Abstract

  1. Preoperative templating essential

  2. Make the right skin incision: most lateral, leave appropriate skin bridge from older incisions, be prepared for plastic surgical consultation for skin expanders or flaps

  3. Release scarring in the medial and lateral gutters

  4. Elevate a proper medial capsular sleeve

  5. Release scarring between patellar ligament and front of tibial plateau

  6. Pin the patellar tendon

  7. Perform a quadriceps release OR a tibial tubercle osteotomy in the very stiff knee

  8. Extract the prior components in an axial direction

  9. Use intramedullary guides to align the cuts

  10. Localise the joint line position by the level of the patellar tendon
