Introduction

Anterior cruciate ligament reconstruction (ACLr) is the most widely published operation in the orthopaedic literature. Over recent years there has been increased interest in the surgical technique and role of concomitant procedures performed during ACLr. The National Ligament Registry (NLR) collects robust data on ACLr performed in the UK. In this registry analysis we explore trends in ACLr surgery and how they relate to published literature and the growing industry portfolio available to surgeons.

Methods

Using data from the NLR, 14,352 ACLr performed between 2013–2021 were analysed. High impact papers on ACLr were then cross referenced against this data to see if surgical practice was influenced by literature or whether surgical practice dictated publication. Common trends were also compared to key surgical industry portfolios (Arthrex, Smith and Nephew) to see how new technology influenced surgical practice.

Individuals with multi-compartment knee osteoarthritis (KOA) frequently experience challenges in activities of daily living (ADL) such as stair ambulation. The Levitation “Tri-Compartment Offloader” (TCO) knee brace was designed to reduce pain in individuals with multicompartment KOA. This brace uses novel spring technology to reduce tibiofemoral and patellofemoral forces via reduced quadriceps forces. Information on brace utility during stair ambulation is limited. This study evaluated the effect of the TCO during stair descent in patients with multicompartment KOA by assessing knee flexion moments (KFM), quadriceps activity and pain.

Nine participants (6 male, age 61.4±8.1 yrs; BMI 30.4±4.0 kg/m2) were tested following informed consent. Participants had medial tibiofemoral and patellofemoral OA (Kellgren-Lawrence grades two to four) diagnosed by an orthopaedic surgeon.

Joint kinetics and muscle activity were evaluated during stair descent to compare three bracing conditions: 1) without brace (OFF); 2) brace in low power (LOW); and 3) brace in high power (HIGH). The brace spring engages from 60° to 120° and 15° to 120° knee flexion in LOW and HIGH, respectively. Individual brace size and fit were adjusted by a trained researcher.

Participants performed three trials of step-over-step stair descent for each bracing condition. Three-dimensional kinematics were acquired using an 8-camera motion capture system. Forty-one spherical reflective markers were attached to the skin (on each leg and pelvis segment) and 8 markers on the brace. Ground reaction forces and surface EMG from the vastus medialis (VM) and vastus lateralis (VL) were collected for the braced leg. Participants rated knee pain intensity performing the task following each bracing condition on a 10cm Visual Analog Scale ranging from “no pain” (0) to “worst imaginable pain” (100).

Resultant brace and knee flexion angles and KFM were analysed during stair contact for the braced leg. The brace moment was determined using brace torque-angle curves and was subtracted from the calculated KFM. Resultant moments were normalized to bodyweight and height. Peak KFMs were calculated for the loading response (Peak1) and push-off (Peak2) phases of support. EMG signals were normalized and analysed during stair contact using wavelet analysis. Signal intensities were summed across wavelets and time to determine muscle power.

Results were averaged across all 3 trials for each participant. Paired T-tests were used to determine differences between bracing conditions with a Bonferroni adjustment for multiple comparisons (α=0.025).

Peak KFM was significantly lower compared to OFF with the brace worn in HIGH during the push-off phase (p Table 1: Average peak knee flexion moments, quadriceps muscle power and knee pain during stair descent in 3 brace conditions (n=9).

Quadriceps activity, knee flexion moments and pain were significantly reduced with TCO brace wear during stair descent in KOA patients. These findings suggest that the TCO assists the quadriceps to reduce KFM and knee pain during stair descent. This is the first biomechanical evidence to support use of the TCO to reduce pain during an ADL that produces especially high knee forces and flexion moments.

Aims

To determine the impact of COVID-19 on orthopaediatric admissions and fracture clinics within a regional integrated care system (ICS).

Methods

A retrospective review was performed for all paediatric orthopaedic patients admitted across the region during the recent lockdown period (24 March 2020 to 10 May 2020) and the same period in 2019. Age, sex, mechanism, anatomical region, and treatment modality were compared, as were fracture clinic attendances within the receiving regional major trauma centre (MTC) between the two periods.

It is estimated that a quarter to half of all hospital waste is produced in the operating room. Recycling of surgical waste in the perioperative setting is largely underutilized, despite the fact that many of the materials being discarded can be potentially recycled safely and easily. Given this mounting waste production, recycling programs have become increasingly popular. Therefore, the primary objective of this study is assess the effect of these recent eco-friendly polices by determining the amount of waste and recycling produced in the pre-operative and operative time period for several orthopaedic subspecialties.

Surgical cases were prospectively chosen and assigned to an orthopaedic subspecialty category, which included trauma, arthroplasty, sports, foot and ankle, upper extremity, and paediatrics. The preoperative phase began with the opening of the surgical case carts and concluded with the end of skin preparation. The intraoperative period began after skin preparation was complete, and concluded after the operating room was cleaned. At the end of the preoperative period all surgical waste was weighed and divided into recyclables and non-recyclables. Following the intraoperative period, surgical waste was divided into recyclables, non-recyclables, linens, and biohazardous waste streams. All bags were weighed in a standardized fashion using a portable hand held scale. The primary outcome of interest was the amount of recyclable waste produced per case. Secondary outcomes included the amount of nonrecyclable, biohazardous and total waste produced during the same time intervals. Statistical analysis was then completed using (ANOVA) to detect differences between specialties.

This study included 55 procedures collected over a 1-month period at two hospitals from October 2017 to November 2017. A total of 341 kg of waste was collected with a mean mass of 6.2 kg per case. In terms of primary outcomes, arthroplasty surgery produced a significantly greater amount of recyclable waste per case in the preoperative (2327.9 g)and intraoperative (938.6 g)period. It also produced the greatest amount of total recyclable waste per case, resulting in a significantly greater ratio of waste recycling per case then nearly all other specialties in the preoperative (86.2%) and intraoperative period (14.5%). In terms of secondary outcomes, arthroplasty surgery similarly produced a significantly greater amount of nonrecyclable waste per case then all other specialties (5823.6 g), the majority of which was produced during the intraoperative period (5512.9 g). Arthroplasty surgery also produced a significantly greater amount of biohazardous waste then all other specialties (409.3 g). The majority of surgical waste was produced in the intraoperative period compared to the preoperative period. In the preoperative period an average of 74.4% of waste was recyclable, compared to 7.6% of waste produced during the intraoperative period. In total, the average amount of waste recycled per case was 25.6%. Biohazardous waste only constituted 1.8% of the total waste mass.

Orthopaedic surgery is a significant source of waste production in our hospital system. Among orthopaedic subspecialties, arthroplasty is one of the largest waste producers, but also has the highest potential for recycling of materials. Effective OR recycling programs can significantly reduce our ecological footprint by diverting waste from landfills. In particular, the preoperative period has significant potential for landfill diversion as our study showed that nearly three quarters of all waste in this period can be effectively recycled.

Aims

The current global pandemic due to COVID-19 is generating significant burden on the health service in the UK. On 23 March 2020, the UK government issued requirements for a national lockdown. The aim of this multicentre study is to gain a greater understanding of the impact lockdown has had on the rates, mechanisms and types of injuries together with their management across a regional trauma service.

Methods

Data was collected from an adult major trauma centre, paediatric major trauma centre, district general hospital, and a regional hand trauma unit. Data collection included patient demographics, injury mechanism, injury type and treatment required. Time periods studied corresponded with the two weeks leading up to lockdown in the UK, two weeks during lockdown, and the same two-week period in 2019.

Periprosthetic joint infection is a significant complication of total hip arthroplasty. The PRSThesis of Antibiotic Loaded Acrylic Cement (PROSTALAC) system can improve health related quality of life during a two step treatment approach for infection resolution. We investigated quality of life with the PROSTALAC in situ and also compared subjects who underwent second stage surgery with those who retained the PROSTALAC on a long term basis.

Twenty nine subjects were enrolled pre-PROSTALAC insertion, recording demographics, physical demand level and comorbidities. Subjects were then followed out to 24 months with either the PROSTALAC in situ or post revision for those who underwent the second stage surgery. Quality of life was evaluated using the Western Ontario McMaster Osteoarthritis Index (WOMAC) and RAND 36-Item Health Survey (RAND-36). Infection resolution was also determined for all subjects enrolled.

Three subjects died and 22/26 (84%) completed the evaluation. Overall 26 (85%) infections resolved. Following PROSTALAC insertion, WOMAC pain and function scores improved within three to six months and did not change out to 24 months. Physical function, bodily pain and vitality also significantly improved within three to six months postoperatively. Only seven (32%) subjects underwent second stage surgery. These individuals were more likely to be high demand (p=0.03) and trended towards being younger, male, with fewer comorbidities and lower BMI (p<0.10). There was no difference in WOMAC scores at 24 months between those who underwent second stage revisions and those who retained the PROSTALAC.

The use of a PROSTALAC implant improves health related quality of life. Long term retention of the PROSTALAC implant may be appropriate for low demand patients and considered for potentially high risk surgical candidates.

Introduction

Although DDH is one of the most common skeletal dysplasias (incidence 1.5 cases per 1000 births), it remains slow and costly to recruit large-scale patient cohorts for powerful genetic association studies. In this work we have successfully used the NJR as a platform to generate a DDH biobank of 907 individuals, upon which we have conducted the first ever genome-wide association study (GWAS) for DDH.

Methods

5411 patients recorded as having a hip replacement for ‘hip dysplasia’ between March 2003 and December 2013 were approached to participate in the study. Following filtering by questionnaire for non-DDH cases and non-European Caucasians, 907 patients returned a completed saliva sample. A randomly selected sample of individuals participating on the UK Household Longitudinal Study that had been previously genotyped using the same platform were used as controls at a case:control ratio of 1:4. A further data set consisting of 332 cases, 1375 controls and 26 variants was used to replicate the top signals.

Purposes of Study and Background

This study investigates the role of the patient and the therapeutic relationship with their medical practitioner in resolving CLBP (chronic low back pain). Many cases of chronic pain persist due to the stress response. The body is equipped with natural self-healing mechanisms that must be activated by the patient. This needs facilitation by someone trained to engage the patient in this process. Evidence shows that conventional medication; surgery and physical rehabilitation do not always resolve chronic pain. To ensure that the stress response doesn't sabotage biomedical intervention the patient may need to address any problems obstructing a healthy lifestyle. This means supporting healthy relationships, and good health in other areas such as: professional life, financial matters, beliefs and creativity.

Summary of Methods used and Results

Recent Case Studies are described where patients presented with severe CLBP. The Roland Morris Low Back Pain Disability Questionnaire was used to measure outcomes. Treatment included education about the ANS (autonomic nervous system); an active health plan in which the patient takes responsibility for their own healing; and movement guidelines to support the parasympathetic NS. The results show a significant pain reduction and increase in mobility.

Background and objectives

Local bone-related adverse events occur more frequently following metal-on metal hip resurfacing (MOMHR) versus convention total hip arthroplasty (THA). High local tissue levels of cobalt and chromium may contribute to impaired bone health, however the systemic effects on bone of exposure to elevated metal levels after MOMHR are unknown.

Methods

In this cross-sectional study we compared whole body bone mineral density (WB-BMD) and biochemical markers of bone turnover in 31 healthy male subjects at a mean of 8 years after MOMHR versus 31 individually age and time since surgery matched male subjects after conventional THA. All subjects had well-functioning prostheses and were in good self-reported health as assessed by Oxford Hip Score and EQ-5D questionnaire. WB-BMD was measured by dual energy x-ray absorptiometry and adjusted for pre-morbid osteoporosis risk factors using the FRAX tool, and for the presence of the metal prostheses using identical exclusion regions. Bone turnover markers were measured on fasting morning serum or 24hr urine collection by electro-chemiluminescent assay. Cobalt and chromium were measured by ICP-MS.
