Osteonecrosis is a potentially devastating condition with poorly defined pathogenesis that can affect several anatomical areas with or without a previous traumatic insult. Post traumatic osteonecrosis (PON) in the foot and ankle has been commonly described in the talus and navicular but rarely in the distal tibia. PON of the distal tibia is a rarely reported and infrequent complication of fracture dislocations of the ankle. Its scarcity can lead to misdiagnosis and inappropriate management due to a lack of clinical knowledge or suspicion with resultant severe functional compromise. We aim to highlight the clinical and radiological features of PON of the distal tibia and report the findings in a series of four patients following a fracture dislocation of the ankle. Three patients sustained a SER4 fracture dislocation and one patient sustained a PER4 fracture dislocation in keeping with standard patterns of injury seen in most trauma units. In each case, PON of the distal tibia presented with progressive anterolateral tibial plafond collapse and valgus deformity of the ankle. The radiological features previously reported in the literature are based on plain film x-ray, CT and MRI but no description of SPECT-CT findings. One of the patients in the series underwent SPECT-CT following clinical suspicion of PON and thus we describe the findings not previously reported. Our objective is to highlight this rare condition as a potential cause for ongoing pain following fracture dislocation of the ankle as well as advocating the use of SPECT/CT as a useful imaging modality to aid in the diagnosis.

Background

This study analyzes position of the peroneal tendons and status of the superior peroneal retinaculum (SPR) whenever a lateral malleolar bony flake fracture occurs.

Methods

Twenty-four patients had a lateral malleolar bony fleck on anteroposterior ankle radiographs, either in isolation or associated with other hindfoot injuries. We studied size of the bony flecks, presence or absence of peroneal tendon dislocation and pathoanatomy on CT scans.

Introduction

Lag screw cut-out following fixation of unstable intertrochanteric fractures in osteoporotic bone remains an unsolved challenge. A novel new device is the X-Bolt which is an expanding type bolt that may offer superior fixation in osteoporotic bone compared to the standard DHS screw type device.

Aims

The aim of this study was to test if there was a difference in cut-out using the X-Bolt implant compared with the standard DHS system

Introduction

Problematic bone defects are encountered regularly in orthopaedic practice particularly in fracture non-union, revision hip and knee arthroplasty, following bone tumour excision and in spinal fusion surgery. At present the optimal source of graft to ‘fill’ these defects is autologous bone but this has significant drawbacks including harvest site morbidity and limited quantities.

Bone marrow has been proposed as the main source of osteogenic stem cells for the tissue-engineered cell therapy approach to bone defect management. Such cells constitute a minute proportion of the total marrow cell population and their isolation and expansion is a time consuming and expensive strategy.

In this study we investigated human bone marrow stem cells as a potential treatment of bone defect by looking at variability in patient osteogenic cell populations as a function of patient differences. We produced a model to predict which patients would be more suited to cell based therapies and propose possible methods for improving the quality of grafts.

Methods

Bone marrow was harvested from 30 patients undergoing elective total hip replacement surgery in Musgrave Park Hospital, Belfast (12 males, 18 females, age range 52-82 years). The osteogenic stem cell fraction was cultured and subsequently analysed using colony forming efficiency assays, flow cytometry, fluorescence activated cell sorting and proteomics.

Aims and Objectives Lateral condyle fractures can be difficult diagnose and the treatment still remains controversial. It is well known that these fractures are prone to a number of complications, both early and late. The aim of this paper was to review the treatment practice of lateral condyle fractures presenting to a children’s hospital fracture unit over the past 5 years to identify any consistency in the management of these fractures. We also aimed to try and determine if a particular treatment method was more favourable than others in terms of complications and the need for further surgery with a view to developing a treatment protocol.

Methods: We conducted a chart and x-ray review of all lateral condyle fractures treated operatively from December 1998 to August 2004. We recorded patients’ age, sex, side of injury and month of injury. The fractures were classified according to the Milch classification. We also measured the preoperative and postoperative fracture displacement. We recorded the nature of surgery (Examination Under Anaesthetic (EUA) and casting, Manipulation Under Anaesthetic (MUA) and wiring and Open Reduction and wiring). We documented whether the wires were percutaneous or buried. Length of time in cast and length of time to wire removal were also noted. Finally any complications and the need for further surgery were documented.

Results: 90 patients were identified. 72% were male and 28% female, with an average age of 5.6. 28% of injuries were right sided, 72% were left sided. 21 (23%) patients were Milch Type 1 fractures and 66 (73%) were Type II fractures. Preoperative fracture classification was unavailable for 3 patients. In 78 patients we were able to determine the initial fracture displacement. 8 (9%) patients were displaced < 2 mm, 18 (20%) were displaced 2–4 mm and 52 (58%) were displaced > 4 mm. 7 patients (10%) had associated elbow dislocations – all of these were Milch type II fractures. 5 patients had EUA and casting, 19 had MUA and K wiring and 63 had open reduction and wiring. In the 19 patients who had MUA and K wiring, 13 were percutaneous and 6 were buried. In the open reduction and wiring group 59 patients had their wires buried and 6 were percutaneous. 1 patient did not have that information recorded.

The average time in cast was 41 days. In those with buried wires average length of time to wire removal was 63 days. Average percutaneous wire removal was at 42 days. For the 5 patients undergoing EUA and casting residual displacement was < 2 mm in all. 2 of these patients (40%) had complications of lateral spur formation and delayed union. For the 19 having MUA and k wiring, 14 had a post op displacement of< 2 mm and 5 had 2–4 mm displacement. 3 of the 14(21%) had the complications of spur formation, pin site infection and wire prominence. 2/5 (40%) of those with residual displacement of 2–4 mm developed complications, 1 patient had ulceration of wires through the skin and another had loss of position requiring further surgery.

In the patients treated with open reduction and wiring 51 had a residual displacement of < 2 mm, 14 had 2–4 mm residual displacement and 1 remained displaced > 4 mm. 11/51 (22%) in the first category developed complications. 6 were problems with the wires, 1 lost position requiring re-operation, 1 lateral spur development. 2 malunions and 1 delay in ossification of the lateral condyle. In the 2–4 mm group 8/14 (57%) developed complications. – 2 wire ulcerations, 2 wound infections, 1 non-union and 3 malunions. Finally the 1 patient with residual displacement > 4 mm developed a malunion requiring further operative intervention.

In total 5 patients had further surgery - 1 patient for wire prominence 2 for loss of position and 2 patients required corrective surgery for malunion.

Conclusion: This study highlights the variety in treatment methods for these fractures. Complications occurred in all treatment groups. The short term complications such as wire problems and initial loss of position had no long term sequelae. All malunions occurred in the open reduction and wiring group, despite 2 patients having post operative fracture displacement of < 2 mm. The patient with a non union was a late referral but underwent open reduction and wiring at our unit and subsequently healed. We recommend that displaced fractures should be reduced either closed or open and all fractures should be secured with k wires to prevent loss of position. These should be bent and buried allowing them to remain insitu for 3 months. Postoperative casting should be for 6 weeks. These fractures need to be followed closely at fracture clinic for the short and long term problems they can develop.

Introduction and Aims: Currently, multiple femoral component types and sizes exist for primary total hip arthroplasty. However, component sizes for small femoral geometry are generally not available. The purpose of this study is to present the short-term use of a femoral component with sizes that extend into small femoral morphometry applications.

Method: Between November 2001 and December 2003, 20 primary THA cases and three revision THA cases were performed utilising a non-cemented, dual threaded, cone shaped (DTCS) modular femoral component manufactured in off-the-shelf sizes, which include those sizes for small femora. The components are made of CoCr and include a size ‘Z’ (19mm proximal, 9mm distal) and a size ‘Y’ (17mm proximal, 8mm distal). Both components have hydroxyapatite coating for stimulating increased bone on-growth and a modular neck allowing intra-operative adjustments of leg length, version, offset and neck length.

Results: The average patient follow-up was 10 months (range 64 days to 27 months). There were 19 (83%) hips in which the ‘Z’ component was used, and four (17%) hips with the ‘Y’ component. Radiographic evaluation revealed well-fixed and positioned components with evidence of bone densing in areas in intimate contact with the DTCS component. Radiographic evidence of minor stress shielding was observed in the greater trochanter (Gruen Zone 1) and the proximal calcar/neck cut region (Gruen Zone 7). Two revision cases (8%) required the additional use of a 6cm modular extension component (MEC) to bridge a proximal femoral deficiency. Two cases (8%) required adjunctive strut allografting at the time of surgery to protect a thin or deficient femoral cortex. There were no reported postoperative complications related to the femoral component. There was no disassociation of the modular neck from the femoral stem and there was no incidence of femoral component fracture.

Conclusion: While expanding component profile offerings into larger sizes is common, developing similar component designs for abnormally small femora is uncommon, beyond the scope of the materials used and only done as a ‘custom’ order. The DTCS modular femoral component used affords a versatile option when presented with cases involving small femoral morphometry. We conclude that the DTCS component in smaller sizes is promising and warranted for continued use.
