Introduction

The new era of shoulder arthroplasty is moving away from long stemmed, cemented humeral components to cementless, stemless and metaphyseal fixed implants and to humeral resurfacing. The early clinical results and functional outcome of stemless shoulder arthroplasty is presented.

Methods

A retrospective single-surgeon series of stemless shoulder prostheses implanted from 2011 to 2013 at our institution was evaluated. Perioperative complications, Theatre time and length of hospital stay (LOS) were recorded. Postoperative radiographic and clinical evaluation including measurement of joint mobility, the Oxford Shoulder Score (OSS), and Disabilities of the Arm, Shoulder and Hand (DASH) score by independent evaluators were made.

The aim was to assess the effect of caudal block on patients who have had proximal femoral &/or pelvic osteotomy compared to patients who have had epidural anaesthesia with regards to pain relief and hospital stay.

We looked at two patient cohorts; epidural & caudal pain relief in aforementioned procedures. Interrogation of our clinical database (WinDip, BlueSpeir&clinical notes) identified 57 patients: 33 proximal femoral osteotomy, 13 pelvic osteotomy and 11 combined(25 Males 32 Females), aged 1–18 years-old between 2012–2014, in two institutions. A database of demographics, operative indications, associated procedures, analgesia and type of anaesthesia was constructed in relation to daily pain score and length of hospital stay. 39 patients had epidural anaesthesia, and 18 had caudal block. Cerebral palsy with unstable hips was the commonest indication(21), followed by dysplastic hip(10), Perthes disease(8) and other causes(18). The Face, Legs, Activity, Cry, Consolability(FLACC) scale was used to assess pain.

Length of hospital stay in caudal block patients was 3.1 days(1–9), in epidural anaesthesia patients stay was 4.46 days(2–13). Paediatrics high dependency unit after an epidural was needed in 20(Average stay 3.4 days) compared to 1 who received caudal block. Caudal block FLACC pain score in the first 36 hours was 1.23(0–4) compared to 0.18(0–2) in patients who had an epidural.

Caudal block is associated with less hospital stay and fewer admissions to the high dependency unit, it also provides adequate pain relief post osteotomies when compared to epidural, therefore could be performed at units lacking epidural facilities. A change in related practice however should be cautious and supported by further studies.

Introduction

Tennis and Golfers elbows (TE, GE) are difficult conditions to treat. Ultrasound guided steroid injection (UGSI) is one of the treatment modalities in treating refractory TE and GE. We present our clinical experience and short to mid-term results of patients with TE and GE treated with UGSI.

Methods

Patients with persistent TE and GE treated with UGSI (Marcaine and kenalog) at a tertiary centre between 2007 and 2010 were retrospectively reviewed. Mean follow up was 4 months (Range 2–21 months). Patients were assessed for pain relief, recurrence of symptoms and surgical release.

Introduction: Functional Foot Orthoses (FFO’s) have been shown to improve one element of balance, postural sway, when prescribed for patients with ankle injuries. Little is known, however, about the effect of these devices on ankle stability/proprioception, or the changes which might occur as the patient becomes habituated to using the device. We studied these effects from the time of initial application of the FFO through to regular usage at six weeks.

Methods: Patients with musculoskeletal conditions affecting the lower limb that required custom made FFO’s were evaluated. A standardised protocol, using the Biodex stability system (a balance platform), to assess several stability indices was performed. Patients were assessed before fitting the orthosis, at the time of fitting and six weeks later. The American Orthopaedic Foot and ankle Society (AOFAS) score was also used to evaluate the progress of these patients.

Results: There were 13 male and seven female patients, aged 10 to 64 years. Patients had a range of orthopaedic conditions and all been assessed by orthopaedic specialist and podiatrist as having correctable foot biomechanics. In 6 patients, proprioception deteriorated on initial application of FFO’s. However, all patients exhibited improved over-all stability by a mean of 2.5 points (Normal range 0.82–3.35) at 6 weeks evaluation. The mean AOFAS on presentation was 72 and the final mean score was 97, both of which were clinically and statistically significant (t test, p< 0.05). Eighteen patients had complete resolution of symptoms of pain and instability.

Conclusions: FFO’s alter foot biomechanics, and in doing so appear also to improve balance and proprioception. Proprioception deteriorated in 30% of cases on initial application of orthotics, but pain and instability improved in more than 90% of patients on extended use of foot orthotics, with this improvement becoming manifest by 6 weeks after starting use of the device.

Introduction: Infection with Neisseria Meningitidis remains one of the most devastating illnesses in paediatrics. Affected patients can progress from a mild viral-like illness to septicaemia and death within a matter of hours. We present our clinical experience in identifying and managing the orthopaedic complications associated with meningococcal septicaemia and highlight the long-term problems of physeal growth arrest especially after limb amputation.

Methods: Between August 1997 and June 2005, 88 consecutive children aged from 1 month to 17 years were admitted to the paediatric intensive care unit with meningococcal septicaemia. These patients were retrospectively assessed for orthopaedic manifestations.

Results: During the acute phase of the disease, there were six deaths and 22 patients suffered tissue loss from amputations. Twenty digits were allowed to demarcate and were subsequently amputated. Ten Limb amputations were performed in 6 patients, all of which developed physeal growth arrests proximal to the level of amputation. Four patients had a substantial rise in lower leg compartmental pressures but only two patients underwent fasciotomies, one of which required bilateral below knee amputations.

Conclusions: Meningococcal septicaemia is a potentially lethal paediatric disease. In the acute phase, 22 patients needed orthopaedic input to address complications related to tissue loss, vascular and ischemic problems. Limb amputations due to meningococcal septicaemia will invariably result in physeal damage and our recommendation is that patients should always be screened for this late sequela after index admission. Additionally, early compartment decompression does not appear to improve limb survival.
