgeons in Christchurch using the ABG hydroxyapatite-coated femoral stem and Duraloc 100-series uncemented acetabulum.

An independent clinical review using interviews and patient notes and radiological review of immediate postoperative and most recent x-rays was carried out.

At six to eleven years with approximately 95% follow-up, there were no stem failures, but two revisions following a fracture from significant trauma. Two cups were revised for loosening and one cup bone grafted for osteolysis. A dislocation occurred in 4.7% of cases requiring two liner exchanges and one cup revision. A low incidence of thigh pain and excellent radiographic results of the stem with high rates of bony ingrowth and no incidence of significant loosening was a feature of this series.

We report excellent results at medium-term follow up for an unceme4nted HA- coated femoral stem.
