Introduction & aims

Patient specific instrumentation (PSI) is a useful tool to execute pre-operatively planned surgical cuts and reduce the number of trays in surgery. Debate currently exists around improved accuracy, efficacy and patient outcomes when using PSI cutting guides compared to conventional instruments. Unicompartmental Knee Arthroplasty (UKA) revision to Total Knee Arthroplasty (TKA) represents a complex scenario in which traditional bone landmarks, and patient specific axes that are routinely utilised for component placement may no longer be easily identifiable with either conventional instruments or navigation. PSI guides are uniquely placed to solve this issue by allowing detailed analysis of the patient morphology outside the operating theatre. Here we present a tibia and femur PSI guide for TKA on patients with UKA.

Method

Patients undergoing pre-operative planning received a full leg pass CT scan. Images are then segmented and landmarked to generate a patient specific model of the knee. The surgical cuts are planned according to surgeon preference. PSI guide models are planned to give the desired cut, then 3D printed and provided along with a bone model in surgery. PSI-bone and PSI-UKA contact areas are modified to fit the patient anatomy and allow safe placement and removal.

The PSI-UKA contact area on the tibia is defined across the UKA tibial tray after the insert has been removed. Further contact is planned on the tibial eminence if it can be accurately segmented in the CT and the anterior superior tibia on the contralateral compartment, see example guide in Figure 1. Contact area on the femur is defined on the superior trochlear groove, native condyle, femur centre and femoral UKA component if it can be accurately segmented in the CT.

Surgery was performed with a target of mechanical alignment using OMNI APEX PS implants (Raynham, MA). The guide was planned such that the OMNI cut block could be placed on the securing pins to translate the cut. Component alignment and resections values were calculated by registering the pre-operative bones and component geometries to post-operative CT images.

Introduction

Knee ligament laxity and soft tissue balance are important pre- and intra- operative balancing factors in total knee arthroplasty (TKA). Laxity can be measured pre-operatively from short-leg radiographs using a stress device to apply a reproducible force to the knee, whereas intra-operative laxity is routinely measured using a navigation system in which a variable surgeon-applied force is applied. The relationship between these two methods and TKA outcome however, has not been investigated. This study aims to determine how intra-operative assessments of laxity relate to functional radiographic assessments performed on pre-operatively. We also investigate how laxity relates to short-term patient-reported outcomes.

Method

A prospective consecutive study of 60 knees was performed. Eight weeks prior to surgery, patients had a CT scan and functional radiographs captured using a Telos stress device (Metax, Germany). This device applies a force to the knee joint while bracing the hip and ankle causing either a varus or valgus response.

3D bone models were segmented from the CT scan and landmarked to generate patient specific axes and alignments. Individual bone models were registered to the 2D stressed X-rays in flexion and extension. Reference axes identified on the registered 3D bone models were used to measure the coronal plane laxity. These laxity ranges were compared with those measured by a navigation system (OMNINAV, OMNI Life Science, MA) used during surgery, and Knee Injury and Osteoarthritis Outcome Scores (KOOS) captured 6 months postoperatively.

Aim

This study aimed to evaluate the impact on length of hospital stay from dedicated infectious diseases input for orthopaedic infection patients compared to sporadic infection specialist input.

Method

We conducted an observational cohort study of 157 adults with orthopaedic infections at a teaching hospital in the UK. The orthopaedic infections included were: osteomyelitis, septic arthritis, infected metalwork and prosthetic joint infections, and adults were aged 18 years or more. Prior to August 2016, advice on orthopaedic infection patients was adhoc with input principally from the on-call infectious diseases registrar and phone calls to microbiology whereas after August 2016 these patients received regular input from dedicated infectious diseases doctor(s). The dedicated input involved bedside reviews, medical management, correct antimicrobial prescribing, managing adverse drug reactions, increased use of outpatient parenteral antimicrobial therapy (OPAT) services especially self-administration of intravenous antibiotics and shared decision-making for treatment failure, whilst remaining under orthopaedic team care. Orthopaedic patients operated on for management of their infection between 29/8/16 and 15/3/17 were prospectively identified and orthopaedic operation records were used to retrospectively identified patients between 29/8/15 and 15/3/16. The length of stay was compared between the 2 groups.

Aim

To demonstrate the use of indium-111 white-cell labelled SPECT CT (In111-WC-SPECT-CT) in bone infection.

Method

This novel imaging modality is useful in bone infection. We present three cases of complex osteomyelitis to illustrate this. All were imaged with conventional modalities, but conclusive diagnosis could not be achieved. In111-WC-SPECT-CT was used to provide the definitive imaging that allowed successful treatment.

The Oxford Hip Score (OHS), the Harris Hip Score (HHS) and WOMAC are examples of patient reported outcome measures (PROMs) have well documented ceiling effects, with many patients clustered close to full marks following arthroplasty. Any arthroplasty that offers superior function would therefore fail to be detectable using these metrics. Two recent well conducted randomised clinical trials made exactly this error, by using OHS and WOMAC to detect a differences in outcome between hip resurfacing and hip arthroplasty despite published data already showing in single arm studies that these two procedures score close to full marks using both PROMS.

We had observed that patients with hip resurfacing arthroplasty (HRA) were able to walk faster and with more normal stride length than patients with well performing hip replacements, but that these objective differences in gait were not captured by PROMs. In an attempt to capture these differences, we developed a patient centred outcome measure (PCOM) using a method developed by Philip Noble's group. This allows patients to select the functions that matter to them personally against which the success of their own operation will be measured.

Our null hypothesis was that this PCOM would be no more successful than the OHS in discriminating between types of hip arthroplasty.

22 patients with a well performing Hip Resurfacing Arthroplasty were identified. These were closely matched by age, sex, BMI, height, preop diagnosis with 22 patients with a well performing conventional THA. Both were compared with healthy controls using the novel PCOM and in a gait lab.

Results

PROMs for the two groups were similar, while HRA scored higher in the PCOM. The 9% difference was significant (p<0.05).

At top walking speed, HRA were 10% faster, with a 9% longer stride length.

Discussion

Outcome measures should be able to detect differences that are clinically relevant to patients and their surgeons. The currently used hip scores are not capable of delivering this distinction, and assume that most hip replacements are effectively perfect. While the function of hip replacements is indeed very good, with satisfaction rates high, objective measures of function are essential for innovators who are trying to deliver improved functional outcome.

The 9% difference in PCOM found in this small study reflects the higher activity levels reported by many, and of similar magnitude to the 10% difference in top walking speed, despite no detectable difference in conventional PROMS. PCOMs may offer further insight into differences in function. For investigators who wish to develop improvements to hip arthroplasty, PCOMs and objective measures of gait may describe differences that matter more to patients than conventional hip scores.

Aim

A case series with functional and radiographic outcomes, of modular endoprosthetic distal femoral replacement in complex cases of periprosthetic fracture.

Method

Sixteen cases were identified of endoprosthetic replacement (EPR) from the bone and soft tissue cancer implant registry. A retrospective review was undertaken.

Although originally designed to aid the management of primary malignant bone tumours, the indications for modular endoprosthetic replacement (EPR) have expanded to include complex periprosthetic fractures and failed internal fixation. The incidence of these challenging cases is increasing with an aged population. We reviewed retrospectively our experience with the use of EPR in patients who had undergone limb salvage following complex trauma presentations.

Between 2003 and 2008 twenty one patients underwent EPR following referral to the Oxford Sarcoma Service following lower limb trauma. The average age was 71 years (44–87). The average number of previous surgical procedures was 3 (range 0–11).

The mean Harris Hip Score was 89.5 (range 64–85). The mean American Knee Society Score was 82 (range 62–100) and the mean functional score was 62 (range 30–75). Complications included two cases of deep infection; one resulted in a two stage revision procedure, while the other retained the EPR following a washout. EPR is an effective salvage procedure for failed trauma fixation and periprosthetic fractures. Immediate weight bearing and a good functional outcome can be expected in this difficult group of patients.
