Modular tapered stems have become increasingly popular in femoral revisions and stem subsidence remains a reported clinical problem. Computer modeling and biomechanical testing demonstrated a minimum initial line-to-line fit of 25 mm was required to reduce the risk of subsidence. We examined XR imaging of 110 consecutive femoral revisions by a single surgeon using two different modular tapered stems to determine if this biomechanical threshold was of clinical relevance. 72% of the revisions were done for aseptic loosening and 28% for periprosthetic fracture or infection. Stem subsidence of any degree was observed in 24 (21.8%), while 12 (10.9%) demonstrated substantial subsidence of > 10 mm. We matched this cohort to 12 patients who had no stem subsidence. The average age of patients with subsidence was 59 years (43 to 79 years). The average of patients without subsidence was 66 years (41 to 77 years). Each group had 7 men and 5 women. Stem subsidence was observed in the first 3 months post-surgery. The demographics, bone deficiency, stem design, and stem diameter were similar between groups.

83% of patients with substantial stem subsidence had less than 25 mm of line-to-line fit compared to 17% of patients without subsidence. The 12 cases of no stem subsidence had a mean line-to-line fit of 48 mm (25 to 55 mm). 75% of patients with substantial stem subsidence had also undergone an extended trochanteric osteotomy (ETO) compared to 33% of patients without subsidence.

We conclude that there is a positive correlation with the biomechanical testing parameters and substantial stem subsidence. ETO was found to be associated with higher stem subsidence.

Introduction

Osteoarthritis commonly affects the first metatarsophalangeal joint. Stress across this joint has been postulated to increase the incidence of osteoarthritis. Certain foot structures have been associated with a higher incidence of osteoarthritis of the big toe. Utilizing finite elemental analysis, bone stress across the first metatarsophalangeal joint was calculated during mid stance phase of gait and compared in different foot structures.

Method

A geometrically accurate three dimensional model of the first metatarsophalangeal joint was created utilising a high resolution 7 tesla MRI and Mimics v14 imaging software. Planus, rectus and cavus feet were simulated by varying the metatarsophalangeal declination angle to 10.1, 20.2 and 30.7 degrees, respectively. A non-manfold computer aided design technique in Mimics v14.2 and finite element method in ANSYS v12 FE were utilised to create the boundary conditions, representing the double support stance phase of gait. Using information from 61 asymptomatic patients with different foot types walking over a Novel emed-x plantar pressure measuring system, plantar loading conditions were applied. Finite elemental analysis was used to predict stress in the first metatarsophalangeal joint in the different foot types.

Scarf osteotomy is widely used as a surgical treatment for hallux valgus. It is a versatile osteotomy, allowing shortening, depression or medial displacement of the capital fragment but it remains uncertain how stresses within the bone subsequently vary. The aim of this study was to design a computerised model to explore the effect on bone stress of changing the position of bony cuts for a scarf osteotomy.

A computerised image was constructed using finite element analysis. This utilises a mathematical technique to form element equations which represent the effect of applied force to the object appropriate to each finite element. Maximum bone stresses were then measured using different osteotomy variables. The osteotomy variables studied were the length of the longditudinal cut, apex of the distal cut to articular cartilage, resection level of the longditudinal cut and combinations of these variables. A saw bone model was used to test the findings of the study.

The results of this study show that lowering the longditudinal resection level and shortening via the distal cut beyond 6 mm will decrease bone stress. Additionally, raising the longditudinal resection level and shortening via the proximal cut caused an increase in bone stress. A saw bone model confirmed the findings of the study.

In conclusion, our experience is that finite element analysis is a very useful model in studying the bony stresses for a scarf osteotomy and assists in optimising the direction and angle of bony cuts used.

A prospective series of patients undergoing intradiscal electrothermal therapy for treatment of lower back pain refractory to physiotherapy.

33 patients with a mean follow up of 16 months were assessed pre- and postoperatively at Mayday University Hospital between 1999–2001 using visual analogue scores and SF-36. These patients failed to show an improvement with physiotherapy and had no evidence of a significant disc prolapse according to MRI with their back pain being reproduced at one or more disc levels on provocative discography. All procedures were performed as day cases with a temperature of 90 degrees being reached in all patients.

Baseline and follow-up outcome measures indicated the SF-36 mean improvement of pcs to be 7.05 (P< 0.001) and the mean improvement of mcs 10.05 (P< 0.001) following IDET with a mean change of 1.5 in the visual analogue score. Overall 25 patients reported a noticeable improvement in their back pain symptoms with 3 patients recording worsening symptoms and the other 5 patients remaining unchanged

Surgical complications included 1 breakage of the catheter within the patient and a disc prolapse at the level of surgery that required subsequent discectomy. 30% of patients were noted to have a temporary foot drop postoperatively which was due to inadvertently performing a lumbar plexus block on infusion of local anaesthetic before catheter insertion. There was no reported discitis, deep infection or nerve root injury.

Patients with proven discogenic lower back pain that have not responded to physiotherapy have generally improved following IDET in this series. However, this improvement is not as dramatic as first indicated in preliminary studies and includes a small number of patients which noted a deterioration in their symptoms, one of which requiring a discectomy as a direct result of the procedure.

The medical treatment of eight patients with paraparesis associated with Paget's disease of the vertebrae is described. Treatment, for 3 to 87 months, with calcitonin or with diphosphonates produced marked clinical improvement in seven of these patients. From this series and a review of 19 additional case reports it is concluded that favourable clinical response is seen in about 90 per cent of patients, and that this may occur very rapidly. Results are as good or better than those obtained by surgical decompression. It seems possible that paraparesis in some cases may be due to diversion of blood supply from the spinal cord to the highly vascular Pagetic bone giving rise to a vascular "steal" syndrome. It is suggested that medical treatment should be used more widely to avoid or delay the need for operation and reduce the risks of recurrence. These patients, however treated, require lifelong follow-up because relapses are common.

The clinical features of eight patients with myositis ossificans progressiva are described and the effects of treatment with the diphosphonate EHDP, together with surgical removal of ectopic bone, are assessed. Early correct diagnosis remains unusual, mainly because the significance of the short great toes is unrecognised, and because myositis may be mistaken for bruising, sarcoma or mumps. The diphosphonate disodium etidronate (EDHP) was given to all patients in an attempt to suppress calcification of new lesions; in five of them ectopic bone was removed during the treatment. EHDP sometimes delayed the mineralisation of newly formed bone matrix after surgical removal but this delay could not be predicted. The variable effect of EHDP may depend particularly on the amount absorbed and on the activity of new bone formation.
