Purpose

A Virtual Spinal Clinic (VSC) was set-up at a regional spinal referral centre to see if patient care could be improved through early advice to provide timely management, early onward referral, improve patient satisfaction and minimise chronicity. The clinic was based on the successful virtual model used throughout the country within orthopaedic fracture clinics. VSC is a Consultant led multi-disciplinary (MDT) clinic run by Advanced Practitioners (AP).

Methods

A 3-month trial of the VSC was completed bi-weekly. Patients diagnosed with conservatively managed spinal fractures were referred from the on-call service. A management plan was devised by a Consultant Spinal Surgeon and communicated to patients by the AP via a telephone-call consultation where clinical advice and management could be discussed.

Background

Low back pain (LBP) with or without leg pain, is one of the most common causes of pain and disability and a frequent cause of attendance to emergency departments (ED). Increasing numbers of patients create a difficult challenge for clinicians to effectively and appropriately manage patients with LBP in an urgent care setting.

Purpose

To improve the management of atraumatic spinal pain patients admitted onto the emergency assessment unit (EAU) thus improving quality of care, reducing bed stay and facilitating appropriate discharge and follow-up within an interdisciplinary model.

INTRODUCTION

Total knee arthroplasty (TKA) is typically performed using cement to secure the prosthesis to bone. There are complications associated with cementing that include intra-operative hypotension, third-body abrasive wear, and loosening at the cement interfaces. A cementless prosthesis using a novel keeled trabecular metal tibial baseplate was developed to eliminate the need for cementing the tibial component in TKA.

METHODS

A retrospective chart review was performed on patients who underwent TKA using cementless tibial and femoral components between August, 2013 and January, 2014. Patients with minimum two-year follow-up including radiographs were included in the analysis. Patient demographics as well as preoperative and postoperative range of motion (ROM) and function were measured using the Knee Society Scoring system (KSS). Post-operative radiographs were assessed for signs of osteolysis, loosening, or subsidence. Paired T-tests were used to identify differences in preoperative and postoperative ROM and KSS.

Back ground:

The aim of this study is to evaluate the long-term outcome after posterior spinal stabilization surgery for the management of de novo non-tuberculous bacterial spinal infection.

Method and Result:

Patients presenting to a single tertiary referral spinal centre between August 2011 and June 2014 were included in the study. 21 patients with nontuberculous bacterial infection were identified and included in the study. All patients were managed surgically with posterior stabilisation, with or without neural decompression, without debridement of the infected tissue. Neurological state was assessed using the frankel grading system before and after urgery. Long-term follow-up data was collected using SpineTango COMI questionnaires and Euro Qol EQ-5D system with a mean follow-up duration of 20 months postoperatively.

The mean improvement in neurological deficits was 0.92 Frankel grade (range 0–4). At final followup, at a mean of 20 months, mean COMI score was 4.59, average VAS for back pain was 4.28. These symptoms were having no effect or only minor effect on the work or usual activities in 52%. 38% of patients reported a good quality of life. The average EQ-5D value was 0.569. There were no problems with mobility in 44% of patients. In 72% there were no problems with self-care.

Back ground:

Previous studies have stated that presence of concomitant back pain has a negative effect on the outcome of lumbar decompression/microdiscectomy but none have actually defined what level of back pain should be considered as significant. This is a study of consecutive patients who underwent a primary single level lumbar micro decompression /microdiscectomy performed by thirty nine surgeons at a single tertiary spinal centre between August 2011 and December 2014. The aim was to determine the differential effect of the intensity of back pain and leg pain as a predictor of outcome.

Method and Result:

Data was prospectively collected using SpineTango COMI questionnaires pre-operatively and at 3 months postoperatively. 995 patients who had a complete dataset were included in the analysis. Multivariate regression analysis and ROC curves were used to evaluate factors associated with poor outcome. At 3 months follow up 72.16% of patients were satisfied with the outcome of surgery. The VAS for low back pain was a significant predictor of poor outcome. Of patients with a VAS of 6 or more 34% had a poor outcome following surgery while of patients with a VAS of less than 6, 17% had a poor outcome at three months.

Painful foot and ankle joints are often pointed out as an impeding factor for lack of mobility and weight reduction. There is an assumption that weight loss will occur after their surgery due to increased mobility. The current study aimed to evaluate the effect of surgery on post-operative body mass index (BMI) in patients who underwent mid-foot or hind-foot arthrodesis. Our secondary aim was to look at the effect of sex, pre-operative obesity and good pain relief (AOFAS> 80) on post-operative BMI.

All patients who underwent mid-foot and hind-foot arthrodesis between April 2005 and November 2006 were identified from the operating theatre records. Each patient’s BMI recorded pre-operatively was compared with that recorded at a minimum of 6 months postoperatively using the paired Student’s t-test.

There were 35 eligible patients. 3 patients were excluded because of multiple trauma and 1 patient died during the period of study. We had 31 patients with 33 procedures with a mean age of 61 years (range 41–80). There were 18 females and 13 males. It was found that there was a mean increase of BMI by 0.25 (95%CI of −.95 to.44; p-value=0.47). It was noted that BMI of patients in obese group (BMI> 30) increased post-operatively by 0.07 (95%CI of −1.52 to 1.66; p-value=0.92).

This study highlights the fact that there is no significant effect on BMI in obese patients after successful fusion surgery. The post-operative BMI is neither significantly affected by sex nor quality of pain relief.

Introduction: Obesity has become a major public health epidemic, with recent reports citing that 22% of English men and 24% of women are clinically obese. Painful foot and ankle joints are often pointed out as an impeding factor for lack of mobility and weight reduction. There is an assumption that weight loss will occur after their surgery due to increased mobility.

The current study aimed to evaluate the effect of surgery on post operative body mass index (BMI) in patients who underwent mid-foot or hind-foot arthrodesis.

Patients and Method: All patients who underwent mid-foot and hind-foot arthrodesis under the care of senior author from April 2005 to Nov. 2006 were identified from the operating theatre records. In total 33 procedures were done in 31 patients. Each patient’s BMI recorded pre-operatively was compared with that recorded at a minimum of 6 months postoperatively using the paired Student’s t-test. Analysis of the data was also conducted by stratifying pre-operative BMI, good pain relief (i.e AOFAS> 80), sex and fusion site.

Results: It was found that there was a mean increase of BMI by 0.25 (95% CI of −0.95 to 0.44) with p-value of 0.47.

It was noted that BMI of patients in obese group increased post-operatively by 0.07 (95% confidence interval of −1.52 to 1.66) with p-value of 0.9.

Discussion: This study highlights the fact that there is no significant effect on BMI in obese patients despite significant increase in mobility and pain levels after mid-foot and hind-foot arthrodesis. The change in BMI after fusion surgery is not significantly effected by sex nor quality of pain relief.

There is an increasing awareness of the need to avoid of homologous blood transfusion in elective surgical practice. This stems from a better appreciation of the adverse effects of homologous blood transfusion and increasing pressure on blood stocks because of increasing restrictions on potential donors.

This study examines the effect of using modern blood conservation methods on the subgroup of our patients having surgery for adolescent idiopathic scoliosis. We chose this group because it is a homogenous group of patients of similar age, all of whom had major surgery of a similar severity, and in whom there were few contraindications to our blood conserving strategies.

We studied 78 consecutive patients with adolescent idiopathic scoliosis who underwent surgery. They were divided into two groups. Patients in the study group had one or more modern blood conservation measures used perioperatively. The patients in the comparison group did not have these measures.

There were 46 patients in the study group and 32 in the comparison group. Eight patients who had anterior only surgery, were excluded. The two groups did not differ in age, body weight, and number of levels fused or the type of surgery.

Only 2 patients in the study group were transfused with homologous blood and even these transfusions were off protocol. Wastage of the autologous predonated units was minimal (6/83 units predonated). In contrast all patients in the comparison group were transfused homologous blood. There was significant decrease (p = 0.005) in the estimated blood loss when all the blood conservation methods were employed in the study group.

Using blood conservation measures, lowering the hemoglobin trigger for transfusion and education of the entire team involved in the care of the patient can prevent the need for homologous blood transfusion in patients undergoing surgery for adolescent idiopathic scoliosis.

Background: To assess the safety and efficacy of intrathecal diamorphine for the postoperative pain relief in patients having lumbar decompressive surgery.

Methods: Retrospective study of case records of 39 consecutive patients who underwent lumbar decompression surgery, carried out by the same surgeon. There were 39 (24 males & 15 females, age range 19–75 years) healthy patients (23 ASA 1, 13 ASA 2 and 3 ASA 3). All patients having lumbar microdiscetomy or decompression for spinal stenosis were treated by a single intrathecal injection of diamrphine, performed by the surgeon prior to wound closure. All were prescribed regular paracetamol, and a NSAID if there were no contraindications. Oral opiates were prescribed for “rescue” analgesia. Retrospective analysis of the case notes of these patients was carried out and visual analogue pain scores, sedation scores and side effects/complications were evaluated.

Results: Five patients required rescue analgesia in the first six hours after surgery. None was required subsequenty. Nausea occurred in 2 patients, 1 patient had vomiting and one pruritus requiring piriton. Respiratory depression and sedation were not found in any of the patients. None of the patients had neurological complications.

Conclusion: Intrathecal diamorphine proved to be safe, effective and eliminated the need for opioid infusions following lumbar decompression surgery.
