Introduction

A proportion of patients with hip and knee prosthetic joint infection (PJI) undergo multiple revisions with the aim of eradicating infection and improving quality of life. The aim of this study was to describe the microbiology cultured from multiply revised hip and knee replacement procedures to guide antimicrobial therapy at the time of surgery.

Patients and Methods

Consecutive patients were retrospectively identified from databases at two specialist orthopaedic centres in the United Kingdom between 2011 and 2019. Patient were included who had undergone repeat revision total knee replacement (TKR) or total hip replacement (THR) for infection, following an initial failed revision for infection.

Aims

The COVID-19 pandemic posed significant challenges to healthcare systems across the globe in 2020. There were concerns surrounding early reports of increased mortality among patients undergoing emergency or non-urgent surgery. We report the morbidity and mortality in patients who underwent arthroplasty procedures during the UK first stage of the pandemic.

Methods

Institutional review board approval was obtained for a review of prospectively collected data on consecutive patients who underwent arthroplasty procedures between March and May 2020 at a specialist orthopaedic centre in the UK. Data included diagnoses, comorbidities, BMI, American Society of Anesthesiologists grade, length of stay, and complications. The primary outcome was 30-day mortality and secondary outcomes were prevalence of SARS-CoV-2 infection, medical and surgical complications, and readmission within 30 days of discharge. The data collated were compared with series from the preceding three months.

Aim

Compare clinical outcomes following staged revision arthroplasty for periprosthetic joint infection (PJI) secondary to either multidrug resistant (MDR) bacteria or non-MDR (NMDR) bacteria.

Method

Retrospective analysis of a prospectively collected bone infection database. Adult patients diagnosed and treated for hip or knee PJI, between January 2011 and December 2014, with minimum one-year follow-up, were included in the study. Patients were divided into two groups: MDR group (defined as resistance to 3 or more classes) and N-MDR group (defined as acquired resistance to two classes of antibiotic or less).

The Charlson Comorbidity Index was used to stratify patients into low, medium and high risk.

The diagnosis of PJI, and any recurrence following treatment, was made in accordance with the Musculoskeletal Infection Society criteria. Failure was defined as recurrence of infection necessitating implant removal, excision arthroplasty, arthrodesis or amputation.

Aim

Periprosthetic joint infections (PJI) are rare and require complex multi-disciplinary management. Successful single and two-stage revision procedures have been described. We describe the clinical features of this rare diagnosis from a single institution.

Method

Patients were identified retrospectively from a prospectively collected institutional infection database. Clinical notes were evaluated for demographic, comorbid and clinical outcomes. The diagnosis of PJI, and any recurrence following treatment, was made in accordance with the Musculoskeletal Infection Society criteria. Failure was defined as recurrence of infection necessitating implant removal, excision arthroplasty or amputation.

Introduction

Patients who present with atypical foot pain in a non specific sensory distribution may benefit from having nerve conduction studies (NCS). The aim of this study was to confirm whether NCS is a useful tool.

Methods

Between July 2005 and March 2011, 78 patients had NCS to investigate foot pain. The management following NCS was compared with the initial management plan.

Introduction

Up to 16% of scaphoid fractures are radiologically occult; failure to diagnose scaphoid fractures may lead to delayed union, nonunion or avascular necrosis. Fractures may take weeks to be excluded and many patients are unnecessarily immobilised increasing work absence, clinical reviews and cost. The use of CT early in the management of suspected occult scaphoid fractures has been evaluated.

Methods

The radiology and clinical notes of all patients that had scaphoid CT scans over the preceding 3 years were retrospectively reviewed. 84 patients that had CT scans within 14 days from injury were identified.

The percutanous repair of the Achilles tendon is a cost efficient method of restoring early limb function and may offer reduced risk of re-rupture and wound infection. This technique has been described in the elderly population and elite athletes; we present an evaluation of this technique in a District General Hospital setting. We have prospectively audited the outcome of 56 patients who have elected to have percutaneous repair for Achilles tendon rupture from 2009–2011. The majority were males (44) with mean age of 46 years (range 27–80). Twenty nine patients ruptured the right tendon and 27 the left. 82% (46) of injuries were sustained whilst exercising: e.g. football (22), badminton (7), running (5). All but 2 patients were managed on a Day Case basis and 4 requested general anaesthesia. Patients were immediately weight bearing in a brace following surgery and commenced physiotherapy at 2 weeks.

Functional outcome was measured using a modified Achilles Tendon Rupture Score (ATRS) at 3, 6, 9 and 12 months: 100 score equals maximal limitation. The mean ATRS scores a 3, 6, 9 and 12 months were 53 (7–82), 31 (0–74), 30 (0–67) and 15 (1–52) respectively. We have had 4 complications: 2 sural nerve injuries, 1 poor wound healing and 1 re-rupture at 8 weeks.

Overall complication rate was 7.1%, comparable to other studies. We have shown a good outcome following percutaneous Achilles tendon repair. The majority of patients show good restoration of function by 3 months and a minor limitation at 6 months. The majority of the improvement in function occurred between 3 and 6 months following surgery. Two patients reported ongoing tendinopathic pain following repair increasing mean scores.

We believe this technique can be introduced in District General Hospitals to give good outcome on a cost effective basis.
