The Diagnosis and Prophylaxis of Venous Thromboembolic Disease in Our Hospital

International Society for Technology in Arthroplasty (ISTA) 2012 Annual Congress



Abstract

Purpose

The frequency of venous thrombo-embolism (VTE) after total hip arthroplasty(THA) is 20–30% and it is serious complication under THA. Therefore it is necessary to detect and prevent VTE. The purpose of this study were examined the frequency of VTE and the factor of incidence of VTE in our hospital.

Patients and methods

The 615 patients(82 men and 533 women) who performed primary THA from Jan. 2006 to Apr. 2011 were examined in this study. The Average age at the operation was 65 years (rage, 20–92 years). MDCT examination was performed the day after operation to detect VTE. 95 patients(15.4%) were positive of VTE and the rest of them were negative. We examined the age at operation, body mass index(BMI), blood loss, operative time, blood soluble fibrin monomer complex(SFMC) in the positive and negative group of VTE. The distance from the tibial joint line to the level of DVT was measured.

Results

Average age of positive group was 67 years and negative group was 64 years. Positive group was older than negative group. BMI, blood loss, operative time was no significant differences. The average SFMC (positive group of DVT) were 24±26μg/ml, versus 14±20μg/ml for the negative group. The mean distance from the tibial joint line to the level of DVT was distally 128.9 mm.

Discussion and conclusion

In this study, the frequency of VTE is about 15% and it is lower frequency than previous reports. The amount of SFMC in positive group was significant higher than VTE positive group, SFMC examination was useful to evaluate DVT blood marker.

Early detection can be grossly by MDCT examination is useful for the diagnosis VTE.

But in the future, we measure significant difference, the amount of SFMC the day after operation, or should be treated VTE.

In addition, the position of DVT and it was from the tibia average is 128.9 mm, that the prevention of DVT incidence by massaging the same site during surgery.
